1. The method of dissection of failed perforating veins in severe forms of chronic venous insufficiency in elderly patients with a pronounced concomitant pathology, including the isolation of the perforating complex and the removal of pathologically altered vascular structures, characterized in that the isolation of the perforating complex and the removal of pathologically altered vascular structures is carried out under the control of equipment for optical magnification and instruments for microsurgery, skin and subcutaneous tissue at the site of surgery they are cut with a length of 10-12 mm, the perforating bundle is secreted to the fascia of the tibia or thigh, a perforating vein is isolated, for which a perforation of up to 3 mm is made between the perforating vein and other neurovascular elements, then the perforating vein is mobilized using the device according to .2, by carrying out careful movements of its working part inward to the superficial fascia and back out to the mouth of the perforating vein, then, the selected vein at a distance of 2-3 mm from the fascia is ligated with 6/0 thread and intersected, after that, also cross a proximal section of the vein is circulated at a distance of 1-2 mm from its superficial mouth, then the edges of the postoperative wound are adapted by applying 1-2 nodal skin sutures with 6/0 suture, and, further, all target insolvent perforants are processed in the manner described above. 2. 2. Device for mobilizing microvessels, characterized in that it contains a handle and a working part, while the working part is made in the form of a hook with a radius of curvature of 4 mm, the end of which is 10 mm long, is bent at an angle of 120 ° relative to the longitudinal axis of the device. 3. 3. The method according to p. 1, characterized in that during dissection of the failed perforating veins in the trophic ulcer, access is carried out in the zone of the least pathological changes in the skin at a distance of not more than 3.5 cm from the failed perforating vein.
