Claims (34)
1. Внутрипросветный протез, содержащий: трубчатый проволочный каркас, имеющий проксимальный конец, дистальный конец и проходящий через протез центральный просвет; причем проволочный каркас содержит по меньшей мере первый и второй смежные в осевом направлении трубчатые сегменты, соединенными проходящим между ними соединителем; при этом первый и второй сегменты и соединитель выполнены из одного куска проволоки.
2. Внутрипросветный протез по п. 1, содержащий по меньшей мере три сегмента и два соединителя.
3. Внутрипросветный протез по п. 1, содержащий по меньшей мере пять сегментов и четыре соединителя.
4. Внутрипросветный протез по п. 1, в котором проволока в каждом сегменте содержит последовательность проксимальных изгибов, последовательность дистальных изгибов, создающие последовательность стоечных сегментов, соединяющих проксимальные изгибы и дистальные изгибы, формируя стенку трубчатого сегмента.
5. Внутрипросветный протез по п. 4, в котором по меньшей мере некоторые из стоечных сегментов являются практически линейными.
6. Внутрипросветный протез по п. 4, содержащий далее петлю по меньшей мере на некоторых изгибах.
7. Внутрипросветный протез по п. 6, в котором одна или более петель на дистальном конце первого трубчатого сегмента соединены с одной или более соответствующих петель на проксимальном конце второго трубчатого сегмента.
8. Внутрипросветный протез по п. 7, в котором соответствующие петли соединяются швом или кольцом.
9. Внутрипросветный протез по п. 4, в котором каждый сегмент содержит от приблизительно 4 проксимальных изгибов до приблизительно 12 проксимальных изгибов.
10. Внутрипросветный протез по п. 1, имеющий по меньшей мере проксимальный сегмент, промежуточный сегмент и дистальный сегмент, причем протез выполнен с возможностью расширяться от уменьшенного поперечного сечения к увеличенному поперечному сечению.
11. Внутрипросветный протез по п. 10, в котором по меньшей мере часть проксимального сегмента и дистальный сегмент шире в поперечном сечении, чем центральный сегмент, при нахождении протеза в состоянии расширенного поперечного сечения.
12. Внутрипросветный протез по п. 1, содержащий далее полимерный слой на проволочном каркасе.
13. Внутрипросветный протез по п. 12, в котором слой содержит трубчатый рукав из политетрафторэтилена, окружающий по меньшей части центральную часть протеза.
14. Способ изготовления внутрипросветного протеза, содержащий следующие шаги: обеспечение куска проволоки, формирование проволоки в виде двух или более зигзагообразных секций, причем каждая зигзагообразная секция отделяется поперечной связью; скручивание сформированной проволоки вокруг оси для получения последовательности трубчатых элементов, расположенных вдоль оси так, что каждый трубчатый элемент соединен связью со смежным трубчатым элементом.
15. Способ по п. 14, содержащий далее шаг размещения трубчатого полимерного рукава концентрически на по меньшей мере одном из трубчатых элементов.
16. Способ по п. 15, в котором шаг размещения включает в себя размещение трубчатого полимерного рукава концентрически на внешней поверхности трубчатого элемента.
17. Способ по п. 16, в котором трубчатый полимерный рукав содержит политетрафторэтилен.
18. Многозонный внутрипросветный протез, содержащий трубчатый проволочный каркас, имеющий проксимальный конец, дистальный конец и проходящий через протез центральный просвет; причем проволочный каркас содержит по меньшей мере первый и второй смежные трубчатые сегменты, соединенные проходящим между ними соединителем; при этом первый трубчатый сегмент имеет радиальную прочность, отличную от второго трубчатого сегмента.
19. Внутрипросветный протез по п. 18, содержащий далее третий трубчатый сегмент, при этом по меньшей мере один из трубчатых сегментов имеет радиальную прочность, отличную от двух других сегментов.
20. Внутрипросветный протез по п. 19, в котором проксимальный конец протеза саморасширяется до большего диаметра, чем центральный участок протеза.
21. Внутрипросветный протез, содержащий удлиненную гибкую проволоку, сформированную в виде множества смежных в осевом направлении трубчатых сегментов, разнесенных вдоль оси, причем каждый трубчатый сегмент содержит зигзагообразную секцию проволоки, имеющую множество проксимальных изгибов и дистальных изгибов, проволока продолжается между каждым смежным трубчатым сегментом, при этом протез выполнен с возможностью радиально сжиматься в первую конфигурацию с уменьшенным поперечным сечением для имплантации в просвет тела и саморасширяться до второй конфигурации с увеличенным поперечным сечением в месте назначения в просвете тела.
22. Внутрипросветный протез по п. 21, содержащий по меньшей мере три сегмента, сформированные из упомянутой проволоки.
23. Внутрипросветный протез по п. 22, содержащий далее внешний трубчатый рукав, окружающий по меньшей мере часть протеза.
24. Внутрипросветный протез по п. 23, в котором рукав содержит далее по меньшей мере один проходящий через него боковой перфузионный порт.
25. Внутрипросветный протез по п. 22, в котором протез имеет проксимальный конец и дистальный конец, и по меньшей мере один из проксимального конца и дистального конца выполнен с возможностью расширяться до большего диаметра, чем центральная часть протеза, при неограниченном расширении.
26. Внутрипросветный протез по п. 21, в котором по меньшей мере один дистальный изгиб на первом сегменте соединен с по меньшей мере одним проксимальным изгибом на смежном сегменте.
27. Внутрипросветный протез по п. 26, в котором соединение содержит шарнирное соединение.
28. Внутрипросветный протез по п. 27, в котором соединение содержит металлическую связку.
29. Внутрипросветный протез по п. 27, в котором соединение содержит шов.
30. Внутрипросветный протез по п. 21, в котором протез имеет коэффициент расширения по меньшей мере приблизительно 1: 4.
31. Внутрипросветный протез по п. 30, в котором протез имеет коэффициент расширения по меньшей мере приблизительно 1: 5.
32. Внутрипросветный протез по п. 21, в котором протез имеет расширенный диаметр по меньшей мере приблизительно 20 - 30 мм при неограниченном расширении, и протез выполнен с возможностью имплантирования с помощью катетера размером не более приблизительно 16 French.
33. Протез по п. 32, в котором протез имеет расширенный диаметр по меньшей мере приблизительно 25 мм, и протез выполнен с возможностью имплантироваться на устройстве доставки, имеющем диаметр не более приблизительно 16 French.
34. Способ имплантации внутрипросветного сосудистого протеза, содержащий следующие шаги: обеспечение саморасширяющегося внутрипросветного сосудистого протеза, имеющего проксимальный конец, дистальный конец и проходящий через протез центральный просвет, причем упомянутый протез выполнен с возможностью расширяться от первого, уменьшенного диаметра до второго, увеличенного диаметра; закрепление протеза на катетере так, что когда протез находится на катетере в конфигурации с уменьшенным диаметром, диаметр катетера по длине протеза не превышает приблизительно 16 French; введение катетера в просвет тела и размещение протеза в месте назначения в просвете тела; освобождение протеза на месте назначения так, что протез расширяется от первого диаметра до второго диаметра; при этом второй диаметр составляет по меньшей мере 20 мм.
