Definitions
- the invention relates to medicine, namely to dermatology and cosmetology, and can be used to treat atrophic skin scars that have arisen due to various reasons.
- a known method of eliminating scars (RU 2030923 C1, 03/20/1995), which consists in exposing the scar tissue to laser radiation.
- Laser exposure is carried out by a focused beam of a holmium laser with a wavelength of 1.96 - 2.15 ⁇ m in a pulse-periodic mode with a pulse duration of 200 - 500 ⁇ s, an energy of 0.1 - 2.0 J.
- Its disadvantage is that it is very difficult to dose the penetration depth laser beam, subsequently, a demarcation line remains at the border of the treated and untreated sections, in addition, the implementation of this method requires the use of expensive equipment.
- a known method of treating hypotrophic skin scars which also includes the use of laser radiation (RU 2323021 C1, 04/27/2008). Scar tissue is subjected to a single exposure to laser radiation with a wavelength of 540 nm, a pulse energy of 50-150 mJ, a pulse repetition rate of 1-4 Hz, a pulse duration of 1-2 ns. This method also requires the use of expensive equipment.
- a known method of treating normo-atrophic scars with mechanical dermabrasion using a combined collagen coating with an antimicrobial agent (RU 2230554 C2, 06/20/2004).
- a complex of preoperative measures and dermabrasion is carried out, while the area is exposed on an outpatient basis in sterile conditions of a small operating room operations, the skin surface is treated twice with 10 ml butol using a gauze cloth, capturing more than 2-3 cm of the area of the operating area, and anesthesia is performed topically with lidocaine, then dermabrasion is performed using a rotary apparatus and the skin surfaces are applied, applying a gauze cloth moistened pre-butol, the surgical wound is treated with butol twice 10-15 ml from a plastic syringe without a needle, and 3-5 minutes after the patient has a burning sensation, the gauze is removed with a forceps, after dermabrasion on the operating surface impose a collagen coating that exceeds the operating area by 1-2 cm, and the surgical wound is treated twice without interruption
- the objective of the invention is to provide a simpler highly effective method for the treatment of atrophic scars, which allows to obtain a pronounced effect in a short time.
- the technical result is to reduce the complexity and cost, as well as to shorten the treatment time and improve functional results.
- the technical result is achieved by a method of treating atrophic skin scars using fully straightened filtered high-frequency radio waves and autologous platelet-rich plasma, characterized in that from the patient’s peripheral vein take blood in vacuum tubes, mix blood with an anticoagulant, centrifuge the blood mixture with an anticoagulant for 4-6 minutes with a centrifuge speed of 700-900 rpm, remove the upper layer of platelet-rich plasma from a tube into a separate tube, activate platelet-rich plasma plasma with calcium ions by adding calcium gluconate and mixing with it, activated platelet-rich plasma is collected in a syringe and injected intradermally into the scar, the injected is heated region using the Surgitron ® radio wave apparatus with Pelleve ® GlideSafe or PelleFirm electrodes up to 41-43 ° C for at least 4 minutes.
- blood is collected from a patient’s peripheral vein into 3-9 ml vacuum tubes; 3.8% sodium citrate solution is used as an anticoagulant; to mix blood with sodium citrate, the tubes are inverted 4-5 times; the upper layer of the resulting platelet-rich plasma is removed into a separate tube using a laboratory dispenser; calcium gluconate in platelet rich plasma is added at the rate of 50 ⁇ l of calcium gluconate per 1 ml of plasma; for injection use insulin syringes with a volume of 1 ml; 0.1-0.2 ml of platelet-rich plasma per 1 cm of the rumen is injected into the rumen; heating is carried out using a radio wave apparatus
- the proposed method is as follows. From the patient's peripheral vein, blood is drawn into vacuum tubes of 3–9 ml in volume (depending on the required plasma volume). Blood is mixed with an anticoagulant, which is used as a 3.8% solution of sodium citrate. To mix blood with citrate, the tubes are inverted 4-5 times.
- the blood in the test tubes is divided into three layers: the lower is red blood cells, the middle is a thin layer of white blood cells, and the upper is plasma enriched with platelets.
- platelet-rich plasma (BTP) is removed into a separate tube. After this, platelet-rich plasma (BTP) is activated with calcium ions: 50 ⁇ l of calcium gluconate per 1 ml of plasma.
- BoTP BoTP is ready for use and should be used within 5-10 minutes.
- the plasma volume required for the procedure depends on the size of the skin defect.
- BTP All obtained BTP are collected in 1 ml insulin syringes. BOTP is injected intradermally into the scar at 0.1-0.2 ml per cm.
- the procedure is carried out 1 time per week 5-7 times.
- the method was applied in clinical conditions in 22 patients with atrophic skin scars aged 20 to 50 years. All patients were female. 12 patients with post-acne without signs of inflammation (atrophic scars resulting from acne). 10 patients with striae (atrophic scars). The disease is over 2 years old. The procedure was performed 1 time per week.
- Example 1 Patient T., 37 years old.
- the patient was treated by the proposed method. 12 ml of blood were taken from peripheral blood (in 4 vacuum tubes of 3 ml each). 3.8% sodium citrate solution was used as an anticoagulant. Further, the blood was centrifuged for 5 min with a centrifuge rotation speed of 800 rpm. During rotation, the blood in the test tubes was divided into three layers: the lower — red blood cells, the middle — a thin layer of leukocytes, and the upper — plasma enriched with platelets. Then, using the laboratory dispenser, the upper layer (BOT) was removed into a separate tube.
- BOTP was activated with calcium ions: 50 ⁇ l of calcium gluconate per 1 ml of plasma. The liquid was mixed by carefully turning the tubes. Received 5 ml of BOTP. All obtained BTPs were collected in 1 ml insulin syringes. BOTP was injected intradermally into the scar at 0.1-0.2 ml per cm. After BOTP injection into the scar, this area was heated using a Surgitron S5 radio wave apparatus with a PelleFirm TM electrode to 42 ° C for 5 minutes (the temperature was measured with an infrared pyrometer on the skin above the scar) to resume regeneration processes. The procedure was carried out 1 time per week 5 times. After 6 months there was a complete regression of striae. By the end of 2 months, a significant densification of scar tissue and alignment of the relief with the surrounding skin were noted. No adverse effects have been reported.
- platelets due to short-term centrifugation for 4-6 minutes, platelets do not have time to sink to the bottom of the tube and mix with the layer of leukocytes and red blood cells, but remain suspended in the upper part of the tube, evenly distributed in the plasma, which does not require separation of the plasma into layers or repeated centrifugation.
- the short time required for the manufacture of platelet-rich plasma, and the simplicity of its preparation make it possible to use this technique in routine medical practice in institutions of any profile.
