Abstract

La présente invention concerne un dispositif utile dans le domaine de l'ophtalmologie. Les lames, les dispositifs et les procédés pour les utiliser, font intervenir la séparation ou le soulèvement de l'épithélium cornéen de l'oeil sous la forme d'une couche sensiblement continue, pour former un élément de tissu épithélial qui est en général toujours rattaché à la cornée. L'élément de tissu épithélial se présente souvent sous la forme d'un volet ou d'une poche. Le dispositif fait intervenir en particulier une lame oscillante non coupante sur la cornée, qui fonctionne comme un élément de séparation ou de dissection conçu par exemple, par structure ou pouvoir lubrifiant, pour séparer l'épithélium au niveau de surfaces de clivage naturelles de l'oeil, en particulier entre l'épithélium et le stroma cornéen (membrane de Bowman) qui se séparent spécifiquement dans la zone de la lamina lucida. La lame peut être émoussée et présenter une zone ouverte à distance du bord de dissection, pour appliquer des forces de frottement inférieures contre l'épithélium séparé. La lame peut présenter un bord de dissection émoussé réalisé tout d'abord par formation d'un bord suffisamment coupant pour découper l'épithélium, puis par électropolissage de ce bord pour donner un bord sensiblement émoussé adapté à la séparation de l'épithélium de la cornée, sans découper la cornée. La lame peut être au moins partiellement revêtue d'une ou plusieurs substances lubrifiantes, de préférence sur les surfaces adjacentes de l'épithélium en cours d'utilisation. L'épithélium séparé peut être soulevé ou retiré par pelage, de la surface de l'oeil pour former un volet ou une poche épithélial(e). L'épithélium peut alors être replacé sur la cornée après une procédure de réfraction, ou replacé sur une lentille oculaire après placement de cette lentille oculaire sur l'oeil.
