Abstract

Способ герниопластики пупочных грыж преимущественно первичных или рецидивных небольших и средних размеров, сочетающихся с околопупочными конусовидными диастазами I-III степени, включающий иссечение лоскута кожи и подкожной клетчатки в виде эллипса вокруг грыжевого выпячивания в области пупка и подготовку из него аутодермального трансплантата по методу Янова В.Н., иссечение грыжевого мешка и ушивание брюшины, отличающийся тем, что, с целью надежной консолидации брюшной стенки, при герниопластики грыж среднего размера первичных или рецидивных устраняют грыжевой дефект и околопупочные конусовидные диастазы, накладывая узловые швы в продольном направлении, рассекают передние стенки влагалищ прямых мышц с обеих сторон на расстоянии 1-2 см, сшивают медиальные края пересеченных влагалищ прямых мышц узловыми швами, которые при этом погружаются вглубь, а медиальные края прямых мышц сближаются до полного соприкосновения, затем сшивают латеральные края, пересеченных передних стенок влагалищ прямых мышц, узловыми швами и поверх укладывают аутодермальный трансплантат, фиксируют его в растянутом положении узловыми швами по периметру и средней линии к передним стенкам влагалищам прямых мышц, причем в случае образования дефекта стенки влагалищ (при диастазах III степени) его устраняют укладкой аутодермального трансплантата, фиксируя в стык узловыми швами по периметру к латеральным краям передних стенок влагалищ прямых мышц, а при герниопластики пупочных грыж небольших размеров (дистаз I-II степени) проводят продольное ушивание узловыми швами край в край грыжевого дефекта, накладывая второй ряд инвагинирующих швовMethod for hernioplasty of umbilical hernias of mainly primary or recurrent small and medium sizes, combined with umbilical conical diastases of I-III degree, including excision of a flap of the skin and subcutaneous tissue in the form of an ellipse around a hernial protrusion in the navel and preparation of an autodermal transplant from it according to the Yunov method.
