Abstract

Description
Disposición de cavidades receptoras de grapas para grapadora quirúrgica
La presente invención se refiere a una cavidad receptora de grapas. Más específicamente, la presente invención se refiere a una disposición de cavidades receptoras de grapas para uso con una grapadora quirúrgica.
Las grapadoras quirúrgicas emplean típicamente un yunque que tiene cavidades receptoras de grapas definidas en él. Las grapas se empujan fuera de un cartucho de grapas a través de una sección de tejido y contra las cavidades receptoras de grapas, estando configuradas las cavidades receptoras de grapas para recibir y plegar progresivamente las patillas de la grapa hacia una posición cerrada. La figura 1 es una vista superior de una porción de disposiciones de cavidades receptoras de grapas convencionales en el yunque de una grapadora quirúrgica, por ejemplo una primera disposición en el lado superior de la ranura de cuchilla y una segunda disposición en el lado inferior de la ranura de cuchilla. Las cavidades receptoras de grapas convencionales son de forma típicamente rectangular y pueden estar dispuestas en filas paralelas.
La figura 2(a) es una vista en perspectiva inferior de una disposición de cavidades receptoras de grapas convencional en el yunque de una grapadora quirúrgica. Esta disposición de cavidades receptoras de grapas emplea una pared de cañón empinada cerca del suelo del cañón, la cual cambia a un ángulo muy pequeño para el resto de la pared del cañón. La figura 2(b) es una vista en sección transversal de una porción de la cavidad receptora de grapas ilustrada en la figura 2(a). La figura 2(b) que ilustra la patilla de grapa está siendo recibida dentro de la cavidad receptora de grapas, por ejemplo, moviéndose en una dirección vertical, y antes de que la patilla de grapa sea plegada a una posición cerrada. La disposición de cavidades receptoras de grapas emplea una pared de cañón empinada cerca del suelo del cañón, que cambia a un ángulo muy pequeño para el resto de la pared del cañón. El ángulo de 37,4 grados mostrado en las figuras 3(a) y 3(b) es el ángulo de la superficie de la cavidad receptora de grapas con respecto al plano de la superficie del yunque, por ejemplo el ángulo de la pendiente de la superficie a lo largo de la cual se desliza la patilla de grapa cuando la patilla de grapa es recibida inicialmente dentro de la cavidad receptora de grapas en una localización aproximadamente a 0,127 mm (0,005 pulgadas) del borde longitudinal de la cavidad receptora de grapas.
La figura 4 es una vista superior de una porción de otra disposición de cavidades receptoras de grapas convencional en el yunque de una grapadora quirúrgica. En esta disposición, existen tres filas longitudinales de las cavidades receptoras de grapas situadas en cada lado de la ranura de cuchilla.
Un problema que puede aparecer en disposiciones de cavidades receptoras de grapas quirúrgicas es que las cavidades receptoras de grapas tengan esquinas internas afiladas que puedan contribuir a estorbar o atascar la patilla de grapa de una grapa cuando la patilla de grapa se mueve progresivamente a través de la cavidad receptora de grapas. Otro problema que puede aparecer en disposiciones de cavidades receptoras de grapas quirúrgicas convencionales es que las cavidades receptoras de grapas tengan un área de captura estrecha, de tal manera que las grapas que están ligeramente desalineadas con respecto a las cavidades receptoras de grapas puedan perder la cavidad. Otro problema que se puede encontrar en disposiciones de cavidades receptoras de grapas quirúrgica convencional es que las cavidades de rapa tengan una pendiente muy pequeña extendida sobre un área amplia de modo que las patillas de grapa entrantes no encuentren un ángulo de deslizamiento suficientemente pronunciado, provocando así que las patillas de grapa se atasquen y se comben.
Las características precaracterizantes de la reivindicación 1 se conocen por el documento US 3.494.533.
Según una realización ejemplar de la presente invención, una disposición de cavidades receptoras de grapas en la porción de yunque de un dispositivo grapador quirúrgico incluye pares de cavidades receptoras de grapas correspondientes cada una pluralidad de grapas que se han de cerrar. El par de cavidades receptoras de grapas pueden disponerse a lo largo de una línea central. Una cavidad receptora de grapas distal puede disponerse para recibir y cerrar una patilla más distal de una grapa, y una cavidad receptora de grapas proximal puede disponerse para recibir y cerrar una patilla más proximal de una grapa. La cavidad receptora de grapas distal puede ser una imagen especular de la cavidad receptora de grapas proximal.
Las cavidades receptoras de grapas pueden tener una forma generalmente triangular cuando se ven desde arriba. Cada patilla de grapa respectiva se recibe en un extremo longitudinal de la cavidad receptora de grapas que proporciona un área objetivo relativamente amplia para recibir la patilla de grapa con el fin de eliminar, o al menos minimizar, la probabilidad de que una patilla de grapa pierda la cavidad receptora de grapas debido, por ejemplo, a desalineación entre una primera mordaza de la grapadora quirúrgica que tiene el yunque y una segunda mordaza de la grapadora quirúrgica que tiene un cartucho configurado para disparar las grapas. Además, cada una de las cavidades receptoras de grapas es más estrecha en su extremo opuesto, por ejemplo, el extremo en el cual la patilla de grapa emerge después de haberse transformado en un arco por el suelo de cañón curvado de la cavidad receptora de grapas. La cavidad 110 receptora de grapas puede proporcionar paredes de cañón, por ejemplo, a lo largo de las cuales se guía una patilla de grapa, que están inclinadas empinadamente en toda la pared de modo que la patilla de grapa se desplace hacia y a lo largo del centro del cañón y luego hacia arriba y hacia fuera del extremo estrecho del cañón. El suelo del cañón puede tener una curvatura generalmente suave y continua que proporciona, durante el plegado de la patilla de grapa, unos radios de curvatura suficientemente grandes para eliminar, o al menos minimizar, esquinas muy pronunciadas que puedan estorbar o bloquear las patillas de grapa que se mueven a lo largo de la superficie.
Cada cavidad receptora de grapas de una primera fila de cavidades receptoras de grapas está anidada en una cavidad receptora de grapas de una fila adyacente de cavidades receptoras de grapas. De esta manera, cuando cada fila de cavidad receptora de grapas está decalada longitudinalmente de otra fila, por ejemplo, en aproximadamente una mitad del paso de centro a centro del par de cavidades (o de las grapas) la cavidad receptora de grapas proximal de una primera fila se anida en una cavidad receptora de grapas distal de una segunda fila. La disposición de cavidades receptoras de grapas puede ser más tolerante de grapas pobremente orientadas, puede hacer un uso más eficiente del espacio en el yunque y puede haber una menor probabilidad de provocar un atasco de grapas o un combado de la patilla de grapa.

La figura 5 es una vista superior de una disposición de cavidades receptoras de grapas sobre el yunque de una grapadora quirúrgica, según una realización ejemplar de la presente invención. Específicamente, la figura 5 ilustra una superficie 100 del yunque que tiene un primer lado 100a y un segundo lado 100b separados por una ranura de cuchilla. En cada uno del primer lado 100a y del segundo lado 100b de la superficie 100 del yunque se definen dos filas de cavidades 110 receptoras de grapas. Haciendo referencia al primer lado 100a de la superficie 100 del yunque, una primera fila de cavidades 110 receptoras de grapas tiene una línea central 101a y una segunda fila de cavidades 110 receptoras de grapas tiene una línea central 101b. Con fines de claridad, sólo se han mostrado varias cavidades 110 receptoras de grapas. Sin embargo, la superficie 100 del yunque puede tener cualquier número de cavidades 100 de grapa en cada una de las filas. Además, aunque las realizaciones ejemplares descritas en el presente documento incluyen filas de cavidades receptoras de grapas que están dispuestas adyacentes a una ranura de cuchilla, las realizaciones ejemplares de la presente invención pueden emplearse en relación con cualquier tipo de grapadora quirúrgica, por ejemplo con o sin ranura de cuchilla o cualquier otra característica estructural.
La superficie 100 del yunque está dispuesta con pares de cavidades 110 receptoras de grapas correspondientes a cada grapa que se ha de cerrar. Por ejemplo, haciendo referencia al par de cavidades 110 receptoras de grapas dispuesto a lo largo de la línea central 101a, se proporciona una cavidad receptora de grapas distal para recibir y cerrar una patilla más distal de una grapa, y una cavidad receptora de grapas proximal para recibir y cerrar una patilla más proximal de una grapa. La cavidad 110 receptora de grapas distal puede ser una imagen especular de la cavidad 110 receptora de grapas proximal.
Las cavidades 110 receptoras de grapas pueden tener diversas formas. Según se muestra en la figura 5, cada cavidad 110 receptora de grapas puede tener una forma aproximadamente triangular cuando se ve desde arriba. Alternativamente, cada cavidad 110 receptora de grapas puede tener una forma, cuando se le ve desde arriba, que se parece a un "asiento de bicicleta", por ejemplo al ser generalmente triangular y tener una serie de curvas convexas y cóncavas a lo largo de sus lados. Asimismo, pueden emplearse otras formas.
Cada patilla de grapa respectiva es recibida en un extremo longitudinal de la cavidad 110 receptora de grapas que proporciona un área objetivo relativamente ancha para recibir la patilla de grapa. Dado que la cavidad 110 receptora de grapas es relativamente ancha en el extremo longitudinal en el cual se recibe la partilla de grapa, la disposición de cavidades receptoras de grapas puede eliminar, o al menos minimizar, la probabilidad de que una patilla de grapa pierda la cavidad receptora de grapas debido a, por ejemplo, desalineación entre una primera mordaza de la grapadora quirúrgica que tiene el yunque y una segunda mordaza de la grapadora quirúrgica que tiene un cartucho configurado para disparar las grapas.
Cada una de las cavidades 110 receptoras de grapas es más estrecha en su extremo opuesto, por ejemplo el extremo en el que la patilla de grapa sobresale después de haberse transformado en un arco por el cañón curvado de la cavidad 110 receptora de grapas. La cavidad 110 receptora de grapas proporciona guiado a la grapa porque las paredes del cañón a lo largo de las cuales se guía una patilla de grapa están fuertemente inclinadas en toda la pared (véase, por ejemplo, la figura 6) de modo que la patilla de grapa se desplaza hacia y a lo largo del centro del cañón y luego hacia arriba y hacia fuera del extremo estrecho alejado del cañón. El suelo del cañón tiene una curvatura generalmente suave y continua que proporciona el plegado a la patilla de grapa, como se muestra, por ejemplo, en la figura 7. Alternativa o adicionalmente, el suelo del cañón puede tener radios cambiantes. Las superficies de las cavidades receptoras de grapas están unidas entre sí con radios suficientemente grandes para eliminar, o al menos minimizar, las esquinas muy pronunciadas que puedan estorbar o atascar las patillas de grapa que se mueven a lo largo de la superficie. Además, el ángulo compuesto entre estas superficies proporciona una rampa de deslizamiento suficientemente empinada para que la sigan las patillas de grapa. Las patillas siguen estas rampas de deslizamiento descendiendo dentro del cañón para un plegado o conformado adecuados incluso cuando la grapa no es orientada correctamente por el cartucho.
Dado que este extremo opuesto de la cavidad 110 receptora de grapas es relativamente estrecho, cada cavidad 110 receptora de grapas de una primera fila de cavidades 110 receptoras de grapas puede estar anidada en una cavidad receptora de grapas de una fila adyacente de cavidades receptoras de grapas. Cada fila de cavidades receptoras de grapas (y las grapas correspondientes) pueden estar decaladas longitudinalmente con respecto a otra fila, por ejemplo en aproximadamente la mitad del paso de centro a centro del par de cavidades (o de las grapas). De este modo, la cavidad receptora de grapas proximal de una primera fila se anida en una cavidad receptora de grapas distal de una segunda fila según se muestra en la figura 5. En una realización ejemplar, véase, por ejemplo en la figura 8, puede disponerse una tercera fila de cavidades receptoras de grapas, en donde la cavidad receptora de grapas distal de la segunda fila también se anide en la cavidad receptora de grapas proximal de la tercera fila.
Una serie de pares de cavidades receptoras de grapas se forma en el yunque de un dispositivo grapador quirúrgico. El número de los pares de cavidades receptoras de grapas y su localización dependen del patrón de grapas deseado. Típicamente, existirán varias filas de cavidades dispuestas longitudinalmente, por ejemplo paralelas entre sí. Por ejemplo, los lados de la cavidad receptora de grapas, por ejemplo aquellas porciones entre los extremos longitudinales interior y exterior, pueden estar inclinadas con respecto a una línea central de la fila. En tal disposición, cuando la cavidad receptora de grapas de la primera fila de las cavidades receptoras de grapas se anida en la cavidad de grapa de la segunda fila de cavidades receptoras de grapas, los lados adyacentes de las cavidades receptoras de grapas pueden ser generalmente paralelos entre sí.
La sección de tejido se sujeta entre el yunque y un cartucho cargado con grapas. Cada grapa del cartucho está alineada generalmente con un par correspondiente de cavidades receptoras de grapas. Tras el disparo, las grapas se empujan fuera del cartucho de modo que las que las patillas de las grapas penetren la sección de tejido y avancen dentro de las cavidades receptoras de grapas respectivas. El funcionamiento continuo del mecanismo de disparo de grapas hace que las patillas de grapa sean recibidas dentro del extremo ancho de la cavidad 110 receptora de grapas y que se deslicen a lo largo del valle curvado de la cavidad para plegarse o conformarse según la curvatura de la cavidad receptora de grapas. Finalmente, las patillas de cada grapa se pliegan totalmente o se conforman de tal manera que la sección de tejido se mantenga entre la espina de la grapa y las patillas de grapa plegadas.
La disposición de cavidades receptoras de grapas quirúrgicas puede proporcionar ventajas frente a las disposiciones de cavidades receptoras de grapas de dispositivos grapadores quirúrgicos convencionales. Por ejemplo, la presente disposición de cavidades receptoras de grapas puede proporcionar un área de captura de patilla de grapa entrante que sea más tolerante frente a grapas pobremente orientadas, por ejemplo que sea capaz de recibir y plegar efectivamente una patilla de grapa que esté ligeramente desalineada con respecto a la línea central de la cavidad receptora de grapas. Esto es debido al menos en parte al área de captura ancha situada en el extremo longitudinal exterior de las cavidades receptoras de grapas y a la forma generalmente triangular de las cavidades receptoras de grapas. De este modo, una característica de las presentes cavidades receptoras de grapas es la disposición de una área de captura ancha en un extremo de la cavidad receptora de grapas, la cual permite un funcionamiento efectivo del dispositivo grapador quirúrgico incluso para patillas de grapa que lleguen descentradas debido a un grado razonable de desalineación entre el yunque y el cartucho de grapas. Otra característica de las mismas es que las esquinas afiladas, que tienden a rozar con los extremos de las patillas de grapa, se eliminan o al menos se minimizan. Otra característica de las mismas es que con independencia de por dónde llegue la patilla de grapa a través del extremo amplio de la cavidad, existe un ángulo de deslizamiento o una pendiente suficientemente empinada de modo que se induzca a la patilla de grapa a seguir la curvatura formadora de la cavidad. Las paredes laterales empinadas de la cavidad receptora de grapas funcionan guiando la patilla de grapa de vuelta hacia el centro de la grapa a medida que las patillas de grapa se pliegan o se conforman, con independencia de la llegada descentrada de la patilla de grapa. Filas de estas cavidades pueden anidarse impecablemente de manera longitudinal unas en otras en estrecha proximidad, por ejemplo cuando se produce un desplazamiento de fase de aproximadamente una mitad del paso de pares de cavidades. En resumen, la presente disposición puede proporcionar, con respecto a disposiciones de cavidades receptoras de grapas convencionales, un área de captura de entrada de patilla de grapa más ancha, un ángulo de deslizamiento más empinado para inducir a la grapa a que comienza a conformarse, una forma que permite que se aniden eficientemente las filas y una disposición en la que los radios de curvatura de las cavidades receptoras de grapas sean más amplios que el radio del alambre de grapa. La disposición de cavidades receptoras de grapas puede eliminar, o al menos minimizar, esquinas afiladas que puedan tender a rozar con las grapas. De este modo, la disposición de cavidades receptoras de grapas puede ser más tolerante frente a grapas pobremente orientadas, hacer un uso más eficiente del espacio del yunque y tener menor propensión a provocar un atasco o combado de grapas.
Claims (14)
- REIVINDICACIONES1. Un dispositivo grapador quirúrgico para cerrar al menos una grapa que tiene una patilla de grapa distal y una patilla de grapa proximal, comprendiendo el dispositivo:una porción (100) de yunque que incluye al menos un par de cavidades (110) receptoras de grapas, incluyendo el al menos un par de cavidades receptoras de grapas una cavidad receptora de grapas distal para recibir y cerrar la patilla de grapa distal y una cavidad receptora de grapas proximal para recibir y cerrar la patilla de grapa proximal;en donde la porción de yunque incluye al menos dos filas de cavidades receptoras de grapas, incluyendo cada uno de las al menos dos filas al menos un par de cavidades receptoras de grapas; y en donde las cavidades (110) receptoras de grapas de la primera fila están decaladas longitudinalmente con respecto a las cavidades receptoras de grapas de la segunda fila; ycaracterizado porque una cavidad receptora de grapas de una primera fila de cavidades receptoras de grapas está anidada en una cavidad receptora de grapas de una segunda fila de cavidades receptoras de grapas.

- El dispositivo grapador según la reivindicación 1, en el que en cada una del al menos un par de cavidades (110) receptoras de grapas una cavidad receptora de grapas proximal es una imagen especular de un cavidad receptora de grapas distal.

- El dispositivo grapador según la reivindicación 1, en el que cada una de las cavidades (110) receptoras de grapas incluye un suelo de cañón curvado que conforma una patilla de grapa respectiva como un arco a medida que la patilla de grapa es recibida y empujada hacia dentro del suelo de cañón curvado de la cavidad receptora de grapas.

- El dispositivo grapador según la reivindicación 1, en el que cada cavidad (110) de grapa incluye paredes a lo largo de las cuales se guía una patilla de grapa, estando fuertemente inclinadas las paredes.

- El dispositivo grapador quirúrgico según la reivindicación 3, en el que el suelo del cañón tiene una curvatura generalmente suave y continua, preferiblemente en donde el suelo de cada cavidad (110) de grapa incluye radios de curvatura que al menos minimizan esquinas pronunciadas que pueda rozar o estorbar a las patillas de grapa que se estén moviendo a lo largo de la superficie durante el plegado de una patilla de grapa respectiva.

- El dispositivo grapador quirúrgico según la reivindicación 1, en el que una primera de entre unas cavidades (110) receptoras de grapas distal y proximal de una primera fila se anida en una segunda de entre unas cavidades receptoras de grapas distal y proximal de una segunda fila.

- El dispositivo grapador quirúrgico según la reivindicación 1, en el que las cavidades (110) receptoras de grapas de la primera fila están decaladas longitudinalmente con respecto a las cavidades receptoras de grapas de la segunda fila un medio paso de centro a centro de un par de cavidades receptoras de grapas.

- El dispositivo grapador quirúrgico según la reivindicación 1, en el que, para cada una de las cavidades (110) receptoras de grapas, un extremo longitudinal exterior de la cavidad receptora de grapas es más ancho que un extremo longitudinal interior de la cavidad receptora de grapas, preferiblemente en el que el extremo longitudinal exterior de cada cavidad receptora de grapas está configurado para recibir en primer lugar una patilla de grapa respectiva.

- El dispositivo grapador quirúrgico según la reivindicación 1, en el que, en cada fila de las al menos dos filas, las cavidades (110) receptoras de grapas están dispuestas a lo largo de una línea central (101a); (101b).

- El dispositivo grapador quirúrgico según la reivindicación 1, en el que los lados de la cavidad (110) de grapa entre los extremos longitudinales interior y exterior están inclinados con respecto a la línea central (101a); (101b).

- El dispositivo grapador quirúrgico según la reivindicación 1, en el que, cuando la cavidad (110) de grapa de la primera fila de cavidades receptoras de grapas se anida en la cavidad receptora de grapas de la segunda fila de cavidades receptoras de grapas, los lados adyacentes de las cavidades receptoras de grapas son generalmente paralelos uno a otro.

- El dispositivo grapador quirúrgico según cualquiera de las reivindicaciones precedentes, en el que cada cavidad
(110) de grapa del al menos un par de cavidades receptoras de grapas tiene una forma generalmente triangular cuando se la ve desde arriba. -

- El dispositivo grapador quirúrgico según la reivindicación 1, en el que, para cada una de las cavidades receptoras de grapas, un extremo longitudinal exterior de la cavidad receptora de grapas es más ancho que un extremo longitudinal interior de la cavidad receptora de grapas, preferiblemente en el que el extremo longitudinal de cada cavidad receptora de grapas está configurado para recibir en primer lugar una patilla de grapa respectiva.

- Un yunque para un dispositivo grapador quirúrgico, siendo el yunque según se define en cualquiera de las reivindicaciones precedentes.
