Abstract

FIELD: medicine.

SUBSTANCE: lateral release of the ankle joint is performed through an incision along an outer surface of the ankle joint. A tendon of the long head of the fibular muscle is isolated. That is followed by a capsulotomy of the ankle joint. The tarsal fusion is isolated from the performed incision. A modelling U-osteotomy of the ankle bone is created within the fusion. The foot is brought into the middle position along the osteotomy and fixed with K-wires.

EFFECT: method eliminates the foot deformity, making it supporting and applicable for prosthetic repair.

Для устранения эквино-плоско-вальгусной деформации стопы при продольной эктромелии голени и наличии тарзальной коалиции 1 (рис.1), первым этапом выполняют: разрез по задней поверхности голени в проекции ахиллова сухожилия; задний релиз голеностопного сустава 2; Z-образное удлинение ахиллова сухожилия с отсечением наружной порции у места прикрепления к пяточной кости; Z-образное удлинение сухожилия мышцы длинного сгибателя большого пальца и капсулотомия голеностопного сустава (рис.2). Вторым этапом из разреза по наружной поверхности голеностопного сустава 2 выполняют: латеральный релиз голеностопного сустава; выделяют сухожилие длинной головки малоберцовой мышцы; осуществляют капсулотомию голеностопного сустава 2 (рис.3). Из разреза по наружной поверхности голеностопного сустава 2 выделяют зону тарзальной коалиции 1, выполняют моделирующую П-образную остеотомию таранной кости в зоне коалиции 3, на линии остеотомии. Стопу выводят в среднее положение и фиксируют спицами Киршнера (рис.4, 5).
