TEHNIČKA OBLAST
[0001] Predmetni pronalazak se odnosi na sredstva za isporučivanje tečnosti namenjena za isporučivanje terapeutskih tečnosti pacijentu, konkretnije na infuzionu pumpu za isporučivanje terapeutskih tečnosti pacijentu.
OSNOVNE INFORMACIJE
[0002] Sredstva za isporučivanje tečnosti imaju brojne primene, kao što je supkutano isporučivanje tečnog leka ili druge terapeutske tečnosti pacijentu. Na primer, ambulantne infuzione pumpe su kod pacijenata sa dijabetesom melitusom korišćene za isporučivanje insulina pacijentu. Ove ambulantne infuzione pumpe mogu da obezbede prefinjene profile isporučivanja tečnosti, uključujući promenljivu bazalnu brzinu i bolusne potrebe. Sposobnost pažljivog kontrolisanja isporučivanja leka može dovesti do poboljšane delotvornosti leka i terapije i manje toksičnosti za pacijenta.
[0003] Neke postojeće ambulantne infuzione pumpe uključuju rezervoar koji sadrži tečni lek i koriste elektomehaničke tehnologije za pumpanje ili doziranje kako bi se tečni lek putem cevčice isporučio u iglu i/ili mekanu kanilu koja je supkutanu umetnuta u pacijenta. [0008] Sredstvo za isporučivanje tečnosti, prema otelotvorenjima predmetnog otkrića, može da se koristi za isporučivanje terapeutske tečnosti (npr. tečnog leka) pacijentu putem alata za transkutani pristup, kao što je igla/trokar i/ili kanila. Mehanizam za inserciju alata za transkutani pristup može da se koristi za primenu alata za transkutani pristup, na primer, umetanjem i povlačenjem igle/trokara jednim neprekidnim pokretom. Mehanizam za inserciju takođe može da obezbedi rastuću silu za inserciju prilikom pomeranja igle/trokara u smeru insercije. Prema predmetnom pronalasku, sredstvo za isporučivanje tečnosti takođe uključuje mehanizam kvačila koji olakšava punjenje rezervoara i aktivaciju pogonskog mehanizma za pokretanje tečnosti iz rezervoara. U određenim otelotvorenjima, sredstvo za isporučivanje tečnosti može da obuhvata ambulantno sredstvo za infuziju insulina.
[0009] U drugim otelotvorenjima, sredstvo za isporučivanje tečnosti može da se koristi za isporučivanje terapeutske tečnosti pacijentu uz integrisano praćenje, kao što je kontinualno merenje glukoze (CGM). U ovim otelotvorenjima, sredstvo za isporučivanje tečnosti može da obuhvata alat za transkutani pristup konfigurisan za uvođenje test trake za praćenje u pacijentovu kožu, na primer, koristeći jednu ili više igala, kanila i/ili trokara.
[0010] Uz upućivanje na SL.1-6 prikazano je i opisano jedno otelotvorenje sredstva za isporučivanje tečnosti 100. U jednom reprezentativnom otelotvorenju, sredstvo za isporučivanje tečnosti 100 se koristi za supkutano isporučivanje tečnosti, kao što je tečni lek (npr. insulin), nekoj osobi ili životinji. Stručnjacima za ovu oblast će biti jasno da sredstvo za isporučivanje tečnosti 100 može da se koristi za isporučivanje drugih vrsta tečnosti. Sredstvo za isporučivanje tečnosti 100 može da se koristi za isporučivanje tečnosti na kontrolisani način, na primer, u skladu sa profilima isporučivanja tečnosti koji zadovoljavaju bolusne potrebe, kontinuiranu infuziju i varijabilnu brzinu isporučivanja.

[0015] Jedno otelotvorenje povezujućeg mehanizma sa kosom oprugom 182 uključuje spiralnu torzionu oprugu 181 i prvu i drugu spojnicu 183a, 183b spojene između torzione opruge 181 i prvog kliznog elementa 184. Povezujući mehanizam sa kosom oprugom 182 takođe smanjuje statičko naprezanje izazvano zaključavanjem i zadržavanjem kliznih elemenata 184, 186 i obezbeđuje glatkije i ugodnije umetanje igle/trokara zbog načina na koji spojnice 183a, 183b vektorišu sile primenjene na klizne elemente 184, 186. Statičke sile na kliznim elementima 184, 186 su relativno male u položaju pre primene, kada su spojnice 183a, 183b u potpunosti povučene. Kada se započne primena i spojnice 183a, 183b počnu da se produžavaju, sila umetanja raste jer se vektori sile povećavaju u smeru umetanja sa produžavanjem spojnica 183 a, 183b sve dok se ne ostvari maksimalna sila umetanja kada su potpuno ispružene, u srednjem položaju. Zahvaljujući postepenom povećavanju sile umetanja, umetanje i izvlačenje igle/trokara je glatkije, mirnije i bezbolnije.

Klizni elementi 284, 286 se klizno prihvataju u okvir 290 i pokreće ih povezujući mehanizam sa kosom oprugom 282 koji obuhvata torzionu oprugu 281 i spojnice 283a, 283b. U ilustrovanom otelotvorenju, pre punjenja rezervoara 130, opruga kvačila 162 zadržana je u napregnutom, neaktivnom položaju putem zasuna opruge 164 koji je u interakciji sa pogonskim točkom 156 (SL.11-13). Nakon što je rezervoar 130 napunjen, opruga kvačila 162 na taj način može da se aktivira rotacijom pogonskog točka 156 sve dok zasun opruge 164 ne oslobodi oprugu kvačila 162 (SL.14) što omogućava da se opruga kvačila 162 oslobodi i stegne cevastu navrtku 154 (SL.15 i 16), nakon čega tečnost može da se dozira iz rezervoara 130 uz kontinualno okretanje pogonskog točka 156.

Ovo smanjuje broj primarnih impulsa putanje tečnosti za pripremu pumpe i osigurava kompletnu i odgovarajuću pripremu putanje tečnosti pre postavljanja na telo. Mehanizam kvačila takođe omogućava promenu koraka navoja za primenu drugih lekova bez potrebe da se menja dizajn navrtke s nagibom koja se koristi u mehanizmima za pogon tečnosti u drugim postojećim pumpama. Komponente mehanizma kvačila se takođe lakše pregledaju od sklopa navrtke s nagibom.

Pored reprezentativnih otelotvorenja koja su ovde prikazana i opisana, u opsegu predmetnog pronalaska su razmatrana i druga otelotvorenja.
