Description
本特許出願は、双方が引用することにより本明細書の一部をなす2011年9月2日に出願された米国仮特許出願第61/530614号及び2012年6月20日に出願された米国仮特許出願第61/662070号の優先権及び全ての利益を主張するものである。

前記方法は、前記移植片を前記移植片ポケットに配置するとともに、前記移植片ポケットの所定の位置に前記移植片を固定するステップを含んでいる。
図1を参照すると、トラッキング及び制御システム100が示されている。トラッキング及び制御システム100は、器具200に取り付けられた切断アクセサリ202の遠位側端チップ204を既定の境界に対して所望の関係に保つように器具200をトラッキングするものである。(ここで、「遠位側」は、器具200を保持する医師から離れて、器具が適用される組織に向かうことを意味する。「近位側」は、医師に向かって、器具が適用される組織から離れることを意味する。)トラッキング及び制御システム100は、器具200の基準点に対して切断アクセサリチップ204の位置を制御するものである。この制御は、切断アクセサリ202が適用される手術部位における境界に切断アクセサリチップ204が突き当たり、又は、これを突破することを防いでいる。
図1を参照して、トラッキング及び制御システム100は、ナビゲーションユニット108を含んでいる。ナビゲーションユニット108は、大腿骨102及び手術器具200の位置及び方位をトラッキングするものである。ナビゲーションユニット108は、カメラ110を含んでいる。ナビゲーションコンピュータ112は、カメラ110から信号を受け取り、処理している。カメラ110は、データ接続107によりナビゲーションコンピュータ112に接続されている。データ接続107は、高速通信及びアイソクロナスリアルタイムデータ伝送に標準のシリアルバスインタフェースであるIEEE1394インタフェースであることができる。データ接続107は、個別企業特有のプロトコルを使用することもできる。
A.概観
図1を参照すると、手術器具200は、データ接続123を介して器具コントローラ120と通信するようになっている。データ接続123は、ナビゲーションコンピュータ112により生成され、器具コントローラ120に伝送される位置及び方位データに基づいて器具200を制御するのに必要な入力及び出力の経路を提供している。
図9〜図17、図24及び図41を参照すると、器具200の上部アセンブリ300の種々の図が示されている。上部アセンブリ300、より詳細には、駆動アセンブリ314は、作業部分、例えば、切断アクセサリ202を支持している。以下に更に説明するように、上部アセンブリ300及び切断アクセサリ202は、複数の自由度で手持ち部分、例えば、ハンドルアセンブリ500に対して移動するようになっている。
ここでは、図28〜図37を参照してハンドルアセンブリ500が説明されている。ハンドルアセンブリ500は、キャリア302をスライド可能に支持している。キャリア302のスライド移動は、器具200の縦軸Z(z軸とも称される)に沿う切断アクセサリ202の直線的な調整を生じさせることになる。ハンドルアセンブリ500は、ハンドル502、トリガアセンブリ506及び直線調整機構513を有している。
ここで図40〜図45を参照して説明するように、キャリア302は、多数の穴を含み、それら穴を通して配線が引き回されている。これらの配線(図示せず)は、これらの配線を介してセンサ信号が受信され、かつ、電力信号がキャリア302に取り付けられた種々の構成部品に印加される配線である。キャリアベース305は、一対の縦の貫通穴612を画定している。各貫通穴612は、フランジ303のうちの個々の1つ上及び内側に位置している。好ましくはプラスチックで形成されるガイドチューブ614が各貫通穴612の内側に位置している。1つのチューブ614の内部のルーメン615は、モータ220に延びる8本の配線の導管として機能している。第2のチューブ614を通るルーメン615は、モータ222に接続する8本の配線のルーメンとして機能している。組立の間、ガイドチューブ614は、穴612の一端に挿入されるようになっている。プラグチューブ616は、各穴612の対向端を閉鎖するものである。各ガイドチューブ614は、関連する穴612に配置された第1の端部と、キャリアベース305の近位側向きの面に当接するヘッド618を備えた第2の端部を有している。図44に示すように、各ガイドチューブ614は、キャリア穴612内に配置された端部である遠位側端部に足617があるように成形されている。チューブ614の本体と同じ弧状寸法を有する足617は、チューブ614の本体の内側面と一致する面を有している(面は特定されていない)。チューブ本体の端部から遠位側に前方に延びるにつれ、この足の面は下方に湾曲している。
図54を参照すると、シェル670は、ハンドル502の底部に取り付けられている。シェル670は、コントローラ230、232、234を収容している。シェル670は、コントローラ230、232及び234が配置される矩形のケース676を含んでいる。ケース676は、頂部が開口している。蓋674は、ケース676の開口した頂部端上に固定されている。蓋674は、締結具672でハンドル502の底部に取り付けられている。ケース676の内部には、形状が柱状の支柱675を有している。コントローラ230、232、234は、ケース676内で他のものの上に積み重ねられている。一組の支柱675は、ケース676の底から最下部のコントローラを離して保持している。第2の組の支柱は、最下部のコントローラから中間のコントローラを離して保持している。第3の組の支柱675は、中間のコントローラから最上部のコントローラを離して保持している。モータ220、222、224並びにホール効果センサ392、492、566からの配線は、コントローラ230、232、234で終端している。
図55を参照すると、必要な場合に、ブラケットアセンブリ700が、トラッキング装置114を保持するように、ハンドル502に取り付けられている。代替的な実施形態では、トラッキング装置114のLEDが器具200に組み込まれてブラケットアセンブリ700の必要性を解消している。
図58を参照すると、システムの操作の準備に向けて取られる基本的ステップが示されている(システムは、トラッキング、制御システム100及び器具200と考えられる)。第1のステップ800では、システムが電源投入される。システムを操作するソフトウェアアプリケーションは、ステップ802で開始する。ステップ804及び806では、トラッカ114、116並びにポインタ(図示せず)が初期化され、トラッカ116及び114は、目標の骨(例えば、大腿骨102)及び器具200に配置される。
ホーミングプロセスの後、器具200のコントローラ120による制御は、(1)ナビゲーションコンピュータ112からの位置及び方位データ;(2)コントローラ230、232、234からの累積カウント;及びトリガ208が作動された程度を示す3つの信号に基づいている。
1)切断アクセサリ202の上下及び左右の旋回の間、切断アクセサリ202は、ジンバル304の中心の回りで旋回するレバーとして機能する。このレバーの一端は、バーヘッド204である。このレバーの対向端は、ナット376又は476である。これは、ナット376又は476の変位が、それぞれ、切断アクセサリ202の上下又は左右の旋回を生じさせるからである。バーヘッド204をそのホーム位置からX軸又はY軸上で旋回させるように、各ナット376、476がナットのホーム位置から変位する必要がある程度の間には、概略一次の相関がある。切断アクセサリ202を軸Zに沿って変位させるには、キャリッジ302、ひいては、キャリッジナット552が前方又は後方に同距離変位される必要がある。したがって、ナット552のそのホーム位置からの変位とバーヘッド204の軸Zに沿う変位との間には、直線相関がある。(X軸又はY軸上での切断アクセサリ202の旋回の結果、バーヘッド204のZ軸上でのホーム位置からのいくらかの変位がある。この変位は、バーヘッド204を偏位位置に配置するようにバーヘッド204の個々のX軸変位、Y軸変位及びZ軸変位を決定するのに使用されるアルゴリズムにおいて考慮される。)
2)各送りネジ336、436、516の回転角度と送りネジに取り付けられたナットであって、それぞれ、ナット376、476、552の線形変位との間には、一次の相関がある。
3)各モータ220、222、224のロータの回転角度とロータにギア連結接続された送りネジであって、それぞれ、送りネジ336、436、516との間には、一次の相関がある。
4)各モータ220、222、224のロータが回転する角度と、その位置を示すそれぞれ関連するコントローラ230、232、234により維持される累積カウントとの間には、一次の相関がある。
図65を参照すると、システムの1つの可能な用途は、上記のような骨の切削に用いるものである。本質的には、除去された骨は、移植片(例えば、膝移植片)の「ネガ」のキャビティ1006を提供する。器具200は、複雑な3次元形状(すなわち、鏡面対称形状)をも切り出することができる。同様に、器具200は、突出した骨及び変形を剃り落とし/平滑化するように使用されることができる。
上記は、システムの特定の一態様に向けられている。本発明のシステムの代替的な態様が可能である。例えば、器具200は、境界の近く又は境界上にあるときに、又は、或る特定の量の偏位を超過した後で振動する機構(例えば、偏心モータ)を有することができる。これは、ユーザに、切断アクセサリの遠位側端チップが境界に接近していることの更なるフィードバックを提供するものである。切断アクセサリの作業境界への接近度の視覚的表示を提供するように、器具200、例えば、ハンドル502に光源(例えば、LED)を設けることができる。例えば、緑の信号=良好、黄色=境界上、赤=問題/停止。
上節で記載した代替的な実施形態に加え、図72〜図111は、ペンシルグリップの構成を有する以下で1200の符号が付される手術器具の別の実施形態を示している。手術器具1200は、図1に示されており、上記されたトラッキング及び制御システム100において使用可能である。上記のように、トラッキング及び制御システム100は、切断アクセサリ202のチップすなわちバーヘッド204を目標体積104に保つように目標体積104及び手術器具1200の位置及び方位をトラッキングするようになっている。手術器具1200は、上記手術器具200と同様の用途で使用されることができる。手術器具1200は、典型的には、トラッキング及び制御システム100、特に、器具コントローラ120に接続するコード1203を有している。
ディスプレイ1402とも称されるディスプレイスクリーン1402が手術器具200、1200と通信状態にあり、切断アクセサリ202を作業境界106において配置及び方位決めするように手術器具200、1200の適切な位置及び方位の指示をユーザに提供している。上記のように、ディスプレイ1402は、作業境界106に対する作業部分の位置を示すように、ナビゲーションシステムと通信状態にある。
いくつかの外科的施術がシステム100及び器具200、1200により行うことができる。これらの施術のうちのいくつかは、骨等の組織の除去を含んでいる。器具200、1200を用いた骨の除去は、器具200、1200に取り付けられた切断アクセサリ202の施術及びタイプに応じて、スカルプティング、シェービング、コアリング、穴開け又は骨を除去する任意の他の方法を含むことができる。器具200、1200は、脊椎、膝、臀部、頭蓋の組織の除去及び他の施術に使用されることができる。これらの施術は、開腹術又は低侵襲施術であることができる。
