Description
本文揭示包含一種去氧糖抗代謝物質之口腔護理組合物及在病患抑制微生物生物膜生長及/或分解微生物生物膜的方法。
抗代謝物質是一種與細胞必需代謝物質極為相似之物質。抗代謝物質在生理反應中,與必需代謝物質競爭、干擾或取代,因而擾亂細胞正常的代謝功能。已知使用抗代謝物質作為治療用途。例如,磺胺類藥是抗代謝物質,其作為對-胺基苯甲酸之類似物。這些化合物擾亂細菌的代謝,而非人類,因此在人類用於根除細菌感染。胺甲喋呤及5-氟尿嘧啶是抗代謝物質進一步的實例,其被用於治療各種癌症。
在此技藝中已知葡萄糖抗代謝物質,例如2-去氧-D葡萄糖是一種葡萄糖分子,其可被細胞之葡萄糖運送者接納。然而,2-去氧-D葡萄糖其2-羥基被氫所取代,所以不像葡萄糖,它不能進行進一步的糖解作用。

然而,在此技藝領域的問題是,使用抗菌劑以減少口腔細菌,同時亦會導致所期望細菌的減少及擾亂健康微菌叢的獲得。此外,抗菌的作用並不必然與減少或抑制生物膜的生成有關,此對那些罹患齒齦炎或牙周炎之病患特別有用。

生物膜

本發明口腔護理組合物之一個具體實例,包含一種在產品中口

本發明的一個具體實例,口腔護理組合物不包含任何額外的抗菌或抗微生物化合物,除了前揭之去氧糖抗代謝物質。

當用於組合物,口感劑包括可賦予所期望的結構或其他感覺之材料。

當含有磨擦劑,平均粒子尺寸一般是約0.1至約30微米,例如約1至約20或約5至15微米。

依本發明之組合物,較佳地包含一種在產品中口腔可接受之載劑,如漱口水、牙膏、牙用乳膏、口香糖、假牙黏著劑或可攜式劑型商品,例如,不是用來限制,一種口含錠、一種薄荷、珠、薄片、在一小型可攜式噴霧器(噴霧瓶)調製成可供經口施用之液劑、在一小型可攜式小滴-產生瓶調製成可供經口施用之液劑,或一柔軟可變形之錠劑(咀嚼物)。本文所用一種”口腔可接受載劑”,係指是可安全用於本發明組合物之材料或材料組合物,具有相當合理的效益/風險比例。
本發明亦提供可攜式劑型商品,包括一種前述定義之口腔護理組合物,其中可攜式劑型商品選自一種口含錠、一種薄荷、珠、薄片、在一小型可攜式噴霧器,其含有調製成以噴霧供經口施用之液劑、在一小型可攜式小滴-產生瓶,其含有調製成以小滴供經口施用之液劑,及一柔軟可變形之錠劑。
較佳地,用於本發明之特定材料及組合物是,照著製藥的或化妝品-可接受的、臨床有用的、及/或臨床有效的。本文所用如”製藥可接
受”或”化妝品可接受的”、”臨床有用的”及/或”臨床有效的”要件,是其適合用於人類及/或動物,且提供適當量(一臨床有效量)以達成所期望治療的、預防的、感官的、裝飾的、或化妝的效益,而沒有不當的副作用(如毒性、刺激性,及過敏反應),具有相當合理的效益/風險比例。
在組合物中所有的成分,除了具有其原有的功能外,也促成組合物整體的性質,包括安定性、功效、一致性、口感、味道、香氣等等。
使用方法
依本發明之組合物可投與或施用至人類或其他動物患者。組合物可適合投與或施用至人類或其他動物患者之口腔。典型地,組合物是用來抑制微生物生物膜的生成及/或分解微生物生物膜。
本發明進一步提供如前述定義之組合物,以預防或治療口腔之疾病狀況。典型地,疾病狀況是因生物膜的生成所造成的。疾病狀況可選自牙菌斑、牙齒蛀蝕、牙周疾病及齒齦炎。
因此,本發明提供一種如前述定義之組合物,以作為藥品,且尤其是,用來抑制微生物生物膜的生成及/或分解微生物生物膜。
本發明亦提供一種在病患抑制微生物生物膜的生成及/或分解微生物生物膜的方法,包括投與病患含有去氧糖代謝物質之組合物。較佳地,組合物是如本文定義之口腔護理組合物,且組合物是施用至口腔。在較佳具體實例,是用來治療或預防因生物膜生成所造成狀況的方法。較佳地,因生物膜生成所造成的狀況,是一種口腔的狀況且可選自牙菌斑、牙齒蛀蝕、牙周疾病及齒齦炎。
本發明進一步提供使用去氧糖代謝物質的用途,以一種口腔護理組合物,在病患抑制微生物生物膜的生成及/或分解微生物生物膜。較佳地,其用途包括在病患口腔抑制微生物生物膜的生成及/或分解微生物生物膜。
在本發明一個具體實例,使用去氧糖代謝物質減少細菌少於5%
或少於2%或少於1%。
在本發明其他具體實例,使用去氧糖代謝物質減少或抑制生物膜的生成,至少25%或至少40%或至少60%。
在本發明其他具體實例,使用去氧糖代謝物質,其具有至少1000%之生物膜效率(生物膜效率定義為(%生物膜抑制/%細菌減少)x 100%)。
一種包含去氧糖代謝物質之組合物,可在病患顯著抑制生物膜生成及/或分解已存在之生物膜。組合物特別有用在口腔中抑制生物膜生成及/或分解生物膜。此外,去氧糖代謝物質不會殺死細菌,因此不具有傳統抗菌劑之缺點,如發展成抗藥性,因而維持口腔中健康的微菌叢。抑制生物膜生成的能力係指在口腔中的細菌將受到較少保護,因此對傳統的去除/殺死較為敏感。
一種藥劑包含依本發明之組合物,可投與至病患。去氧糖代謝物質可有效抑制生物膜生成及/或分解生物膜,而不會與其他口腔護理活性成分不相容或不穩定,且不會去活化其他所需的口腔護理成分,同時在活體內容易傳送。
在本發明一個具體實例,病人因罹患齒齦炎或牙周炎,需要減少或抑制生物膜生成。依次,治療方法可直接導入治療齒齦炎或牙周炎。
實施例
實施例1-健康的生物膜
0.5" Ca2+
-缺乏羥磷石灰(HAP)圓盤以UV-滅菌之人類唾液包覆
隔夜。經唾液包覆的HAP(scHAP)圓盤以測試化合物(0.01%2-去氧-D-葡萄糖)溶液或對照化合物((0.01%)氯化十六烷基吡啶(CPC))處理1小時,讓圓盤再吸收與包覆有潛力的活性成分。
1個小時後,以抽吸方式除去過多的處理,圓盤接種1毫升細菌,細菌來自在OD(600)約0.2之連續培養系統。連續培養系統在37℃下維持約3個月,其包含5種代表性的口腔細菌菌種:粘放線菌、酪蛋白乳酸桿菌、口腔鏈球菌、小型範永氏菌及核細梭菌。先前的工作顯示此混合物生成可重複的生物膜,其可作為在牙菌斑所發現更複雜,混合500-700株細菌菌種之代表。
經接種的scHAP圓盤在37℃下培養48小時,讓生物膜在其表面生成。生物膜生成48小時後,將圓盤移轉至含1毫升0.25%胰蛋白酶溶液之試管中。圓盤在37℃震盪下培養45分鐘,將試管短暫的渦旋以將細菌再懸浮,並將上清液移轉至新的24孔盤中,將盤震盪2分鐘以打散任何殘存的HAP薄層,將上清液移至新的盤中,因此其吸光度可在610nm下測定。所有檢體測試3次並測定3孔的平均值。
除了胰蛋白酶-分解經再懸浮的細菌溶液之吸光度外,亦測量經48小時長成生物膜後之最終培養基上清液之吸光度,此顯示活性成分是否具有抗菌作用及防止細菌族群的生長。
在生物膜長成之培養基上清液中,2-去氧-D-葡萄糖對細菌生長的影響顯示於下表中,CPC(氯化十六烷基吡啶),其是一種常見於各種口腔護理產品之強力抗菌劑,尤其是漱口水,其作為實驗的正對照組。CPC在漱口水典型地用量約0.075%,但先前的研究顯示約0.01%溶液,是一個非常有效的抗菌劑。
儘管在上表中,0.01%CPC、三氯沙及氯化烷基二甲基苄基銨顯示高度的抑制細菌生長,這些化合物具有中等的生物膜抑制效率(C),其中C=B/A x 100%。DL-5-羥基離胺酸鹽酸鹽具有顯著的細菌減少活性,但也具有可忽略的生物膜抑制作用。2-去氧-D-葡萄糖作為生物膜生成抑制劑是更有效的,但仍顯示近10%的細菌減少。
相反的,6-去氧-D-葡萄糖及2-氟-2-去氧-D-葡萄糖二者均顯示,生物膜抑制遠遠大於從他們被預測之細菌減少。因此可下結論,6-去氧-D-葡萄糖及2-氟-2-去氧-D-葡萄糖是一有效及有效率的生物膜生成抑制劑,其亦可維持健康的微菌叢。
實施例2-健康的生物膜
進行類似實施例1的試驗步驟,測試其對抑制浮游生長及抑制生物膜生成的影響。
用於生物膜生成的菌種是:粘放線菌(ATCC 43146)、酪蛋白乳
酸桿菌(ATCC 334)、口腔鏈球菌(ATCC 35037)、小型範永氏菌(ATCC 17745)及核細梭菌(ATCC 10953)。菌株在37℃、無氧環境下生長72小時,所有細菌在無氧血液瓊脂上生長。
無氧血液瓊脂之配方如下:52克/升腦心輸液瓊脂(DIFCO),高壓蒸氣滅菌後加入下列產品:1毫升/升氯化血黑素(自5毫克/毫升溶液)、1毫升/升維他命K1(自100微克/毫升溶液)及50毫升馬血。使用細胞培養用24孔微量滴盤(Nalgene Nunc International,Denmark),在試管中產生牙齒生物膜。
微量滴盤的孔注入800微升之黏蛋白生長培養基(MGM),Kinniment等人所用一種修飾的培養基(Kinniment,S.L.,Wimpenny,J.W.,Adams,D.,and Marsh,P.D.(1996)Development of a steady-state oral microbial biofilm community using the constant-depth film fermenter.Microbiology 142(Pt 3):631-638)其存在高濃度的蛋白質:2.5克/升黏蛋白、5克/升心輸液培養液(DIFCO)、2.0克/升碳酸氫鈉、1.0克/升酵母萃取物及0.1克/升半胱胺酸及4毫升刃天青(自25毫克/100毫升溶液)。
在高壓蒸氣滅菌前調整pH至7.4,高壓蒸氣滅菌後加入下列產品:1毫升/升氯化血黑素(自5毫克/毫升溶液)及1毫升/升維他命K1(自100微克/毫升溶液)。而後將無菌的羥磷石灰圓盤7mm x 1.8mm(Clarkson Inc.,USA)置入孔中,為了生成後來的薄膜,在接種前,將圓盤與培養基接觸,在無氧條件下靜置至少60小時,此前-培養期可供確認可能的汙染,並調整最終pH值至7.5。
生長在瓊脂的每一菌株再懸浮於MGM培養基直至達成McFarlan 3的混濁度(約108
CFU/毫升)。最後,混和來自不同菌株各1毫升之懸浮液,將此懸浮液50微升加入微量滴定盤的每一個孔中,微量滴定盤在37℃無氧條件之無氧箱中培養,同時一併進行一個對照生長。將用來製備接種液之25微升各懸浮液滴入無氧血液瓊脂盤,並在
37℃無氧條件之無氧箱中培養,以確認存活度。
24小時後保存浮游相,測量在OD600之生長,且羥磷石灰圓盤以無菌PBS沖洗,並以番紅0.1%染色15分鐘,15分鐘後圓盤以蒸餾水沖洗兩次,以95%酒精/1%SDS溶液將番紅自經染色的生物膜中釋出,在492nm之吸光度,測定被生物膜所保留番紅的含量。
類似實施例1之細菌減少/生物膜抑制,DL-5-羥離胺酸鹽酸鹽顯示無浮游生長抑制,但亦顯示極小的生物膜抑制。相反的,D-甘露糖胺顯示有效的抑制生物膜生成,同時可預期的維持健康的微菌叢。
實施例3-致病性的生物膜
使用類似致病性生物膜模式進行實驗,生物膜以前揭健康生物膜之方式接種,開始細菌的貼附及生物膜的生成,讓其進行4小時,4小時後,每一生物膜加入100微升培養隔夜之牙齦吡咯單胞菌,在以胰蛋白酶處理前,後讓生物膜再生長48小時,並如前揭方法測定吸光度的。
整體來看,這些結果顯示去氧糖抗代謝物質抗菌劑,如6-去氧-D-葡萄糖及2-氟-2-去氧-D-葡萄糖是非常好的口腔生物膜生成抑制劑,同時顯示這個方法在疾病-造成微生物整合入生物膜,具有一增強的效
果,即具預防疾病發展的潛力,超越僅單純地殺死細菌。
實施例4-口腔護理組合物
本發明的一個代表性口腔護理組合物,以牙膏組合物描述如下表所示:
那些習於此技藝者可以理解,只要在不悖離本發明範圍的精神,在本文所揭示具體實例可做多種變化及修飾。這是預期的,所有這些變化都落在本發明所附申請專利範圍的範圍內。
Claims (28)
- 一種去氧糖抗代謝物質於製造含口腔可接受載劑而不含其他抗菌劑之口腔護理組合物之用途,該口腔護理組合物用於抑制病患口腔微生物生物膜的生成及/或分解微生物生物膜,其中存在組合物之去氧糖抗代謝物之質濃度介於0.005wt.%至0.7wt.%,以組合物總重量計;且其中該口腔可接受載劑為用於牙膏、牙用乳膏、漱口水、口香糖或假牙黏著劑之口腔可接受載劑,包括一或多種選自下列所組成之群組:抗牙菌斑劑、增白劑、清潔劑、香味劑、甜味劑、黏著劑、界面活性劑、泡沫調節劑、磨擦劑、pH改良劑、保濕劑、口感劑、著色劑、磨擦劑、結石控制(抗牙結石)劑、氟離子來源劑、唾液刺激劑、抗敏感劑、抗氧化劑、營養劑、黏度改良劑、遮光劑、呼吸清新劑及鋅鹽類及其組合物。
- 如申請專利範圍第1項之用途,其中去氧糖抗代謝物質是式(I)之化合物:
