Description
本出願は、2016年8月30日出願の米国仮特許出願第62/381,423号の利益を主張し、その出願は、参照により本明細書に全体として組み込まれる。

本開示は、生物医学的システムをターゲティングする方法を提供する。「生物医学的システム」は、被験対象の処置、被験対象の状態の診断、被験対象に実施される生物医学的調査などを含むがこれらに限定はされず、医療用途又は治療用途で使用される構成要素の任意の装置又はシステムを意味する。本方法の態様は、排他的にではないが一般に、被験対象の表面にターゲティング装置を配置することと、被験対象上又は被験対象内での医療装置の適切な配置のためにターゲティング装置の軌道に依存する1つ又は複数の下流側用途のために、ターゲティング装置の軌道を確立、位置合わせ、及び/又は調節することとを含む。場合によっては、本方法の態様は、たとえば薬物デリバリー装置、ウイルスベクターデリバリー装置、ナノ粒子デリバリー装置、細胞投与デリバリー装置、細胞デリバリー装置などを含むがこれらに限定はされず、治療物質投与装置用の保持器として、本明細書に記載の装置又はその構成要素を利用することも含むことがある。本開示のターゲティング装置の実際の構成は様々であることになる。
本方法の態様は、場合によっては、たとえば上述のターゲティング装置の1つ又は複数のターゲティングしたチャネルを使用した、被験対象の所望の領域への磁気共鳴画像(MRI)支援デリバリーを含む。任意の所望の治療薬剤又は治療用装置が、本明細書に記載の方法に従って、被験対象の所望の領域にターゲティング及びデリバリーされ得る。本明細書に記載のターゲティングしたチャネルを通じてデリバリーすることができる薬剤及び治療用装置の非限定的な例には、たとえば薬物、ナノ粒子、生物由来物質(たとえば細胞、ウイルスなど)、電気プローブ(たとえば電極)、サーマルプローブ(たとえばヒートプローブ、コールドプローブなど)、撮像装置(たとえば内視鏡、ライトなど)、手術用具などが含まれるが、これらに限定はされない。
本開示は、本明細書に記載の方法を実施する際に有用な装置及びシステムを含め、被験対象の所望の領域に生物医学的装置又は治療薬をターゲティングする方法において有用な装置及びシステムを提供する。本明細書に記載の装置は、軌道ガイド、調節可能ターレットターゲティングシステム、及びその構成要素を含む。場合によっては、記載のシステムは、撮像装置、生物医学的システム、コンピュータ支援手術ナビゲーションシステムなどを含んでもよい。記載の装置、システム、及びその構成要素は、適宜、手動で制御されてもよく、本明細書においてより詳細に説明するように、完全に又は部分的に自動化されてもよい。
本開示の態様は、ターゲティングした治療薬を被験対象にデリバリーするための1つ又は複数の生物医学的システムをさらに含み得るターゲティング装置を含む。当該生物医学的システムは、その装置又はその動作可能部分が、本明細書に記載のターゲティング装置のチャネルを通って物理的に投入され得る限り、上述の治療方法のいずれかをデリバリー又は実施するための任意の適した装置を含むことになる。
本開示は、本明細書に記載の方法のための命令を記憶する、非一時的コンピュータ可読媒体を含めたコンピュータ可読媒体を含む。本開示の態様は、コンピューティング装置によって実行されたときに本明細書に記載の方法の1つ又は複数のステップをコンピューティング装置に実施させる命令を記憶する、コンピュータ可読媒体を含む。

条項49:遠位端部、球形端部、及び前記遠位端部から前記球形端部へと走る1つ又は複数のチャネルを備える調節可能ターレットと、
台座であって、
前記球形端部を受容するような寸法にされたソケットを形成する複数の環状壁部、
前記環状壁部の外表面のねじ切り部、
前記複数の環状壁部の間に配置される複数のスロット、並びに
被験対象の組織表面に前記台座を固定するための、前記環状壁部のうちの少なくとも1つと直交するフランジ
を備える台座と、
前記台座の前記外表面の前記ねじ切り部と適合可能な内表面のねじ切り部を備えるロッキングカラーとを備え、
前記ロッキングカラーを第1の方向に回すことにより、前記球形端部が圧縮されて、前記調節可能ターレットが所望の軌道にロックされ、前記ロッキングカラーを第2の方向に回すことにより、前記球形端部が除圧されて、前記調節可能ターレットの前記軌道の再ターゲティングが可能になる、調節可能ターゲティングシステム。
条項50:前記ロッキングカラーを前記第1の方向に回すことにより、前記台座と前記ロッキングカラーの間で前記球形端部が圧縮されて、前記調節可能ターレットが所望の軌道にロックされる、条項49に記載のシステム。
条項51:前記ロッキングカラーを前記第1の方向に回すことにより、前記ソケットの前記複数の環状壁部の間で前記球形端部が圧縮されて、前記調節可能ターレットが所望の軌道にロックされる、条項49又は50に記載のシステム。
条項52:前記調節可能ターゲティングシステムが、前記被験対象の前記組織表面に固定されたとき前記台座及び前記ロッキングカラーが生体外になるように構成される、条項49から51のいずれかに記載のシステム。
条項53:前記調節可能ターゲティングシステムが、前記被験対象の前記組織表面に固定されたとき前記調節可能ターレットが生体外になるように構成される、条項49から52のいずれかに記載のシステム。
条項54:前記球形端部が、前記1つ又は複数のチャネルへの開口を備えた平坦な部分を前記遠位端部に対向して備える、条項49から53のいずれかに記載のシステム。
条項55:前記球形端部及び前記平坦な部分が、前記ソケットに挿入されたとき、前記平坦な部分が前記台座の前記底表面と同一平面になるような寸法にされる、条項54に記載のシステム。
条項56:前記ロッキングカラーが、把持を可能にするローレット加工された外表面を備える、条項49から55のいずれかに記載のシステム。
条項57:前記台座が、前記環状壁部のうちの少なくとも1つに直交する複数のフランジを備える、条項49から56のいずれかに記載のシステム。
条項58:前記調節可能ターレットが、複数の平行なチャネルを備える、条項49から57のいずれかに記載のシステム。
条項59:前記調節可能ターレットが、複数の平行でないチャネルを備える、条項49から58のいずれかに記載のシステム。
条項60:条項31から41のいずれかに記載の軌道ガイドをさらに備える、条項49から59のいずれかに記載のシステム。

前記調節可能ターレットの前記1つ又は複数のチャネルに挿入されるような寸法にされたデリバリー装置又は電極とを備える、
調節可能なターゲティングデリバリーシステム。
条項63:前記デリバリー装置又は電極が、前記デリバリー装置又は電極の長手方向に沿ったある点に配置される深さ止めを備えて、前記デリバリー装置が前記の点を越えて前記1つ又は複数のチャネルに挿入されることを防止する、条項62に記載のシステム。
条項64:命令を記憶する非一時的コンピュータ可読媒体であって、前記命令は、コンピューティング装置によって実行されたとき、
調節可能ターレットのチャネルに挿入された軌道ガイドのMRI可視針の磁気共鳴画像(MRI)を受け取るステップと、
前記受け取ったMRIに基づいて、前記チャネルの軌道を決定するステップと、
前記決定した軌道を所望のユーザ入力軌道と比較するステップと、
前記比較するステップに基づいて、前記決定した軌道を前記所望のユーザ入力軌道に位置合わせするのに必要な、前記調節可能ターレットの推奨調節を計算するステップと、
前記推奨調節を表示するステップとを前記コンピューティング装置に実施させる、
非一時的コンピュータ可読媒体。
条項65:自動化された調節可能ターゲティングシステムであって、
条項62又は63に記載の調節可能ターゲティングシステムと、
前記調節可能ターレットに接続され、命令でプログラムされたプロセッサによって制御されるアクチュエータとを備え、
前記命令は、前記プロセッサによって実行されたとき、前記プロセッサに、
前記チャネルに挿入された軌道ガイドMRI可視針の、受け取った磁気共鳴画像(MRI)に基づいて、前記調節可能ターレットのチャネルの軌道を決定させ、
前記決定した軌道を所望のユーザ入力軌道と比較させ、
前記比較に基づいて、前記決定した軌道を前記所望のユーザ入力軌道に位置合わせするのに必要な、前記調節可能ターレットの調節を計算させ、
前記調節を実行するように前記アクチュエータをトリガし、それによって前記決定した軌道を前記所望のユーザ入力軌道に位置合わせさせる、システム。
Claims (22)
- 調節可能ターレット、台座、及び軌道ガイドを備える、被験対象の所望領域の磁気共鳴画像(MRI)支援ターゲティング用の調節可能ターゲティングシステムであって、
前記調節可能ターレットは、遠位端部、球形端部、及び前記遠位端部から前記球形端部へと走る複数のチャネルを備え、
前記台座は、
前記球形端部を受容するような寸法にされたソケットを形成する複数の環状壁部、
前記複数の環状壁部の間に配置される複数のスロット、及び、
被験対象の組織表面に前記台座を固定するための、前記環状壁部のうちの少なくとも1つと直交するフランジを備え、
前記軌道ガイドは、
平坦な表面を備える基部(または固体支持体)と、
各々が造影剤を含む内部空洞を備える複数のMRI可視針とを備え、
前記複数のMRI可視針の各々が、一方の端部で前記平坦な表面に固定され、複数のチャネルを有する調節可能ターレットの各々のチャネルに挿入されるような寸法にされ、前記調節可能ターレットは被験対象の組織表面に固定され、それにより、MRI画像装置を使用して前記挿入されたMRI可視針の各々の軌道を画像化することにより、前記チャネルをターゲティングすることが可能になり、
前記チャネルは、一方の端部が軌道ガイドの基部の平坦な表面に固定された複数のMRI可視針がチャネルに挿入されるように設けられている、調節可能ターゲティングシステム。 - ロッキングカラーを更に備え、
前記ロッキングカラーは、前記台座の外表面に設けられるねじ切り部と適合可能なねじ切り部を内表面に備え、
前記ロッキングカラーを第1の方向に回すことにより、前記球形端部が圧縮されて、前記調節可能ターレットが所望の軌道にロックされ、前記ロッキングカラーを第2の方向に回すことにより、前記球形端部が除圧されて、前記調節可能ターレットの軌道の再ターゲティングが可能になり、
前記ロッキングカラーを前記第1の方向に回すことにより、前記台座と前記ロッキングカラーの間で前記球形端部が圧縮されて、前記調節可能ターレットが所望の軌道にロックされる、請求項1に記載のシステム。 - ロッキングカラーを更に備え、
前記ロッキングカラーは、前記台座の外表面に設けられるねじ切り部と適合可能なねじ切り部を内表面に備え、
前記ロッキングカラーを第1の方向に回すことにより、前記球形端部が圧縮されて、前記調節可能ターレットが所望の軌道にロックされ、前記ロッキングカラーを第2の方向に回すことにより、前記球形端部が除圧されて、前記調節可能ターレットの軌道の再ターゲティングが可能になり、
前記ロッキングカラーを前記第1の方向に回すことにより、前記ソケットの前記複数の環状壁部の間で前記球形端部が圧縮されて、前記調節可能ターレットが所望の軌道にロックされる、請求項1に記載のシステム。 - 前記調節可能ターゲティングシステムが、前記被験対象の前記組織表面に固定されたとき前記台座及び前記ロッキングカラーが生体外になるように構成される、請求項2又は3に記載のシステム。
- 前記調節可能ターゲティングシステムが、前記被験対象の前記組織表面に固定されたとき前記調節可能ターレットが生体外になるように構成される、請求項1に記載のシステム。
- 前記球形端部が、前記遠位端部に対して反対側に、1つ又は複数のチャネルへの開口を備えた平坦な部分を備える、請求項1に記載のシステム。
- 前記球形端部及び前記平坦な部分が、前記ソケットに挿入されたとき、前記平坦な部分が前記台座の底表面と同一平面になるような寸法である、請求項6に記載のシステム。
- 前記ロッキングカラーが、把持を可能にするローレット加工された外表面を備える、請求項2に記載のシステム。
- 前記ロッキングカラーが、把持を可能にするローレット加工された外表面を備える、請求項3に記載のシステム。
- 前記台座が、前記環状壁部のうちの少なくとも2つに直交する複数のフランジを備える、請求項1に記載のシステム。
- 前記複数のMRI可視針が、前記平坦な表面の幾何中心に対して対称に前記平坦な表面に固定される2本以上の針を備える、請求項1に記載のシステム。
- 前記複数のMRI可視針が、前記複数のMRI可視針のうちの1本又は複数本の針に対して非対称に前記平坦な表面に固定される少なくとも1本の針を備える、請求項1に記載のシステム。
- 少なくとも1つのMRI可視針が、前記平坦な表面に対して垂直に固定される、請求項1に記載のシステム。
- 少なくとも1つのMRI可視針が、前記平坦な表面に対して広がった角度で固定される、請求項1に記載のシステム。
- 前記基部が、前記平坦な表面に対向して開口を備え、前記開口は、前記MRI可視針の前記内部空洞と一続きである、前記基部の中の空隙に隣接し、それにより、前記空隙及び前記内部空洞へのアクセスが可能になる、請求項1に記載のシステム。
- 前記開口を閉じるためのキャップをさらに備える、請求項15に記載のシステム。
- 前記キャップと前記開口が、適合可能なねじ切り部を備える、請求項16に記載のシステム。
- 前記造影剤が、ガドリニウムを含む、請求項1に記載のシステム。
- MRI画像装置を更にそなえ、当該MRI画像装置は、前記軌道ガイドのMRI可視針が前記調節可能ターレットのチャネルに挿入されたときに前記MRI可視針を撮像するように配置される、請求項1に記載のシステム。
- 請求項1から19のいずれか一項に記載の調節可能ターゲティングシステムと、
前記調節可能ターレットの複数のチャネルの中のチャネルに挿入されるような寸法にされたデリバリー装置又は電極とを備える、
調節可能ターゲティングデリバリーシステム。 - 前記デリバリー装置又は電極が、前記デリバリー装置又は電極の長手方向に沿ったある点に配置された深さ止めを備え、前記デリバリー装置が前記の点を越えて前記1つ又は複数のチャネルに挿入されることが防止される、請求項20に記載のデリバリーシステム。
- 請求項20に記載の調節可能ターゲティングデリバリーシステムと、
前記調節可能ターレットに接続され、命令でプログラムされたプロセッサによって制御されるアクチュエータとを備え、
前記命令は、前記プロセッサによって実行されたとき、前記プロセッサに、
前記チャネルに挿入された軌道ガイドのMRI可視針の、受け取った磁気共鳴画像(MRI)に基づいて、前記調節可能ターレットのチャネルの軌道を決定させ、
前記決定した軌道を所望のユーザ入力軌道と比較させ、
前記比較に基づいて、前記決定した軌道を前記所望のユーザ入力軌道に位置合わせするのに必要な、前記調節可能ターレットの調節を計算させ、
前記調節を実行するように前記アクチュエータをトリガし、それによって前記決定した軌道を前記所望のユーザ入力軌道に位置合わせさせる、
自動化された調節可能ターゲティングシステム。
