Поставленная задача решается предлагаемым способом, путем комбинированного доступа, резекции половины языка до уровня подъязычной кости с органной лимфоаденоэктомией и пластикой образовавшегося дефекта языка и дна полости рта. Комбинированный доступ начинают с разреза в подчелюстной области от срединной линии шеи до внутреннего края кивательной мышцы, отступя от края нижней челюсти на 1,5-2,0 см, обнажают подчелюстной футляр и клетчатку сонного треугольника, выделяют сосудистую ножку из лицевой артерии и вены, выкраивают на ней кожно-мышечный или слизисто-мышечный лоскут из щеки и отводят кнаружи, фасциальный футляр отделяют от нижней челюсти, определяют органные лимфоузлы, иссекают их с помощью устройства для лимфодиссекции в едином блоке с резецируемой частью языка и внеорганными лимфатическими сосудами, соединяющими язык с регионарными лимфатическими узлами, образовавшийся дефект замещают лоскутом на сосудистой ножке. Под интубационным наркозом производят разрез кожи в подчелюстной области от срединной линии шеи до внутреннего края кивательной мышцы, отступя от нижней челюсти на 1,5-2,0 см, рассекают поверхностную фасцию, подкожную мышцу, выделяют сосудистый пучок из лицевой артерии и вены у края нижней челюсти, формируют ножку кожно-мышечного или слизисто-мышечного лоскута, который выкраивают из щеки по размерам планируемого дефекта языка, отводят кнаружи, фасциальный футляр отделяют от нижней челюсти, в нем и сонном треугольнике определяют органные лимфоузлы, которые отделяют от ложа раны с помощью разработанного нами устройства для лимфодиссекции, сохраняя блочную связь с удаляемой частью языка, отсекают мышцы дна полости рта и языка от подъязычной кости и нижней челюсти на стороне поражения, со стороны полости рта производят известные разрезы и удаляемую часть языка выводят в рану на шее, уточняют границы опухоли и производят удаление препарата. В образовавшийся дефект в полости рта проводят под нижним краем нижней челюсти ранее сформированный лоскут на сосудистой ножке, который подшивают к оставшейся части языка и слизистой дна полости рта. Снаружи рана ушивается послойно.
