Definitions
- the invention relates to a method for producing a metal cup which is implantable directly into the pelvic bone of a patient and is part of a hip joint prosthesis, with a permitted cup deformation by the elasticity of the pelvic bone in the loading state and the metal cups produced by this method.
- the market has a variety of prosthetic systems for the replacement of the natural hip joint. These usually consist of a shaft 1 coupled to a ball head 2 and a metal pan 4 coupled with a pan insert 3. Shank 1 and metal pan 4 are connected to the body by ingrowth in the Oberschenkel construction.

- Metal pans are coupled to the pelvic bone by various techniques during the primary fixation operation.
- a common type of fixation is the clamping of the metal cup by means of pressfit. This represents a form of coupling which in the technical sense is attributable to the oversize connections, i. the external geometry of the metal cup is greater than the cup holder created by the doctor by milling out the acetabulum. The thus acting after the introduction of the pan from the pelvic bone on the pan normal forces and consequently frictional forces ensure the primary anchoring of the metal pan.
- the deformation of the pan can lead to an increased risk of the formation of stripe-wear, which is a possible cause of the occurrence of the squeaking phenomenon.
- the invention has for its object to improve a method according to the preamble of claim 1 so that asymmetric deformations of the metal pan are compensated by the elasticity of the pelvic bone in the load state. Furthermore, a metal pan is to be specified, which is produced by this method.
- the metal pan is provided with a thick-walled, porous outer coating whose stiffness is adjusted so that the layer collapses in the load state when a voltage corresponding to the permitted pan deformation is exceeded.
- asymmetrical deformations of the metal cup are compensated by the elasticity of the pelvic bone in the load state.
- Collapse is understood to mean the collapse or breakage of the porous outer coating. This collapse takes place at the points of the porous outer coating, at which the permitted corresponding voltage is exceeded.
- a pan deformation therefore means the deformation of the metal pan.
- the porous outer coating is realized with a closed-pore structure, which collapses when the metal pan is inserted into the pelvic bone when the corresponding tension is exceeded. However, the fragments remain within the structure so that there is no inflammatory risk.
- the porous outer coating is realized with an open-pore structure, which collapses during the introduction of the metal pan in the pelvic bone when the corresponding voltage is exceeded. This collapse is to be understood as a plastic deformation. There are no free fragments. The pelvic bone can grow into the remaining pores of the biocompatible material.
- the porous outer coating can be realized with areas with a closed-pore structure and with areas with an open-pore structure.
- the outer coating is produced in an inventive embodiment with a thickness of at least 0.1 mm, preferably with a thickness of at least 0.5 mm.
- a metal cup according to the invention for direct implantation into the pelvic bone of a patient, as part of a hip joint prosthesis, produced in particular by the method described, is characterized in that the metal cup has a thick-walled, porous outer coating which has a closed-pore and / or open-pore structure.
- the metal pan can compensate for asymmetrical deformations due to the elasticity of the pelvic bone in the loaded state.
- the thickness of the porous outer coating is at least 0.1 mm, preferably at least 0.5 mm. - A -

Abstract
Description
The invention relates to a method for producing a metal cup which is implantable directly into the pelvic bone of a patient and is part of a hip joint prosthesis, with a permitted cup deformation by the elasticity of the pelvic bone in the loading state and the metal cups produced by this method.
Pfanneneinsatz 3. Der Kugelkopf 2 ist in der Kalotte des Pfanneneinsatzes 3 drehbar gelagert - Freiheitsgrad: 1 (siehe Figur 1).The market has a variety of prosthetic systems for the replacement of the natural hip joint. These usually consist of a shaft 1 coupled to a ball head 2 and a metal pan 4 coupled with a pan insert 3. Shank 1 and metal pan 4 are connected to the body by ingrowth in the Oberschenkelbzw. The ball head 2 is rotatably mounted in the cup of the cup insert 3 - degree of freedom: 1 (see Figure 1).
Metal pans are coupled to the pelvic bone by various techniques during the primary fixation operation. A common type of fixation is the clamping of the metal cup by means of pressfit.
Due to the inhomogeneous properties of the pelvic bone, asymmetric loading of the metal cup in the press fit situation usually occurs, which can result in asymmetric deformation of the metal cup. This is in principle undesirable because the asymmetric deformation of the metal pan complicates the introduction of the cup inserts or can lead to asymmetric loading and deformation of the cup inserts in the further course. Possible further effects are then damage to the socket insert and / or limitation of the function of the sliding mating by local reduction of the calotte diameter of the socket insert.
Furthermore, the deformation of the pan can lead to an increased risk of the formation of stripe-wear, which is a possible cause of the occurrence of the squeaking phenomenon.
The invention has for its object to improve a method according to the preamble of claim 1 so that asymmetric deformations of the metal pan are compensated by the elasticity of the pelvic bone in the load state. Furthermore, a metal pan is to be specified, which is produced by this method.
According to the invention this object is achieved in that the metal pan is provided with a thick-walled, porous outer coating whose stiffness is adjusted so that the layer collapses in the load state when a voltage corresponding to the permitted pan deformation is exceeded. As a result, asymmetrical deformations of the metal cup are compensated by the elasticity of the pelvic bone in the load state. Collapse is understood to mean the collapse or breakage of the porous outer coating. This collapse takes place at the points of the porous outer coating, at which the permitted corresponding voltage is exceeded. Under pan is always understood in this description, the metal pan. A pan deformation therefore means the deformation of the metal pan.
The collapse can be achieved by two mechanisms, which are presented below. According to a first embodiment of the invention, the porous outer coating is realized with a closed-pore structure, which collapses when the metal pan is inserted into the pelvic bone when the corresponding tension is exceeded. However, the fragments remain within the structure so that there is no inflammatory risk.
According to a second embodiment of the invention, the porous outer coating is realized with an open-pore structure, which collapses during the introduction of the metal pan in the pelvic bone when the corresponding voltage is exceeded. This collapse is to be understood as a plastic deformation. There are no free fragments. The pelvic bone can grow into the remaining pores of the biocompatible material.
The porous outer coating can be realized with areas with a closed-pore structure and with areas with an open-pore structure.
The outer coating is produced in an inventive embodiment with a thickness of at least 0.1 mm, preferably with a thickness of at least 0.5 mm.
A metal cup according to the invention for direct implantation into the pelvic bone of a patient, as part of a hip joint prosthesis, produced in particular by the method described, is characterized in that the metal cup has a thick-walled, porous outer coating which has a closed-pore and / or open-pore structure. As a result, the metal pan can compensate for asymmetrical deformations due to the elasticity of the pelvic bone in the loaded state.
- A -In an inventive embodiment, the thickness of the porous outer coating is at least 0.1 mm, preferably at least 0.5 mm. - A -
