TIJDELIJK-FILTERCATHETER
De uitvinding heeft betrekking op een tijde-lijk-filtercatheter zoals bijvoorbeeld toegepast wordt bij een trombolytische behandeling. Het filter wordt 10 daarbij in de vena cava aangebracht en voorkomt dat bloedtromben zich vrij door het bloedvatstelsel kunnen verspreiden en op plekken terecht zouden kunnen komen die een gevaar voor de gezondheid van de patiënt met zich meebrengen.
15 Voor een bepaalde groep van patiënten waarbij dergelijke ingrepen hoge risico's met zich meebrengen kan het nodig zijn een dergelijk filter gedurende langere tijd, bijvoorbeeld meer dan tien dagen op zijn plaats te houden.
20 Gebruikelijke filtercatheters van de onderhavi ge soort omvatten een aan een distaai einde van een slangvormig basislichaam aangebracht filterelement dat gevormd wordt door een aantal, verdeeld over de omtrek aangebrachte stroken die met het basislichaam zijn ver-25 bonden en welke stroken door het naar elkaar toe bewegen van de tegenoverliggende einden uitgebogen kunnen worden teneinde aldus in de werkzame stand te worden gebracht.
De maximale verblijfsduur van dergelijke filtercatheters wordt beperkt, doordat het weefsel van de wand van het 30 betreffende bloedvat, waartegen de strookvormige elementen aanliggen, deze stroken inkapselen, zodat het filterelement ingegroeid raakt. Hierdoor kan het filter slechts nog worden verwijderd, wanneer het weefsel wordt beschadigd, hetgeen uiteraard ongewenst is.
35 De uitvinding beoogt nu een filtercatheter van de genoemde soort te verschaffen die gedurende lange tijd in het lichaam van de patiënt kan verblijven.

Bij de filtercatheter volgens de uitvinding wordt dit bereikt doordat de relatief distale einden van de stroken die het filterelement bepalen, door koppelmid-delen losneembaar met het basislichaam zijn verbonden.
5 Bij het verwijderen van de catheter worden de distale einden van de stroken ontkoppeld, waarna het filterelement zonder beschadiging van het weefsel teruggetrokken kan worden. De stroken glijden bij het terugtrekken door de "kanalen" die in het de stroken omgevende weefsel zijn 10 gevormd.
Een geschikte uitvoeringsvorm van de filtercatheter volgens de uitvinding is gekenmerkt in conclusie 2.
Het ontkoppelen van de distale einden van de 15 stroken kan hierbij geschieden door het binnenste slangvormige lichaam in distale richting te verschuiven ten opzichte van het buitenste slangvormige lichaam. Hierbij wordt een trekkracht uitgeoefend op de stroken, die daardoor loskomen van de koppelmiddelen.
20 Een geschikte uitvoerings is daarbij gekenmerkt in conclusie 3. De huls houdt de distale strookeinden vast zolang niet een bepaalde minimale trekkracht op de stroken wordt uitgeoefend. Zodra deze minimale trekkracht wordt overschreden schuiven de strookeinden onder de huls 25 uit zodat deze van de koppelmiddelen loskomen.
Bij voorkeur wordt daarbij de maatregel van conclusie 4 toegepast. De strookeinden worden daarbij elastisch vastgeklemd, zodat een betrouwbare verbinding in stand blijft, tot aan het moment dat de verbinding 30 verbroken moet worden. Door de elasticiteit van de huls is de losneemkracht goed doseerbaar.
Teneinde een positieve opsluiting van de distale strookeinden te bereiken wordt bij voorkeur de maatregel van conclusie 5 toegepast. Een geschikte uitvoerings 35 is daarbij gekenmerkt in conclusie 6.
De uitvinding zal verder worden toegelicht in de volgende beschrijving aan de hand van de bijgevoegde figuren.

Figuur 3 toont het in figuur 1 weergegeven cathetereinde met ontplooid filterelement.
Figuur 4 toont een met figuur 3 overeenkomend aanzicht 10 na relatief langduring verblijf in het lichaam van de patiënt en waarbij de stroken in het weefsel zijn gegroeid.
Figuur 5 toont het verwijderen van het filter na het losnemen van de strookeinden.

Figuur 7 toont een alternatieve uitvoeringsvorm van kop-pelmiddelen voor een catheter volgens de uit-20 vinding.
In fig. 1 is het distale einddeel van een catheter 1 volgens een voorkeursuitvoeringsvorm van de uitvinding weergegeven. De catheter 1 is ingebracht in 25 een bloedvat 2 van een patiënt en het filterelement 6 ervan moet nog worden ontplooid teneinde de filterfunctie te kunnen uitvoeren.
De catheter 1 omvat een buitenste slangvormig lichaam 3 waarin een binnenste slangvormig lichaam 4 30 verschuifbaar is opgenomen. Het binnenste slangvormige lichaam heeft een lumen waardoorheen zich een leiddraad 5 uitstrekt, bij het op zijn plaats brengen van de catheter en in het bijzonder het filterelement 6 daarvan.
Het filterelement 6 wordt bij dit voorkeursuit-35 voeringsvoorbeeld gevormd door een aantal stroken, die bestaan uit delen van de wand van het buitenste slangvormige lichaam 3 welke van elkaar gescheiden zijn door langssneden.

De relatief proximale einden van de stroken 7, dat zijn in fig. 1 de rechter einden van deze stroken 7, vormen één geheel met het buitenste slangvormige lichaam 3.
5 De relatief distale einden 10 van de stroken 7 zijn met het binnenste slangvormige element losneembaar verbonden door koppelmiddelen 8. De koppelmiddelen 8 houden de distale einden 10 van de stroken 7 vast door een klemverbinding. Deze klemverbinding wordt verkregen 10 doordat de koppelmiddelen 8 een huls 11 omvatten van elastisch materiaal, bijvoorbeeld siliconenrubber.
De huls 11 is op de catheter bevestigd met behulp van een laag uitgeharde kunststof 13, bijvoorbeeld een uitgeharde epoxy. Het einddeel 14 van de catheter 1 15 is bij voorkeur van zacht materiaal, om een maximale atraumatische werking te bereiken bij het inbrengen van de catheter.
Zoals fig. 2 laat zien is om het binnenste slangvormige lichaam 4 nog een markeerring 12 aangebracht 20 en is een tweede markeerring om het buitenste slangvormige lichaam 3 aangebracht. Beide ringen zijn bijvoorbeeld van goud gemaakt om de positie van het filterelement goed zichtbaar te maken op een röntgenscherm in een cathe-terlaboratorium.
25 Fig. 3 toont de ontplooide, werkzame toestand van het filterelement 6. Deze wordt tot stand gebracht wanneer de cahteter 1 en in het bijzonder het filterelement 6 in de beoogde positie in het bloedvat 2 is gemoeu-vreerd. De leiddraad 5 kan dan worden verwijderd.
30 Voor het ontplooien van het filterelement 6 wordt het binnenste slangvormige lichaam 4 ten opzichte van het buitenste slangvormige lichaam 3 in proximale richting verschoven. Hierdoor worden de einden van de stroken 7 naar elkaar toe bewogen, zodat deze stroken 35 uitbuigen tot in contact met de wand van het bloedvat 2. De koppelmiddelen 8 houden de einden van de stroken 7 op betrouwbare wijze vast.

Wanneer de catheter met aldus ontplooid filter-element 6 langere tijd in het lichaam wordt gelaten zal weefsel 20 om de tegen de wand van het bloedvat 2 aanliggende stroken 7 gaan groeien. De stroken 7 vormen in dit 5 weefsel 20 als het ware "doorgangen". De toestand waarbij de stroken 7 zijn vastgegroeid, is in fig 4 weergegeven. Wanneer in deze situatie zondermeer de catheter zou worden verwijderd, zou het om de stroken 7 gegroeide weefsel kapot getrokken moeten worden, met alle risico's 10 van dien.
Bij de hier beschreven catheter worden echter de distale einden 10 van de stroken 7 uit de koppelmidde-len 8 losgenomen voordat de catheter wordt verwijderd.
Dit losnemen geschiedt eenvoudig door het 15 binnenste slangvormige element 4 ten opzichte van het buitenste slangvormige lichaam 3 in distale richting te verschuiven. De elastische huls 11 wordt dan van de distale einden 10 van de stroken 7 afgeschoven, waardoor deze einden 10 vrij komen zoals weergegeven is in fig. 5 20 en 6.
Wanneer nu de catheter wordt verwijderd, glijden de stroken 7 eenvoudig door de in het weefsel 20 gevormde doorgangen 21, zonder dat enige beschadiging van dit weefsel 20 optreedt. De catheter kan dus zonder 25 bezwaar en zonder beschadiging worden verwijderd.
Zoals fig. 2, 5 en 6 laten zien zijn de einden 10 van de stroken 7 voorzien van een nok die een goed houvast van de huls op de einden 10 verzekerd.
Fig. 7 laat een enigszins andere uitvoerings-30 vorm van de stroken 7 zien waarbij de hier met verwij-zingscijfer 25 aangegeven nokken gevormd zijn door een uitsparing 24 in het buitenoppervlak van elke strook 7 op een kleine afstand van het distale einde daarvan. De uitsparing 24 heeft een diepte die gelijk is aan de dikte 35 van de huls 11, zodat het buitenoppervlak van de catheter met de stroken 7 in de gekoppelde toestand, gelijkmatig en glad verloopt.

De uitvinding is niet beperkt tot de getoonde en beschreven uitvoeringsvorm. In het bijzonder kunnen de koppelmiddelen waarmee de relatieve distale einden van de stroken zijn verbonden met het basislichaam anders dan 5 getoond worden uitgevoerd. De distale einden van de stroken kunnen zijn verbonden door middel van kleefstof, voorgesneden gebieden, haken enzovoort. Tevens kan het einddeel 14 zodanig zijn uitgevoerd, dat het direct aan de elastische huls is bevestigd, zodat eind-sectie 14, 10 ring 4 en element 13 één geheel vormen.

Claims (6)
1. Filtercatheter omvattende een slangvormig basislichaam met een proximaal en een distaai einde, een aan het distale einde aangebracht filterelement gevormd door een aantal, verdeeld over de omtrek aangebrachte 10 stroken die met weerseinden met het basislichaam zijn verbonden, waarbij de relatief distale einden van de stroken door koppelmiddelen losneembaar met het basislichaam zijn verbonden.
2. Filtercatheter volgens conclusie 1, waarbij 15 het basislichaam een buitenste slangvormig lichaam en een verschuifbaar daarin opgenomen binnenste slangvormig lichaam omvat, de stroken met hun relatief proximale einden met het buitenste slangvormige lichaam en met hun relatief distale einden met het binnenste slangvormige 20 lichaam zijn verbonden en waarbij de koppelmiddelen de distale einden van de stroken door een klemverbinding vasthouden.
3. Filthercatheter volgens conclusie 2, waarbij de koppelmiddelen een met het binnenste slangvormige 25 lichaam verbonden, om de distale strookeinden klemmende huls omvatten.
4. Filtercatheter volgens conclusie 3, waarbij de huls van elastisch materiaal, zoals siliconenrubber is.
5. Filtercatheter volgens één van de voorgaande conclusies, waarbij de distale strookeinden een nok omvatten.
6. Filtercatheter volgens conclusie 5, waarbij de nok gevormd wordt door een aan een uitsparing in een 35 buitenoppervlak van elke strook, op een kleine afstand van het distale einde daarvan, grenzend resterend wand-deel. 1002423
