Description
図9は、脳神経外科等の手術において仮に従来の手術具9(持針器)を用いた場合の様子を示す模式図である。脳神経外科等の手術においては、例えば図9に示すように、人体(患者PA)の頭部HDの一部を開口し、頭部HD内に形成した狭く深い穴CAに対し上方から手術具9を挿入し、手術具9の挿入方向とほぼ同じ方向に顕微鏡900の対物レンズ910の光軸OAを向け、顕微鏡900を覗いて手術具9の先端付近の術野を目視しながら手技を行うことがある。
かかる手術具は、棒状体の外周面に装着されたスライダハンドルを更に備える。また、かかる手術具は、スライダハンドルが棒状体(ここでは押し棒を除く)に対し長手方向線に沿って前後にスライドし、かつ、長手方向線の回りに回動するように構成されている。さらに、かかる手術具は、スライダハンドルを長手方向線に沿って前後させることで処置ユニットの一対の処置片が開閉し、かつ、スライダハンドルを長手方向線の回りに回動させることで処置ユニットが長手方向線の回りに回動するよう構成されている。
1.実施形態1に係る手術具1の構成
(1)手術具1の概要
処置ユニット10は、先端が開閉する一対の処置片11,12からなる。
処置ユニット10は、少なくとも2片に分かれており、開閉軸19(図2参照)を中心に先端同士が開閉するものであれば如何なるものであってもよい。ここでは吻合針を持つための持針具を例として説明を続ける。
なお、処置ユニット10は持針具に限定されるものではなく、対象物を握る・掴む等を行うための鑷子・鉗子であったり対象物を切断するための剪刀についても処置ユニット10として好ましく適用することができる。つまり、処置ユニット10は持針具、鑷子又は鉗子であってもよい。また処置ユニット10は剪刀であってもよい。
棒状体100は、処置ユニット10と固定筒部20(後述)との間を接続し、長手方向線LLに沿って延びる棒状の部材の総体をいうものとする。棒状体100には、後述する中空パイプ130、中空パイプ130の内側に嵌挿された押し棒110、中空パイプ130の外周面に接続されたバレル140等が含まれる。棒状体100の先端側には処置ユニット10が接続され、後端側には後述する固定筒部20、把持部30等が配置されている。詳細は後述するが、押し棒110の先端では平板120、平板120から突出した上ボス123,下ボス124を介して処置ユニット10が係合している。また、バレル140の内側には、長手方向線LLに沿って進退可能でかつ長手方向線LLの回りに回動可能となっている中空部材150が配置されている。中空部材150は、その中心軸が長手方向線LLに一致するように回動可能かつ、長手方向線LLに沿って前後に移動可能になっている。
なお、本明細書において「先端側」とは棒状体100に対して処置ユニット10が配置された側をいうものとし、「後端側」とは棒状体100に対して固定筒部20が配置された側を言うものとする。
固定筒部20は、先端側に開口した穴22を有する略筒状の部位である。かかる穴22の内壁においては円周方向に沿って切られたガイド溝24(図2参照)が形成されている。一方、棒状体100を構成するバレル140の後端側には長手方向線LLから径方向RDに突出したフランジ形状のフランジ部144が形成されており、当該フランジ部144は固定筒部20のガイド溝24に係合している。別言すると、固定筒部20は、穴22の内壁において棒状体100の後端側(より具体的にはバレル140の後端側)が長手方向線LLを中心に回動可能に係合したものである。
なお、実施例では固定筒部20の開口部の形状が略円形になっているが、最終的に固定筒部20と棒状体100とが回動可能となっていれば、特に略円形に限定されるものではない。
把持部30は、手術具1の使用者が把持する部位として固定筒部20と一体に設けられた部分である。実施形態1において、把持部30は、長手方向線LLから径方向RDに突出して形成されている。固定筒部20と把持部30とは一体的に形成(連成)されていてもよいし、双方別個に形成されたものが互いに連結されていてもよい。
指掛部32は、使用者の指を掛けるための面である指掛面32aを含む。指掛面32aの接線L1の方向は長手方向線LLと交差する方向となるように設定されている。逆にいうと、指掛面32aの接線は長手方向線LLと平行にはなっていない。実施例では、指掛面32aの接線が長手方向線LLと直交するものになっている。
指掛部32は、長手方向線LLから棒状体100の径方向に所定の寸法だけオフセットした位置に配置されている(図2参照)。オフセット量は、使用者の掌の寸法等に応じて設定され、概ね5mm~50mmの範囲内で設定されていることが好ましい。
実施例では、一方で掌当部34の掌当面34aと反対の側にステー35を連成しておき、他方で指掛部32から長手方向線LLの後端側に把持部中間部36を突設した上で、当該把持部中間部36の後端側に開口するステー穴36aを設け、ステー35をステー穴36aに進退自在に嵌挿している。このような構成とすることで掌当部34と指掛面32aとの間の距離が変更可能となり、その結果、掌当部34とスライダハンドル200との間の距離が変更可能となっている。なお、実施例では距離を設定した段階で止めねじ37を締結することにより、掌当部34の位置を固定できるようになっている。
スライダハンドル200は、処置ユニット10の先端の回動や先端の開閉を行うために使用者によって操作される部分である。
スライダハンドル200は棒状体100の外周面100aに装着されている。スライダハンドル200は、棒状体100の外周面100aに接触するように装着されていてもよいし、外周面100aに接触しないようにしてやや浮いた状態で外周面100aの径方向外側に配置されていてもよく、このような態様についても「装着」に含まれるものとする。スライダハンドル200は棒状体100の外周面100aを、周の半分程度巻き付けるように構成されていてよいし(実施例)、単に市販のスライドスイッチのように一部にのみ被さるように構成されていてもよいし、ほぼ全周/又は完全に全周に渡って巻き付けるように構成されていてもよい。ほぼ全周/又は完全に全周に渡って巻き付けた態様の場合には、大きな角度で回動させたとしても指がスライダハンドル200から外れづらい手術具とすることができる。
なお、スライダハンドル200の回動の中心軸と、棒状体100(少なくともスライダハンドル200が装着された付近の棒状体100の一部は)の中心軸とは同軸の関係になっている。また、スライダハンドル200の前後動の方向は長手方向線LLの方向と同じ方向になっている。
以下、これを実現するための具体的な構成を中心に説明を続ける。
上記したように棒状体100には、先端に処置ユニット10が係合した押し棒110が含まれている。なお、「押し棒110」は平板120(後述)を先端側に押す機能を有するだけでなく後端側に引く機能も有する。
手術具1は、第1連結部101を備える。第1連結部101は、スライダハンドル200と押し棒110とを連結し、スライダハンドル200の前後動を押し棒110の前後動に変換するものである。
爪210が設けられるスラスト方向(長手方向線LLに沿った方向)の位置は先端側の縁に限られず、スライダハンドル200の先端側と後端側との間の中間位置に配置されていてもよい。

第2連結部102は、スライダハンドル200と「処置ユニット10の開閉軸19を固定する棒状体100の固定部107」とを連結し、スライダハンドル200の回動を処置ユニット10の回動に変換するものである。
ここでの処置ユニット10の開閉軸19を固定する棒状体100の固定部107は、開閉軸19を固定するものであればいかなる部材によって構成されていてもよい。実施形態1では、図4(a)に示すように、「開閉軸19を固定する棒状体100の固定部107」は、先端側から開閉軸19を支える処置ユニットベース135と、先端に処置ユニットベース135が結合された中空パイプ130と、内側に中空パイプ130が嵌挿され、ねじ192によって当該中空パイプ130と結合されたバレル140と、によって構成されている。
また、スライダハンドル200は、バレル140のスリット142越しに、ねじ194(図5では図示を省略)によって中空部材150のねじ穴156に締結されている。
以上のような構成により、スライダハンドル200は棒状体100から脱落することなく結合されている。
参考までに、スライダハンドルの爪210は、中空部材150の切り欠き152の部分を通過するため、中空部材150と干渉しないようになっている。
手術具1は、開閉変換部103を備える。開閉変換部103は、押し棒110の前後動を処置ユニット10の開閉に変換するものである。
各処置片11,12には軸穴11c,12cがそれぞれ形成されている。各処置片11,12は、軸穴11c,12cよりも先端側に処置部11a,12aをそれぞれ有しており、軸穴11c,12cよりも後端側に操作部11b,12bをそれぞれ有している。これら一対の処置片11,12は、それぞれの処置片11,12の軸穴11c,12cに共通に嵌挿された開閉軸19で結合されている。実施形態1では開閉軸19はピン18及びピンキャップ18aによって構成されている。
一方、押し棒110の先端には平板120が配設されている。平板120は、第1面121に上ボス123が設けられ、かつ、第1面121と反対側の面である第2面122には下ボス124が設けられている。なお、上ボス123と下ボス124は平面視したときに同じ位置に配置されている。しかし、それに限定されるものではない。
そうして、上ボス123は一方の処置片11のカム溝11dに係合し、下ボス124は他方の処置片12のカム溝12dに係合している。
実施形態1に係る手術具1は、外科手術、例えば脳外科手術で好適に用いることができる。
図7は、実施形態1に係る手術具1の使用例を説明するために示す斜視図である。図7(a)は手術具1の把持を始める第1ステップの様子を示す図であり、図7(b)はスライダハンドル200の前後動や回動を行う第2ステップの様子を示す図である。なお、図7においては右手に手術具1を持つ例を示しているが、左手で持つ場合にはこれと逆の関係となる。
そうしたうえで、親指81及び人差し指82を使ってスライダハンドル200を長手方向線LLに沿って前後に動かすことにより処置ユニット10の先端を開閉したり、スライダハンドル200を長手方向線LLの回りに回動することにより処置ユニット10全体を回動したりする。これにより、親指81及び人差し指82による操作のみによって使用者(医師)が意図した手技を先端側の処置ユニット10で実現することができる。
(2-1)[発明が解決しようとする課題]の欄でも触れた通り、中指83は精密な操作に不向きな指である。従来の手術具9を用いて処置ユニットの開閉を行うべく中指83で精密な操作を行おうとすると力んでしまい、その結果、操作の土台となっている掌86や手首89もブレてしまう。特に棒状体の寸法が大きい長尺な手術具を用いた場合には、手元のブレが処置ユニットの先端で増幅され微細な手技を損なってしまう。
このため、図7(b)に示すように、使用者は親指81と人差し指82でスライダハンドル200を挟み込むように摘まみ、親指81と人差し指82を回動させることで、処置ユニット10を回動させることだけでなく、親指81と人差し指82を前後(進退)させることで処置ユニット10を開閉させることもできる。
つまり、中指83を何ら使うことなく処置ユニット10の開閉を実現できるため、上記した中指83を動かすことによる掌86や手首89のブレが生じることがない。
また、従来の手術具9においてはトリガーの角度に応じて開閉度合いが定まるものとなっており、一般的にトリガーの角度の制御はしづらいものとなっている。これに対し、実施形態1に係る手術具1は、一軸(長手方向線)上の位置によって処置ユニット10の開閉度合いが定まるため、開閉の微調整を行いやすく、対象物を挟んだ感覚が直感的に分かり易いものとなっている。
このため、実施形態1に係る手術具1は、従来の手術具9に比べて棒状体100の外径が細い設計にしやすい。棒状体100の外径を細くすることにより、例えば図9に示す脳神経外科等の手術において顕微鏡900で処置ユニット10の先端を見ようとしたとき、棒状体100が視野の邪魔にならないようにできる。つまり、狭く深い部位においても術野が確保しやすい構造とすることができる。
図8は実施形態2に係る手術具2の斜視図である。基本的な構成及び特徴が実施形態1と同じ構成要素については、実施形態1における符号を援用しここでの説明は省略する。
Claims (9)
- 先端が開閉する一対の処置片からなる処置ユニットと、
先端側に前記処置ユニットが接続され、長手方向線に沿って延びる棒状の棒状体と、
前記先端側に開口した穴を有し、当該穴の内壁において前記棒状体の後端側が前記長手方向線を中心に回動可能に係合した略筒状の固定筒部と、
使用者が把持する部位として前記固定筒部と一体に設けられた把持部と、を備えた手術具であって、
前記棒状体の外周面に装着されたスライダハンドルを更に備え、
前記スライダハンドルが前記棒状体に対し前記長手方向線に沿って前後にスライドし、かつ、前記長手方向線の回りに回動するように構成されており、
前記スライダハンドルを前記長手方向線に沿って前後させることで前記処置ユニットの前記一対の処置片が開閉し、かつ、前記スライダハンドルを前記長手方向線の回りに回動させることで前記処置ユニットが前記長手方向線の回りに回動するよう構成されている、
ことを特徴とする手術具。 - 請求項1に記載の手術具において、
前記スライダハンドルは使用者の親指及び人差し指によって操作されるものであることを特徴とする手術具。 - 請求項1又は2に記載の手術具において、
前記把持部は、
前記長手方向線から前記棒状体の径方向に突出して形成され、
使用者の指を掛けるための面であって当該面の接線が前記長手方向線と交差する方向に設定された指掛面を含み、前記長手方向線から前記径方向にオフセットした位置に配置されている指掛部と、
前記指掛部からみて前記後端側に配設された掌当部と、を有する、
ことを特徴とする手術具 - 請求項3に記載の手術具において、
前記指掛部は使用者の少なくとも中指を掛ける部位であることを特徴とする手術具。 - 請求項1~4のいずれかに記載の手術具において、
前記棒状体には、先端に前記処置ユニットが係合した押し棒が含まれており、
前記スライダハンドル及び前記押し棒を連結し、前記スライダハンドルの前後動を前記押し棒の前後動に変換する第1連結部と、
前記スライダハンドル及び「前記処置ユニットの開閉軸を固定する前記棒状体の固定部」を連結し、前記スライダハンドルの回動を前記処置ユニットの回動に変換する第2連結部と、
前記押し棒の前後動を前記処置ユニットの開閉に変換する開閉変換部と、を備えた、
ことを特徴とする手術具。 - 請求項5に記載の手術具において、
各処置片には軸穴がそれぞれ形成されており、各処置片は前記軸穴よりも先端側の処置部と前記軸穴よりも後端側の操作部とをそれぞれ有しており、
前記一対の処置片は、それぞれの前記処置片の前記軸穴に共通に嵌挿された前記開閉軸で結合されており、
さらに、各前記操作部には、前記処置ユニットが閉じた状態のとき前記長手方向線に対し所定の角度を有するカム溝がそれぞれ形成されており、
前記押し棒の先端には、第1面に上ボスが設けられ、かつ、前記第1面と反対側の面である第2面には下ボスが設けられた平板が配設されており、
前記上ボスは一方の前記処置片の前記カム溝に係合し、前記下ボスは他方の前記処置片の前記カム溝に係合しており、
前記長手方向線に垂直な面で断面視したときに、前記処置ユニットの前記操作部及び前記平板は、前記押し棒を収容している中空パイプの外径の内側に納まっている、
ことを特徴とする手術具。 - 前記スライダハンドルの表面に位置マーカーが設けられていることを特徴とする請求項1~6のいずれかに記載の手術具。
- 前記掌当部と前記スライダハンドルとの間の距離が変更可能となっていることを特徴とする請求項1~7のいずれかに記載の手術具。
- 請求項1~8のいずれかに記載の手術具において、
前記処置ユニットの先端が、前記長手方向線から径方向に離れた位置に位置していることを特徴とする手術具。
