Description
本発明は、一般的に医療用デバイス及び方法に関する。特に、本発明は、胃腸消化管中で機能を実行するデバイスおよび方法に関する。
本発明は、患者の胃腸消化管中で一またはそれ以上の機能を実行するデバイス、方法、およびシステムに関する。一般的に、このデバイスは、係留部材と、一またはそれ以上のアクチュエータ、一又はそれ以上のセンサ、あるいは双方の組み合わせとを具える。係留部材は、患者の胃の幽門部内にデバイスを維持し、デバイスが幽門弁を通過することを防ぎ、間欠的に胃壁組織に接触するだけにして、胃壁の糜爛と潰瘍を防ぐ。さまざまな実施例では、胃腸消化管内でさまざまな機能を実行するために、アクチュエータ、センサ及び/又は追加の部品のいずれかが、係留部材と連結されていても良い。自身を胃の中に維持して、幽門弁の通過を防止し、間欠的にのみ胃壁と接触する係留デバイスは、さまざまな治療を管理し、さまざまな特徴を検出し、及び/又は、胃腸消化管内でその他の有益な機能を実行するための、侵襲性が最小のプラットフォームという利点を提供する。
以下の説明のほとんどは、肥満の治療に役立つ幽門弁を間欠的に塞ぐ実施例に焦点を当てている。この説明はほとんど、また、食道を介して送出するためのより小さな形状から、胃の内部にデバイスを確実に滞留させるより大きな形状に拡大する実施例に焦点を当てている。しかしながら、代替の実施例では、デバイスは実際に幽門弁を塞がなくとも良いが、これに代えて、幽門弁を介して食物を通過させる導管として働き、いくつかのケースでは、小腸における栄養の吸収を低減するものである。また、いくつかの実施例は、胃壁の切開を含む外科的手順を介して配置するように構成されており、従って、本発明は、食道を介して送出される拡大するデバイスに限定されない。このように、以下の説明は、主に例示の目的で提供されており、どの実施例も本発明の範囲全体を制限するものと解すべきではない。
Claims (22)
- 患者の胃腸消化管内で一またはそれ以上の機能を実行するデバイスにおいて、当該デバイスが:
前記デバイスの少なくとも一部を患者の胃の幽門部内に維持し、胃組織に直接取り付けることなく間欠的に接触するように構成された係留部材であって、前記胃組織に間欠的に接触する展開構成をとる係留部材と;
近位端部が前記係留部材に結合され、前記係留部材から延在する長さを有し、さらに幽門弁を少なくとも部分的に通るスパニング部材と;
胃腸消化管内の一又はそれ以上の特徴を検出する係留部材に連結した少なくとも一のセンサと;
を具えることを特徴とするデバイス。 - 請求項1に記載のデバイスにおいて、前記少なくとも一のセンサが、pH、温度、胆汁含有率、栄養分含有率、脂肪、糖、アルコール、オピエート、薬物、分析物、電解質、及びヘモグロビンのうちの少なくとも一つを検出するよう構成されていることを特徴とするデバイス。
- 請求項1に記載のデバイスにおいて、更に、患者の胃腸消化管内で一の機能を実行する少なくとも一のアクチュエータを具えることを特徴とするデバイス。
- 請求項3に記載のデバイスにおいて、前記少なくとも一のアクチュエータが、エネルギィ送出部材、前記係留部材に放出可能に連結した物質、吸収低減腸管スリーブ、トリガデバイス、スペース占有デバイス、撮像デバイス、データ送信器、データ保存デバイスから成る群から選択されることを特徴とするデバイス。
- 請求項3に記載のデバイスにおいて、更に、検出した信号に関係するデータを処理して、前記少なくとも一のアクチュエータへこの処理したデータを提供するように構成したプロセッサを具えることを特徴とするデバイス。
- 請求項3に記載のデバイスにおいて、更に、患者の外に配置した、あるいは患者内にインプラントした一またはそれ以上の送信器からの信号を受信する少なくとも一の受信器を具えることを特徴とするデバイス。
- 請求項6に記載のデバイスにおいて、更に、受信した信号を処理して、前記少なくとも一のアクチュエータへこの処理したデータを提供するように構成したプロセッサを具えることを特徴とするデバイス。
- 患者の胃腸消化管内で一の機能を実行するシステムにおいて、当該システムが、胃腸デバイスを具えており、当該胃腸デバイスが:
前記デバイスの少なくとも一部を患者の胃の幽門部内に維持し、胃組織に直接取り付けることなく間欠的に係合するように構成された係留部材であって、前記胃組織に間欠的に接触する展開構成をとる係留部材と;
近位端部が前記係留部材に結合され、前記係留部材から延在する長さを有し、さらに幽門弁を少なくとも部分的に通るスパニング部材と;
患者の胃腸消化管内で一の機能を実行する、前記係留部材に連結された少なくとも一のアクチュエータと;
前記アクチュエータの動きを始動する及び/又は調整するための少なくとも1の送信器と;
を具えることを特徴とするシステム。 - 請求項8に記載のシステムにおいて、更に、胃腸消化管内の一又はそれ以上の特徴を検出する係留部材に連結した少なくとも一のセンサを具えることを特徴とするシステム。
- 請求項9に記載のシステムにおいて、前記少なくとも一のセンサが、pH、温度、胆汁含有率、栄養分含有率、脂肪、糖、アルコール、オピエート、薬物、分析物、電解質、及びヘモグロビンのうちの少なくとも一つを検出するよう構成されていることを特徴とするシステム。
- 請求項9に記載のシステムにおいて、更に、検出した信号に関係するデータを処理して、前記少なくとも一のアクチュエータへこの処理したデータを提供するように構成したプロセッサを具えることを特徴とするシステム。
- 請求項8に記載のシステムにおいて、更に、前記送信器からの信号を受信するための少なくとも一の受信器を具えることを特徴とするシステム。
- 請求項12に記載のシステムにおいて、更に、受信した信号を処理して、前記少なくとも一のアクチュエータへこの処理したデータを提供するように構成したプロセッサを具えることを特徴とするシステム。
- 請求項8に記載のシステムにおいて、更に、患者の外に配置した外付のチャージ装置を介して再チャージするように構成した再チャージ可能な電源を具えることを特徴とするシステム。
- 請求項8に記載のシステムにおいて、前記少なくとも一のアクチュエータが、エネルギィ送出部材、前記係留部材に放出可能に連結した物質、吸収低減腸管スリーブ、トリガデバイス、スペース占有デバイス、撮像デバイス、データ送信器、データ保存デバイスから成る群から選択されることを特徴とするシステム。
- 請求項8に記載のシステムにおいて、前記少なくとも一の送信器が、患者の外に配置されるよう構成されることを特徴とするシステム。
- 請求項8に記載のシステムにおいて、前記少なくとも一の送信器が、患者内にインプラントされるよう構成されることを特徴とするシステム。
- 請求項8に記載のシステムにおいて、更に、患者の食道を介して胃腸デバイスを送出する細長カテーテルデバイスを具えることを特徴とするシステム。
- 請求項8に記載のシステムにおいて、更に、患者の食道を介して胃腸デバイスに連結して、アクチュエータを再チャージする細長カテーテルデバイスを具えることを特徴とするシステム。
- 請求項19に記載のシステムにおいて、細長カテーテルデバイスの磁気端部が、胃腸デバイスの逆にチャージされた磁石に連結するように構成されることを特徴とするシステム。
- 請求項19に記載のシステムにおいて、前記細長カテーテルデバイスは、胃腸デバイスの上の一又はそれ以上の薬物リザーバを再チャージするように構成されることを特徴とするシステム。
- 請求項19に記載のシステムにおいて、前記細長カテーテルデバイスは、胃腸デバイスの電源を再チャージするように構成されることを特徴とするシステム。
