SUBSTANCE: invention relates to field of medicine, namely to psychiatry and deals with correction of psychic condition of patients and antioxidant system in case of organic personality disorder.

EFFECT: claimed is method of correcting psychic condition of patients in case of organic personality disorder, which includes traditional therapy with nootropics and vitamins, which is characterised by the fact, that 400 ml per day of reamberine is additionally introduced by intravenous drop infusion with speed of introduction 90 drops per minute, for 10 days.

Description

The invention relates to medicine, and specifically to psychiatry, and for the correction of the mental state of patients and the antioxidant system in organic personality disorder.

Previously, drugs, specifically for correcting the mental state of patients and the antioxidant system in organic personality disorder, were not used.

The well-known drug Reamberin - created on the basis of succinic acid, is a balanced isotonic infusion solution that contains sodium succinate (dissociated salt of succinic acid), magnesium chloride, potassium chloride, sodium chloride. The drug has antihypoxic, antioxidant, detoxification effects (1). This drug has not been used in the treatment of organic personality disorder.

EFFECT: development of a method for correcting the mental state of patients with organic personality disorder.

A method for correcting the mental state of patients with organic personality disorder is proposed, characterized by the fact that patients are injected intravenously with 1.5% 400 ml reamberin solution. per day, the rate of administration of 90 drops per minute for ten days.

The proposed method for the correction of the mental state of patients with organic personality disorder allows patients with organic personality disorder against the background of traditional therapy to most effectively reduce the severity of mental disorder of patients in comparison with traditional therapy. Because Reamberin is an indirect antioxidant, it activates enzymes of the antioxidant system, which in turn inhibit lipid peroxidation in ischemic organs, reducing the production of free radicals and restoring the energy potential of cells, and has a membrane-stabilizing effect on brain cells.

We studied patients with organic personality disorder, in particular two groups from this section:

F 07.98 Organic personality disorder and behavior in connection with a mixed disease (traumatic, toxicometabolic encephalopathy).

F02.8 Dementia due to mixed diseases (traumatic, toxicometabolic encephalopathy).

Reamberin was administered intravenously at 400 ml. per day, the rate of administration of 90 cal. per minute for 10 days.

60 patients were examined.

1. "Control group 1". 15 people - patients with F07.98 receiving standard treatment (nootropics, vitamin therapy).

2. "Control group 2". 15 people - patients with F02.8 receiving standard treatment.

3. The group - "Reamberin 1". 15 people - patients with F07.98 receiving standard treatment + reamberin.

4. The group - "Reamberin 2". 15 people - patients with F02.8 receiving standard treatment + reamberin.

Solving the tasks, we determined the mental status of patients and the level of indicators of free radical lipid peroxidation (LPO) and antioxidant system (AOS) in the blood serum of patients before and after treatment, such as ceruloplasmin, diene conjugates, lipid hydroperoxides, malondyldehyde, vitamin E.

Blood sampling was carried out on the 2-3rd day of admission to the hospital, in the morning on an empty stomach. To assess the dynamics of the studied parameters, repeated blood sampling was carried out on the 10th day after the first analysis in the control group of patients and on the 10th day of treatment in patients in whose treatment reamberin was used.

The results of the study:

Statistical processing of the results was carried out according to generally accepted formulas. In connection with the parametric distribution of the studied trait, the comparison of the results was carried out using Student's criterion.

We analyzed the initial level of lipid peroxidation and AOS in patients with organic personality disorder and behavior of mixed origin and in patients with dementia (in comparison with healthy people). Changes were determined in terms of the concentration of all studied lipid peroxidation products in comparison with healthy people. In the statistical processing of the obtained results, we did not reveal significant differences between the indices at F07.9 and at F 02.8., But a significant difference was found in LPO products in these disorders compared with healthy people.

In addition to biochemical parameters, the clinical effect of the treatment was also evaluated. In particular, a pathopsychological examination of the studied patients was carried out before treatment and 20 days after the completion of the course of basic therapy + reamberin. The main areas to which attention was paid are memory and attention. Attention was studied by the method of finding numbers (Schult's table), memory by the method of memorizing ten words.

Patients with a diagnosis of F07.9, when using Reamberin before treatment, spent on the Schult table on average 1, 5 minutes, 20 days after the course of treatment, the result changed on average up to 1 minute.

In patients with a diagnosis of F 02.8, when using reamberin before treatment, one table took an average of 2.5 minutes, 20 days after the course of treatment, if it has changed, it is not significant, i.e. the rate improved by an average of 10 seconds.

When assessing memory using the 10-word memorization method, patients with F07.9 before treatment reproduced, on average, 6 words out of 10 proposed to them, 20 days after the end of the course of treatment and reamberin 8 words out of 10. Patients with F02.8 showed the following results - 4 words out of 10 before treatment and 20 days after the course of treatment, the result remained practically unchanged, in some cases it improved to 5 words out of 10.

Clinical example:

A patient born in 1957, a resident of Blagoveshchensk. Admission to the hospital is repeated, associated with complaints of memory impairment (memorization is difficult), fatigue, weakness, emotional instability.

From the anamnesis it is known: Grew up and developed according to age parameters. He studied secondary, graduated from 9 classes and technical school. He served in the army. He worked as a driver, the last 10 years as a handyman. Married, has an adult son. Of the illnesses noted childhood infections, suffered appendectomy. It is registered in the Regional Psychoneurological Dispensary with a diagnosis of Alcoholism 2 degrees in the form of pseudo-drunken drinking. Alcohol abuse for about 10 years. He was treated in a hospital 3 times, the last hospitalization two years before the present admission, in connection with similar complaints. Mental illness in relatives is not noted.

Over the past month, the mental state changed, became emotionally unstable, weak, noticed a sharp upset in the process of memorization, difficulty in focusing, there was some unsteadiness of the gait. According to the patient, over the past three weeks he did not drink alcohol.

Mental status: Consciousness is clear. Contact is available. In place, time and self oriented correctly. Perceptual disorders are not noted. Memory reduced for all kinds of events. Consistent thinking is somewhat slow. Speech is monotonous. In an interview with the doctor is not active, but answers all questions correctly, describes his condition. The general background of mood is reduced, emotionally labile. The criticism is formal.

Somatic condition: medium height, proper physique, moderate nutrition. The skin is clean. In the lungs without pathology. Heart sounds are muffled. Pulse 70 beats. per minute. HELL 130 and 80 mm Hg The gastrointestinal tract without a pronounced pathology.

In the department examined:

Conclusion of a neurologist - encephalopathy of complex genesis (toxic vascular)

Optometrist - retinal angiopathy.

EEG - moderate diffuse changes in the bioelectric activity of the brain.

REG - arterial hypotonic type; REG 1 type.

Clinical blood test without pronounced violations.

Blood chemistry -

Psychologist - the patient spent 1.5 minutes on the Schultz table, from the proposed 10 words he remembered 5. there was an instability of mental processes, a weakening of memorization.

Nootropics, sedatives, vitamin therapy + reamberin were used in the treatment.

During treatment, the patient's condition improved. He practically doesn’t notice any weaknesses, his working capacity has improved, he is concentrating more easily, the memorization process has improved, he is emotionally even, his mood corresponds to the events that are taking place.

The psychological tests were repeated: the patient spent 1.1 minutes on the Schultz table, from the proposed 10 words he memorized 8.

In addition, in order to assess the antioxidant activity of reamberin, lipid peroxidation indices were determined before and after the course of treatment.
