Claims (1)

Способ комплексного хирургического лечения отслойки сетчатки; осложненной пролиферативной витреоретинопатией, включающий выполнение трансцилиарной витрэктомии, расправление сетчатки с помощью жидкого перфторорганического соединения (ПФОС), эндолазеркоагуляцию сетчатки и внутреннюю тампонаду, отличающийся тем, что витрэктомию начинают с выделения задней гиалоидной мембраны (ЗГМ), для этого с помощью инструмента исследуют зону ЗГМ вокруг диска зрительного нерва, выявляют место ее локальной отслойки, а при отсутствии, создают локальную отслойку ЗГМ с помощью пикообразного инструмента в носовой половине у диска зрительного нерва и наконечником витреотома производят дырчатый дефект в этой зоне, далее механически, с помощью инструмента, через сформированное отверстие перипапиллярно отслаивают ЗГМ и отделяют от диска зрительного нерва, затем в сформированную полость вводят ПФОС и путем дальнейшего введения ПФОС и механического воздействия инструментом на ЗГМ под прикрытием ПФОС, отслаивают ее до зубчатой линии, после чего с помощью витреотома иссекают стекловидное тело и отслоенную ЗГМ.A method for complex surgical treatment of retinal detachment; complicated proliferative vitreoretinopathy, including performing transciliary vitrectomy, straightening the retina with a liquid perfluororganic compound (PFOS), endolasercoagulation of the retina and internal tamponade, characterized in that vitrectomy is started by isolating the posterior hyaloid membrane (ZGM), for this, using the instrument of the optic nerve disc, locate the place of its local detachment, and in the absence, create a local detachment of ZGM with the help of a peak-like instrument in the nasal a hole in the optic nerve disk and the tip of the vitreotome produce a hole defect in this zone, then mechanically, using a tool, the DGM is peri-capillarily peeled off and separated from the optic disk, then PFOS is introduced into the cavity and further injection of PFOS and mechanical action of the instrument on ZGM under the cover of PFOS, exfoliate it to the dentate line, after which the vitreous and exfoliated ZGM are excised using a vitreotome.
