Sistema para esterilizar un instrumento electroquirúrgico.
ANTECEDENTES
Campo técnico La siguiente descripción se refiere a un aparato y un método para la esterilización de instrumentos electroquirúrgicos y, más particularmente, a un aparato y un método para la esterilización de instrumentos laparoscópicos.
Descripción de la técnica relacionada La cirugía laparoscópica requiere por lo general la introducción de un instrumento laparoscópico en una cavidad del cuerpo. Con anterioridad a la operación laparoscópica, se deben esterilizar el instrumento laparoscópico y los componentes asociados al mismo.
Los métodos comúnmente empleados para esterilizar instrumentos laparoscópicos incluyen la pasteurización, que requiere calentamiento, los métodos químicos tradicionales, tales como los métodos de cámara, que requieren la inundación de una cámara con un esterilizante, por lo general una mezcla de óxido de etileno (comúnmente denominado EtO) y otros gases , y métodos de microdosis, que requieren la introducción de un esterilizante, tal como óxido de etileno, en un dispositivo especialmente diseñado para ello.
La pasteurización puede ser un método eficaz para la esterilización de algunos, pero no todos, los instrumentos laparoscópicos; esto se debe a que el calor aplicado durante la pasteurización puede causar daño a algunos, si no a todos, los materiales sensibles al calor situados en o unidos al instrumento laparoscópico.
Los métodos químicos tradicionales de esterilización (por ejemplo, los métodos de la cámara) pueden tener inconvenientes inherentes a la utilización de grandes cantidades de esterilizante que son liberadas en un espacio grande, algunos de los cuales pueden incluir el aumento del costo, aumento del tiempo de producción y pueden requerir el tratamiento de mayores cantidades de tóxico. Aunque los métodos de microdosis reducen algunos de los inconvenientes asociados con los métodos de la cámara de esterilización, los métodos de esterilización con microdosis tienen también inconvenientes. Por ejemplo, el método de esterilización con microdosis es adecuado cuando una pequeña cantidad de instrumentos deben ser esterilizados.
Se encamina al solicitante al reconocimiento de la existencia del documento US-A-4016881 que divulga un instrumento laparoscópico con un vástago que muestra ranuras longitudinales para alojar los electrodos. Se contempla esterilizar el instrumento con un gas tal como EtO. El estado de la técnica acorde con el documento DEA-4403252 se refiere a un endoscopio que tiene una funda exterior amovible para su vástago, con agujeros provistos en dicho vástago.
SUMARIO DE LA INVENCIÓN La invención está definida en las reivindicaciones independientes que siguen. Las reivindicaciones dependientes están dirigidas a características opcionales y formas de realización preferentes.
En consecuencia, la presente descripción se refiere a un aparato para uso en la esterilización de un instrumento quirúrgico. El instrumento quirúrgico incluye un alojamiento que tiene un vástago que se extiende desde el mismo. El vástago incluye una o más ranuras definidas en el mismo que se extienden, al menos parcialmente, a lo largo del mismo. En una forma de realización, el vástago incluye una pluralidad de ranuras que se extienden a todo lo largo del mismo, dispuestas en una relación espacial fija la una respecto a la otra. Por ejemplo, la pluralidad de ranuras pueden estar dispuestas en una relación entre ellas generalmente ortogonal. En una forma de realización, la pluralidad de ranuras pueden tener una profundidad de aproximadamente 50 μm (0,002'') y una anchura de aproximadamente 100 μm (0,004''). La ranura o ranuras proporcionan un camino para un esterilizante y está configurado para permitir que un paso de esterilizante a su través introduzca esterilizante en el alojamiento. En una forma de realización, el esterilizante incluye óxido de etileno.
El aparato incluye también una camisa o recubrimiento que encierra el vástago. El vástago incluye una o más ranuras definidas en el mismo que se extienden, al menos parcialmente, a lo largo de la longitud del mismo. El método incluye la etapa de encerrar el vástago con una envoltura retráctil. El método incluye también la etapa de introducir un esterilizante en la o las ranuras.
El método puede incluir además las etapas de introducir un esterilizante en un aparato de esterilización y someter envoltura retráctil a una etapa final de contracción, en la que una vez completada la etapa final de contracción la envoltura retráctil forma una junta hermética contra el vástago.

DESCRIPCIÓN DETALLADA En este documento se describen varias formas de realización detalladas de la presente divulgación, sin embargo, las realizaciones descritas son meramente ejemplos de la divulgación, los cuales pueden realizarse de diversas formas. Por lo tanto, los detalles específicos estructurales y funcionales descritos aquí no se han de interpretar como una limitación, sino meramente como una base para las reivindicaciones y como una base representativa que permita a un experto en la materia realizar la invención reivindicada.
Como se ha mencionado anteriormente, los instrumentos laparoscópicos, al igual que la mayoría de los instrumentos quirúrgicos, requieren la esterilización antes de que puedan ser introducidos en una zona quirúrgica. Los métodos tradicionales de cámara requieren la colocación de un instrumento laparoscópico en una cámara e inundar la cámara con un esterilizante, el cual puede incluir una mezcla de óxido de etileno (en lo sucesivo EtO) con otros gases. Durante el proceso de esterilización el EtO y otros gases entran en el instrumento laparoscópico y esterilizan zonas objetivo y / o componentes, por ejemplo, aquellos componentes internamente situados dentro del mango del instrumento laparoscópico (véase la FIGURA 5A). Sin embargo, debido al hecho de que el extremo distal del vástago, que se conecta a un actuador final del instrumento laparoscópico, puede incluir una junta, un material termorretráctil, cinta adhesiva, caucho u otra funda aislante o de silicona, el esterilizante puede verse impedido a entrar en el instrumento laparoscópico en el momento oportuno. El vástago de la presente divulgación, debido a la configuración única de ranuras única y túneles definida en el mismo, proporciona rutas adicionales y / o alternativas para el esterilizante, facilitando así al esterilizante que llegue a la zona objetivo.
La presente divulgación se refiere a la esterilización de instrumentos laparoscópicos. Como se conoce en el estado de la técnica, los instrumentos laparoscópicos generalmente incluyen un conjunto de mango, un vástago, y un conjunto de actuador final. Convencionalmente, el vástago es un tubo largo y estrecho generalmente un tubo cilíndrico que tiene superficies exterior e interior, que aloja los componentes mecánicos y eléctricos que permiten que el mango y el actuador final funcionen como se pretende. La presente divulgación incluye un vástago que puede incluir una o más ranuras que se extiendan a lo largo del mismo (o parcialmente a lo largo de la longitud del mismo), que se explican con mayor detalle más adelante. La o las ranuras están configuradas para proporcionar una
ruta para un esterilizante. Adicionalmente, puede haber una envoltura retráctil envolviendo al vástago, que puede permitir que el esterilizante se desplace a lo largo de las ranuras. Tener un vástago configurado de tal manera facilita el procedimiento de esterilización de instrumentos laparoscópicos.
Dado que la presente divulgación se refiere a la esterilización, y no al uso, de los instrumentos laparoscópicos, una descripción detallada en profundidad de las características de funcionamiento de los instrumentos laparoscópicos no es vital para el entendimiento de la presente invención. A fin de que expertos en la materia puedan apreciar el aparato de esterilización y el método, tal y como se describe en la presente memoria, sólo se expone a continuación una breve descripción de dos instrumentos laparoscópicos.
Volviendo ahora a la FIGURA 1, se muestra una forma de realización de un instrumento laparoscópico 10. Para el resto de la memoria se supondrá que el instrumento laparoscópico es unas pinzas bipolares; teniendo en cuenta que cualquier instrumento laparoscópico que incluye un vástago puede ser empleado con la presente invención. Se muestra unas pinzas bipolares 10 para su uso con diversas operaciones electroquirúrgicas y que generalmente incluyen un alojamiento 20, un conjunto de asidero o mango 30, un conjunto giratorio 80, un conjunto disparador o gatillo 70 y un conjunto actuador extremo100 que cooperan mutuamente para agarrar, obturar y dividir gruesos vasos tubulares y grandes tejidos vasculares. Aunque la mayoría de los dibujos de las figuras representan unas pinzas bipolares 10 para uso en relación con operaciones quirúrgicas endoscópicas, la presente invención se puede utilizar para operaciones quirúrgicas abiertas más tradicionales. Para los fines de la presente invención, las pinzas 10 se describen en términos de un instrumento endoscópico; sin embargo, una versión abierta de las pinzas puede incluir también los mismos o similares componentes operativos y características que se describen a continuación.
Las pinzas 10 incluyen un vástago 12, que se describirá en mayor detalle más adelante, que tiene un extremo distal 16 configurado para acoplarse mecánicamente al conjunto actuador extremo 100 y un extremo proximal 14 que se acopla mecánicamente al alojamiento 20. En los dibujos y en las descripciones que siguen, el término "proximal", como es tradicional, se refieren al extremo de las pinzas 10 que está más cerca del usuario, mientras que el término "distal" se refiere al extremo que está más lejos del usuario.
Como se ve mejor en la figura 3, las pinzas 10 también incluyen un cable de electroquirúrgico 310 que conecta las pinzas 10 a una fuente de energía electroquirúrgica, por ejemplo, un generador 500 (mostrado esquemáticamente). Se contempla que generadores, tales como los vendidos por Valleylab - una división de Tyco Healthcare LP, ubicada en Boulder Colorado, pueden ser utilizados como una fuente de energía electroquirúrgica, por ejemplo, el Generador Ligasure®, Generador electroquirúrgico, FORCE EZ® , el Generador electroquirúrgico FORCE FX®, FORCE 1C®, Generador FORCE 2®, SurgiStat® II u otros generadores adecuados que pueden desempeñar funciones diferentes o mejoradas.
El cable 310 está dividido internamente en pociones de cable 310a, 310b y 325b que están diseñados para transmitir potenciales eléctricos a través de sus respectivas vías de alimentación a través de las pinzas 10 al conjunto de actuador efector 100. Más concretamente, el cable de alimentación 325b se conecta a través del alojamiento 20 de las pinzas y a través del conjunto giratorio hasta miembro de mordaza 120.
El conductor 310a se conecta a un lado del interruptor 60 y el conductor 310c se conecta al lado opuesto del interruptor 60 de tal manera que tras la activación del interruptor la energía se transmite desde el conductor 310a al conductor 310c. El conductor 310c se empalma con el conductor 310b que se conecta a través del conjunto giratorio al miembro de mordaza 110. Los conductores 310a-310c son sólo un ejemplo de los diversos componentes internos que necesitan ser esterilizados antes de su uso.
El conjunto de asidero 30 incluye una mango fijo 50 y un mango móvil 40. El mango fijo 50 está asociado integralmente con el alojamiento 20 y el mango 40 es móvil con relación al mango fijo 50. El mango fijo 50 puede incluir uno o más elementos con mejoras ergonómicas para facilitar la manipulación, por ejemplo, vainas, protuberancias, material elastómero, etc.
El conjunto giratorio 80 está operativamente asociado con el alojamiento 20 y es giratorio aproximadamente 180 grados alrededor de un eje longitudinal "AA" (Véase la figura 1).
Tal y como se mencionó anteriormente, un conjunto de actuador extremo100 está unido al extremo distal 14 del vástago 12 e incluye un par de elementos de mordaza opuestos 110 y 120. El mango movible 40 del conjunto de asidero 30 está finalmente conectado a un conjunto de accionamiento 130 que, juntos, cooperan mecánicamente para impartir movimiento a los elementos de mordaza 110 y 120 desde una posición abierta, en la que los elementos de mordaza 110 y 120 están dispuestos en relación espaciada el uno con respecto al otro, a una posición cerrada o de apriete, en la que los elementos de mordaza 110 y 120 cooperan para atrapar el tejido entre ellos.
Respecto a la figura 4, se muestra otra forma de realización de un instrumento laparoscópico 10' que se pueden emplear con la presente invención. El instrumento laparoscópico 10' incluye también un mango 50', un vástago 12' y un conjunto de actuador 100'. El vástago 12' de las pinzas 10' incluye una serie de ranuras 200` definidas a lo largo
del mismo y cuya área está configurada para llevar un esterilizante a los diversos componentes operativos internos del mismo. Se hace referencia a la solicitud de patente de Estados Unidos Nº 11/595.194, publicada como US-A-7 766910 el y 11/540.335 (US-A-2007/0078456) para una explicación más detallada del funcionamiento de ambas pinzas 10 y 10 ', respectivamente.
Para el resto de la memoria, y para los fines de brevedad, el aparato y método para la esterilización de un instrumento laparoscópico se describirán con mayor detalle haciendo referencia instrumento laparoscópico 10.
Con referencia a las figuras 1, 2, 4, 5A, 5B y 6A, se ilustran ranuras 200 del vástago 12 en líneas de trazos. Las ranuras 200 pueden ser moldeadas como parte del vástago 12 durante el proceso de fabricación. En una forma de realización alternativa, se puede mecanizar al menos una ranura 200 al tiempo después de la fabricación del vástago 12.
De forma similar a los vástagos convencionales del estado de la técnica, el vástago 12 puede estar definido por las superficies interior y exterior, 204 y 206, respectivamente, que se muestran en la figura 6B. En una forma de realización, la ranura 200 puede estar situada sobre la superficie exterior 206 del vástago 12. La ranura 200 del vástago 12 puede incluir una o más aberturas o rendijas 208 o cualquier combinación de las mismas, extendiéndose desde la ranura 200 hasta la superficie interior 204 del vástago 12, tal y como se ilustra en las figuras 6A y 6B. 10. Las aberturas o rendijas 208 puede estar dispuestas en cualquier lugar dentro de la ranura 200 y / o en el vástago 12 del instrumento laparoscópico 10. La forma y dimensiones de las aberturas o rendijas 208 dependerán de las necesidades de un usuario.
Al menos una ranura 200 puede extenderse desde el extremo distal 16 al extremos proximal 14 del vástago 12, como se muestra en la Figura, 1. Tener una ranura 200 configurada de esta manera permitirá al esterilizante 210 trasladarse desde el extremos distal 16 dentro de la ranura 200 hasta el extremo proximal 14, como se muestra en al figura 6A y se indica con la flecha, en la que esterilizante 210 puede llegar a un zona objetivo, por ejemplo, los componentes internos del alojamiento 20.
En una forma de realización alternativa, ilustrada en la figura 6B, la ranura 200 puede extenderse desde el extremo distal 16 del vástago 12 una distancia menor que la longitud del vástago 12 a una o más aberturas o rendijas 208 situadas dentro de la ranura 200 del vástago 12. Tener un vástago configurado de esta manera puede permitir al agente esterilizante 210 trasladarse dentro de la ranura 200 a través de la abertura 208 hacia dentro del vástago 12, donde el agente esterilizante 210 puede alcanzar una zona objetivo especifica.
En referencia a las figuras 2, 7, 8A-8B, el vástago 12 puede estar configurado para incluir una pluralidad de ranuras
200. Por ejemplo, el vástago 12 puede incluir cuatro ranuras 200a-200d configuradas para extenderse a lo largo la longitud del vástago 12. Al proporcionar cuatro (o más) ranuras 200a-200d, más esterilizante puede trasladarse a lo largo del vástago 12 en menos tiempo, lo que puede ser beneficioso de cara a la fabricación.
Las cuatro ranuras 200a-200d pueden estar dispuestas en una relación de separación fija una con respecto a la otra en el vástago 12. En una forma de realización, las cuatro ranuras 200a-200d pueden estar separadas a intervalos aproximadamente de 90º, como se muestra en las figuras 8A y 8B. Se prevé que las cuatro ranuras 200a-200d puedan estar dispuestas en otras relaciones de separación fijas que dependan de un determinado propósito. Por ejemplo, las ranuras 200a y 200b pueden estar dispuestas formando un ángulo X una respecto a otra, mientras que las otras dos ranuras 200c y 200d pueden estar dispuestas en un ángulo Y una con la otra. La manera en que las cuatro ranuras 200a-200d pueden colocarse, la una con respecto la otra, puede depender de unas pinzas en particular 10.
Las ranuras 200a-200d puede tener una sección transversal generalmente arqueada, como se ve en la figura 8A o una forma cuadrada, como se ilustra en la figura 8B. También se pueden emplear otras formas adecuadas conocidas en el estado la técnica, incluyendo, pero sin limitación a ellas, rectangulares, triangulares y similares con las ranuras 200a-200d de la presente divulgación. Se prevé que las ranuras 200 puedan tener cada una su propia forma única.
Siguiendo con la referencia a las figuras 8A y 8B, las ranuras 200 pueden tener una profundidad de aproximadamente 50 μm (0,002'') y una anchura de aproximadamente 100 μm (0,004''). Otras formas de realización pueden incluir ranuras 200a-200d con profundidades mayores o menos que 100 μm (0.004”) y anchuras mayores o menos que 50 μm (0,002'').
Refiriéndose ahora a la figura 10, se muestra una camisa o recubrimiento 202 que encierra el vástago 12. En una forma de realización, la camisa 202 puede ser en forma de una envoltura retráctil u otro revestimiento adecuado que encierra el vástago 12 y permite que el esterilizante 210 se desplace a lo largo de las ranuras 200a-200d. La envoltura retráctil 202 puede Retráctil 202 puede estar configurada para encerrar todo el vástago 12 o sólo una porción del mismo.
La envoltura retráctil 202 puede estar hecha de cualquier material adecuado conocido en el estado de la técnica disponible, incluyendo cualquier película de polímero plástico, pero sin limitarse a ella. Las películas de plástico que se pueden emplear como envoltura retráctil 202 pueden incluir polietileno, PVC, y otras similares. Además, la envoltura retráctil 202 puede estar configurada para diferentes claridades, coeficientes de contracción, etc. Por otro lado, el envoltorio 202 puede ser del tipo que se contrae en una dirección o en múltiples direcciones (por ejemplo, una envoltura retráctil unidireccional o bidireccional, respectivamente).
Durante el proceso de fabricación de pinzas bipolares 10, la envoltura retráctil 202 puede ser aplicada al vástago 12 por cualquiera de los medios adecuados conocidos en el estado de la técnica. En una forma de realización alternativa, la envoltura retráctil 202 puede ser aplicada y encogerse parcialmente alrededor del vástago 12, describiéndose a continuación con mayor detalle la utilidad de tener una envoltura retráctil aplicada de esta manera.
Un tipo de agente esterilizante que es adecuado para su uso con la presente invención es EtO. Como se ha mencionado anteriormente, el óxido de etileno puede ser mezclado con otros gases durante el proceso de esterilización. Gases que se pueden utilizar como diluyentes pueden incluir, pero sin limitarse a ellos, los CFCs y dióxido de carbono.
En funcionamiento normal, antes de introducir las pinzas bipolares 10 en un sitio quirúrgico, dichas pinzas bipolares 10 deben ser antes esterilizados. Como se mencionó anteriormente, esto puede lograrse por lo menos mediante un par de métodos. Para los fines de la presente divulgación se supondrá que las pinzas bipolares 10 se esterilizan mediante esterilizante EtO. Como parte del proceso de esterilización, las pinzas bipolares 10 se colocan en una cámara de esterilización (no mostrada). El esterilizante 210 se introduce después en la cámara de esterilización de cualquier manera adecuada. Cuando el esterilizante 210 se introduce en la cámara de esterilización, tal esterilizante 210 entra en el instrumento laparoscópico 10, donde llegará a las zonas objetivo para fines de esterilización, es decir, los componentes internos asociados a las pinzas bipolares 10. El tener una o más ranuras 200 proporciona vías adicionales para que el esterilizante 210 se desplace a la zona objetivo.
De lo que antecede y haciendo referencia a los diferentes dibujos de las figuras, los expertos en la técnica apreciarán que también se puede hacer ciertas modificaciones en la presente invención. Por ejemplo, en lugar de emplear una o más ranuras 200, el vástago 12 puede incluir uno o más túneles 214 definidos en el vástago 12 ó 12'. El túnel 214 puede configurarse para funcionar de una manera similar a la descrita anteriormente con respecto a la ranura 200. Es decir, uno o más túneles 214a-214d se pueden extender a lo largo de la longitud del vástago 12 (o al menos una parte de la misma), proporcionando los túneles 214a-214d un camino para que el esterilizante 210 llegue hasta una zona objetivo (véase la figura 10). El túnel 214 puede incluir aberturas o rendijas 208 como las que se han descrito anteriormente con referencia a las ranuras 200.
La presente invención también proporciona un método para la esterilización de un instrumento laparoscópico, incluyendo el método las etapas de: proporcionar un instrumento laparoscópico 10 que comprende: un vástago 12 que incluye una o más ranuras 200a-200d que se extienden al menos parcialmente a lo largo de la longitud del mismo, proporcionando las ranuras una vía para un esterilizante; y proporcionando una envoltura retráctil 202 que rodea al vástago y permite que el esterilizante se traslade a lo largo de las ranuras. El método también incluye las etapas de: colocar el instrumento laparoscópico en un aparato de esterilización, e introducir el esterilizante en el aparato de esterilización con el fin de esterilizar el instrumento laparoscópico.
En una forma de realización, la envoltura termorretráctil 202 puede ser sometida a una fase inicial de contracción, en la que la envoltura termo-retráctil 202 no está completamente encogida antes de colocar el instrumento laparoscópico 10 en un aparato de esterilización. El aparato de esterilización puede ser configurado para contraer aún más la envoltura retráctil 202. Algunas maneras convencionales para contraer la envoltura termorretráctil 202 pueden incluir un túnel de calor, pistola de calor, etc., que no alcanzarán una temperatura suficiente para causar daños a los componentes internos de las pinzas 10. De este modo, después de colocar un instrumento laparoscópico 10 dentro del aparato de esterilización y de introducir esterilizante 210, dado que la envoltura retráctil 202 no está completamente contraída, el esterilizante puede entrar libremente en el instrumento laparoscópico 10 y llegar a una zona objetivo. El método puede incluir además la etapa de: someter la envoltura retráctil a una fase final de contracción, en la que, una vez se ha completado la etapa de contracción final, la envoltura retráctil forma una junta hermética contra el vástago.
Aunque se han mostrado en los dibujos diversas formas de realización de la invención, no se pretende que la invención quede limitada a ellas, si no que se pretende que la invención tenga un alcance tan amplio como permita el estado de la técnica y que la memoria sea leída en ese sentido. Por lo tanto, la descripción anterior no debe interpretarse como una limitación, sino simplemente como ejemplos de realizaciones particulares. Los expertos en la materia podrán prever otras modificaciones dentro del alcance y el espíritu de las reivindicaciones que se adjuntan.
Claims (4)
- REIVINDICACIONES1. Un instrumento quirúrgico (10) que comprende: 5un alojamiento (820) que tiene un vástago (12) extendiéndose desde el mismo; definiendo el vástago una superficie interior (204) y exterior (206); incluyendo al vástago al menos una ranura (200) o túnel (214) definido en el interior del mismo, que se extiende al menos parcialmente a lo largo de la longitud del mismo, estando la al menos una ranura o túnel10 configurado para permitir el paso de un esterilizante a través del mismo; al menos una abertura o rendija (208) que conecta la ranura o túnel y la superficie interior del vástago; una camisa (202) que rodea al vástago y que permite que el esterilizante se desplace a lo largo de la al menos una ranura o túnel.15 2. El instrumento según la reivindicación 1, en el que el vástago incluye una pluralidad de las mencionadas ranuraso túneles que se extienden a lo largo de toda la longitud del mismo, dispuestas según una relación espacial fija entre ellas.
- 3. El instrumento según la reivindicación 1, en el que la pluralidad de ranuras son cuatro, dispuestas a intervalos 20 de 90º alrededor de la circunferencia del vástago.
- 4. El instrumento según la reivindicación 2 ó 3, en el que la pluralidad de ranuras tienen una profundidad de aproximadamente 50 μm (0,002'') y una anchura de aproximadamente 100 μm (0,004'').25 5. El instrumento según cualquiera de las anteriores reivindicaciones, en el que la camisa es una envoltura termorretráctil.
- 6. El instrumento según cualquiera de las anteriores reivindicaciones, en el que la ranura o túnel es un túnel (214).
