Abstract
1. Способ хирургического лечения макулярных разрывов, включающий выполнение витрэктомии через трехпортовые микродоступы с иссечением задней гиалоидной мембраны, окрашивание внутренней пограничной мембраны (ВПМ) в макулярной области путем подачи эндовитреального красителя через ирригационную канюлю, удаление излишек красителя после экспозиции, пилинг ВПМ путем макулорексиса и окончательную тампонаду сетчатки газовоздушной смесью, отличающийся тем, что окрашивание ВПМ выполняют вокруг макулярного разрыва, не давая красителю попасть за края разрыва, для этого, во-первых, используют порты, оснащенные клапанной системой, во-вторых, во время окрашивания полностью прекращают подачу солевого раствора в витреальную полость, а сам краситель наносят дискретно и под минимальным давлением, последовательно продвигаясь дистальным концом канюли по окружности, диаметр которой, соответствует диаметру будущего макулорексиса, а после прокрашивания мембраны излишки красителя удаляют сначала вблизи разрыва путем пассивной аспирации, затем, включив подачу солевого раствора в витреальную полость под низким давлением, удаляют оставшийся краситель.2. Способ по п.1, отличающийся тем, что в качестве красителя используют краситель марки «Brilliant Blue G».3. Способ по п.1, отличающийся тем, что экспозиция прокрашивания мембраны составляет 30-90 секунд.4. Способ по п.1, отличающийся тем, что при макулярных разрывах 2, 3 и 4 стадии после макулорексиса выполняют сведение краев разрыва.5. Способ по п.1, отличающийся тем, что низкое давление ирригационной системы во время удаления излишек красителя составляет 12-15 мм рт.ст.1. The method according to claim 1, characterized in that as a dye using dye brand "Brilliant Blue G". The method according to claim 1, characterized in that the exposure of the membrane staining is 30-90 seconds. The method according to claim 1, characterized in that for macular ruptures of stages 2, 3 and 4 after maculorexis, the edges of the rupture are reduced.
