  • the present invention relates to an intramedullary (endomedullary) nail for the promotion of osteosynthesis of bone fractures, and in particular for the surgical treatment of femur's trochanteric fractures.
  • Femur's trochanteric fractures are usually treated with intramedullary nails provided with a tubular element that is longitudinally inserted in the bone marrow, a cephalic screw which is introduced into a corresponding seat obtained in the greater trochanter, in order to guide the osteosynthesis in correspondence of the fracture, and stabilizing screws positioned in the distal portion of the nail, i.e., that nail portion which, upon application of the device, is near to the knee, to allow the blocking of the nail on the femur bone.
  • An intramedullary nail of this type for the treatment of trochanter fractures has some drawbacks. It has been, in fact, found that with this type of device, the infection risks increase because of the additional incisions necessary for the positioning of the stabilizing screws in the nail distal portion.
  • these devices require a rather long recovery time for the patient.
  • the stabilizing screws are coupled in a stable manner, without any clearance, in special seats obtained on the nail, which is why during this first rehabilitation stage, it is not possible to load the leg and to stimulate the fracture rima.
  • the type of coupling of the stabilizing screws with the intramedullary nail change, passing from a stable distal blocking to a dynamic distal blocking, following a further operation, and the stimulation of the fracture rima becomes therefore possible.
  • Object of the present invention is to provide an intramedullary nail for the treatment of femur's trochanteric fractures, such as to allow to solve all the drawbacks of the known art.
  • an object of the present invention is to provide an intramedullary nail for femur's trochanteric fractures, such as to reduce the risk of infections.
  • Another object of the present invention is to provide an intramedullary nail such as to reduce the operation and biological recovery times.
  • Another object of the present invention is to provide an intramedullary nail whose application is easy and allows to reduce the exposure of patient and surgeon to x rays.
  • an intramedullary nail for the surgical treatment of femur's trochanteric fractures comprising a tubular element provided with a cephalic screw and at least a stabilizing element, characterized in that said stabilizing element comprises a couple of wire rods which are angled at one end and arranged longitudinally and internally relatively to the tubular element, to come out from two openings located in the distal portion of said tubular element.
  • An intr ⁇ medull ⁇ ry nail of this type needs no additional incisions to. block the stabilizing element to the femur, and reduces, therefore, infection risks.
  • the screw and/or the tubular element have grooves for housing wire rods.
  • the grooves extend longitudinally on at least a portion of the internal surface of the tubular element. According to another advantageous aspect of the present invention, the grooves extend on the external surface of the cephalic screw in parallel with its inclination axis.
  • the wire rods comprise a pointed and angled end, to block the nail to the femur bone, a central flat portion and at least a portion having a circular section.
  • a distal blocking of this type is stable and at the same time dynamically controlled, and the micro-movements realized by the wire rods allow an early load on the femur that promotes a rapid biological recovery.
  • the intramedullary nail comprises also a device for blocking in their position both the wire rods and the cephalic screw.

  • tubular element 2 for the surgical treatment of femur's trochanteric fractures, comprising a tubular element 2 provided with a cephalic screw inclined with respect to the axis of the tubular element 2, and at least a stabilizing element composed of at least a couple of wire rods 4, 5 that are angled at an end and positioned longitudinally and internally relatively to the tubular element 2, to come out from two openings 9, 10 obtained in the distal portion of said tubular element.

  • distal portion of nail 1 or tubular element 2 there is intended to indicate that portion of nail 1 or tubular element 2 that, upon the application of device 1 on the femur lies near the knee. Consequently, in the present description by “proximal portion of nail 1 or tubular element 2” there is intended to indicate that portion of nail 1 or tubular element 2 that, upon the application of device 1 on the femur, lies near the acetabulum of the iliac bone.
  • the tubular element 2 is provided with special grooves.
  • the intramedullary nail is provided wit two grooves 6, 7 for housing the wire rods that extend longitudinally for a portion of the tubular element 2 and that are located in a diametrically opposite manner on the internal surface of such element.
  • cephalic screw 3 is provided with grooves 8 (Fig. 3), suitable to prevent the interference with wire rods 4,5, in correspondence of their intersection with the cephalic screw 3 and also to provide an engaging seat for the blocking device of said screw.
  • cephalic screw 3 is provided, as shown in Figures 2 and 3, with four screws 8 located in a diametrically opposite manner on the external surface thereof, which extend in parallel with its inclination axis (indicated by a broken line in Figures 1 and 2).
  • Wire rods 4,5 have a first end portion having a circular section that continues, going down towards the distal portion of nail 1 , and a second flat portion in correspondence of the cephalic screw 3, and a third portion, having also a circular section, pointed and angled, that allows an easy coming out from openings 9, 10, obtained in the distal portion of the nail.
  • openings 9, 10, obtained in the distal portion of nail 1 are inclined and show the same inclination of the pointed end of wire rods 4, 5.
  • wire rods 4,5 are blocked in their position by a blocking device located at the end of the proximal portion of the tubular element 2.
  • the blocking device has a threaded plug element 1 1 (Fig. 3) that blocks wire rods 4,5 against the internal surface of the tubular element 2.
  • the plug element 1 1 has on its internal surface, in correspondence of its proximal portion, a thread 12 complementary to the one realized on the plug element 1 1.
  • the plug element 1 1 has also, arranged along its symmetry axis, a threaded coupling seat 17 for a nut 13 and a counter-nut 14, suitable to block in its position, with respect to the rotation, the cephalic screw 3.
  • the cephalic screw 3 has a flaring head 15 or a step-like head, that prevents any translations of the cephalic screw 3 along its inclination axis, and in particular prevents the cephalic screw from further penetrating into the greater trochanter.
  • a thread 16 On the opposite end of the cephalic screw there is positioned in a known manner a thread 16 that realizes the engagement of the cephalic screw 3 and therefore of the whole nail 1 with the femur bone, and in particular, with the greater trochanter.
  • an inclined through-hole 20 is positioned that allows, during the application of the intramedullary nail 1 , thanks to a guide screw, not shown and removed afterwards, to fix nail 1 in its position before the insertion of the cephalic screw 3 into the greater trochanter.

Abstract

Clou intramédullaire (1) conçu pour le traitement chirurgical de fractures trochanteriennes du fémur et consistant en un élément tubulaire (2) pourvu d'une vis céphalique (3) et en au moins un élément stabilisateur comportant au moins une paire de fils (4, 5) orientés angulairement à une extrémité et placés dans la longueur et à l'intérieur de l'élément tubulaire afin de sortir par des ouvertures (9, 10) pratiquées dans la partie distale dudit élément tubulaire.
