Description
本発明は、一般的に医療用デバイス及び方法に関する。特に、本発明は、胃腸消化管中で機能を実行するデバイスおよび方法に関する。
本発明は、患者の胃腸消化管中で一またはそれ以上の機能を実行するデバイス、方法、およびシステムに関する。一般的に、このデバイスは、係留部材と、一またはそれ以上のアクチュエータ、一又はそれ以上のセンサ、あるいは双方の組み合わせとを具える。係留部材は、患者の胃の幽門部内にデバイスを維持し、デバイスが幽門弁を通過することを防ぎ、間欠的に胃壁組織に接触するだけにして、胃壁の糜爛と潰瘍を防ぐ。さまざまな実施例では、胃腸消化管内でさまざまな機能を実行するために、アクチュエータ、センサ及び/又は追加の部品のいずれかが、係留部材と連結されていても良い。自身を胃の中に維持して、幽門弁の通過を防止し、間欠的にのみ胃壁と接触する係留デバイスは、さまざまな治療を管理し、さまざまな特徴を検出し、及び/又は、胃腸消化管内でその他の有益な機能を実行するための、侵襲性が最小のプラットフォームという利点を提供する。
以下の説明のほとんどは、肥満の治療に役立つ幽門弁を間欠的に塞ぐ実施例に焦点を当てている。この説明はほとんど、また、食道を介して送出するためのより小さな形状から、胃の内部にデバイスを確実に滞留させるより大きな形状に拡大する実施例に焦点を当てている。しかしながら、代替の実施例では、デバイスは実際に幽門弁を塞がなくとも良いが、これに代えて、幽門弁を介して食物を通過させる導管として働き、いくつかのケースでは、小腸における栄養の吸収を低減するものである。また、いくつかの実施例は、胃壁の切開を含む外科的手順を介して配置するように構成されており、従って、本発明は、食道を介して送出される拡大するデバイスに限定されない。このように、以下の説明は、主に例示の目的で提供されており、どの実施例も本発明の範囲全体を制限するものと解すべきではない。
Claims (60)
- 患者の胃腸消化管内で一またはそれ以上の機能を実行するデバイスにおいて、当該デバイスが:
患者の胃の中で第1の形状からより大きな第2の形状に再構成可能であり、前記デバイスの少なくとも一部を患者の胃の幽門部内に維持し、胃組織に直接取り付けることなく間欠的に接触するように構成し、幽門弁から胃に向けて近位に移動可能であり、係留部材と前記幽門弁との間の通路が間欠的に開かれる係留部材と;
前記係留部材に結合される近位端を有し、前記係留部材から延在する長さを有し、さらに、前記幽門弁を少なくとも部分的に通過するよう構成された形状を有する位置決め部材と;
前記位置決め部材の長さの遠位端に結合された保持部材であって、前記保持部材によって前記幽門弁と間欠的に接触するよう前記係留部材を維持するために、展開したときに前記幽門弁を通って通過し、胃の中にとどまりつつ患者の胃腸消化管に配置するための大きさの形状を有する保持部材と;
前記係留部材に連結され、前記患者の胃腸消化管内で作用するために前記胃の中から動作される少なくとも一のアクチュエータと;
を具えることを特徴とするデバイス。 - 請求項1に記載のデバイスにおいて、前記係留部材が、前記位置決め部材の近位の、前記より大きな第2の形状で十分なサイズと剛性を有する支持部であって、前記幽門弁を介して胃滞留部が通過することを防止する支持部を具えることを特徴とするデバイス。
- 請求項2に記載のデバイスにおいて、少なくとも一のアクチュエータが前記胃滞留部に連結されていることを特徴とするデバイwス。
- 請求項2に記載のデバイスにおいて、前記胃滞留部が、患者の食道を通って送出する前記第1の形状から、前記胃滞留部が前記幽門弁を通過することを防止する前記より大きな第2の形状へ拡大可能であることを特徴とするデバイス。
- 請求項2に記載のデバイスにおいて、前記デバイスが、前記胃壁内の切開を介して前記胃内に配置されるように構成されていることを特徴とするデバイス。
- 請求項2に記載のデバイスにおいて、前記係留部材が更に、幽門部の胃組織に、当該組織に有意なダメージを起こすことなく間欠的に係合するように構成された、位置決め部材に隣接する組織接触部を具えることを特徴とするデバイス。
- 請求項6に記載のデバイスにおいて、前記少なくとも一のアクチュエータが前記組織係合部に連結されていることを特徴とするデバイス。
- 請求項6に記載のデバイスにおいて、前記組織係合部が、少なくとも一のコンプライアント材料を具えることを特徴とするデバイス。
- 請求項1に記載のデバイスにおいて、前記少なくとも一のアクチュエータが前記位置決め部材に連結されていることを特徴とするデバイス。
- 請求項1に記載のデバイスにおいて、前記保持部材が、前記患者の十二指腸に存在するように構成されることを特徴とするデバイス。
- 請求項1に記載のデバイスにおいて、前記少なくとも一のアクチュエータが、前記位置決め部材と前記保持部材のうちの少なくとも一方に連結されていることを特徴とするデバイス。
- 請求項1に記載のデバイスにおいて、前記少なくとも一のアクチュエータが、前記保持部材に連結されており、前記患者の小腸へ延在するように構成されていることを特徴とするデバイス。
- 請求項10に記載のデバイスにおいて、前記係留部材、前記位置決め部材、および前記保持部材が、胃腸消化管内にある間に形状を変化するように構成されていることを特徴とするデバイス。
- 請求項1に記載のデバイスにおいて、前記位置決め部材がその長さが変化するように構成されていることを特徴とするデバイス。
- 請求項1に記載のデバイスにおいて、前記位置決め部材がその直径が変化するように構成されていることを特徴とするデバイス。
- 請求項1に記載のデバイスにおいて、前記デバイスの受信器とプロセッサによって一またはそれ以上の信号を受信して処理したときに、前記係留部材が、前記第1の形状から前記より大きな第2の形状に再構成することを特徴とするデバイス。
- 請求項16に記載のデバイスにおいて、前記一またはそれ以上の信号が、高周波信号、電磁信号、マイクロ波信号、および超音波信号からなる群から選択された信号を送信するように構成された一またはそれ以上の外付または内部にインプラントされたデバイスによって送信されることを特徴とするデバイス。
- 請求項1に記載のデバイスにおいて、前記デバイスの少なくとも一のセンサが、前記胃腸消化管中の、pH、温度、胆汁含有率、栄養分含有率、脂肪、糖、アルコール、オピエート、薬物、分析物、電解質、及びヘモグロビンのうちの少なくとも一つを検出したときに、前記係留部材が、前記第1の形状から前記より大きな第2の形状に再構成することを特徴とするデバイス。
- 請求項1に記載のデバイスが更に、前記デバイスに取り付けるための取り付け機構を有するカテーテルデバイスを具えることを特徴とするデバイス。
- 請求項19に記載のデバイスにおいて、前記取り付け機構が磁石を具えることを特徴とするデバイス。
- 請求項1に記載のデバイスにおいて、前記少なくとも一のアクチュエータが、小腸の少なくとも一部内に延在して小腸による栄養の吸収を低減するスリーブを具えることを特徴とするデバイス。
- 請求項21に記載のデバイスが更に、前記スリーブを前記係留部材に連結する少なくとも一の繋索を具えることを特徴とするデバイス。
- 請求項21に記載のデバイスにおいて、前記スリーブが部分的に消化した食物が前記スリーブに入るようにする少なくとも一の近位側開口を具えることを特徴とするデバイス。
- 請求項21に記載のデバイスにおいて、前記スリーブが、栄養の吸収を低減する不浸透性または半浸透性膜を具えることを特徴とするデバイス。
- 請求項1に記載のデバイスにおいて、前記係留部材が前記デバイスに物質を通過させる少なくとも一の通路を具えることを特徴とするデバイス。
- 請求項25に記載のデバイスにおいて、前記アクチュエータが、患者の十二指腸に延在するために少なくとも一の通路と液通するスリーブを具え、前記デバイスと前記スリーブに物質を通過させて、十二指腸による栄養の吸収を低減するようにしたことを特徴とするデバイス。
- 請求項26に記載のデバイスにおいて、前記スリーブが十二指腸を越えて延在しており、栄養の吸収を低減することを特徴とするデバイス。
- 請求項26に記載のデバイスにおいて、前記スリーブが栄養の吸収を低減する不浸透性または半浸透性膜を具えることを特徴とするデバイス。
- 請求項1に記載のデバイスにおいて、前記少なくとも一のアクチュエータが、胃腸消化管の組織にエネルギィを与える少なくとも一のエネルギィ送出部材を具えることを特徴とするデバイス。
- 請求項29に記載のデバイスにおいて、前記少なくとも一のエネルギィ送出部材が、高周波エネルギィ、超音波エネルギィ、マイクロ波エネルギィ、低温エネルギィ、レーザエネルギィ、光エネルギィ、電気エネルギィ、機械エネルギィ、熱エネルギィからなる群から選択されたエネルギィを送出することを特徴とするデバイス。
- 請求項29に記載のデバイスにおいて、前記少なくとも一のエネルギィ送出部材が、患者の胃、幽門弁、および小腸のうちの少なくとも一つへ高周波エネルギィを与えるように構成された複数の高周波電極を具えることを特徴とするデバイス。
- 請求項1に記載のデバイスにおいて、前記少なくとも一のアクチュエータが、前記係留デバイスに放出可能に連結した一又はそれ以上の物質を具えることを特徴とするデバイス。
- 請求項32に記載のデバイスにおいて、前記一またはそれ以上の物質が、脂質、薬物、酵素、診断用薬剤、脂質、ビタミン、およびミネラルからなる群から選択されることを特徴とするデバイス。
- 請求項32に記載のデバイスにおいて、前記少なくとも一の物質が、前記係留デバイスの少なくとも一の外側表面に放出可能に連結されており、当該物質が時間の経過につれて前記表面から自動的に放出されるようにしたことを特徴とするデバイス。
- 請求項34に記載のデバイスが更に、前記デバイスに前記物質を放出可能に連結するために、前記外側表面に連結した基板を具えることを特徴とするデバイス。
- 請求項32に記載のデバイスにおいて、前記少なくとも一の物質が前記係留デバイス上の少なくとも一のリザーバ内に収納されていることを特徴とするデバイス。
- 請求項36に記載のデバイスにおいて、前記少なくとも一の物質が前記少なくとも一のリザーバから時間の経過につれて自動的に放出されることを特徴とするデバイス。
- 請求項36に記載のデバイスにおいて、前記少なくとも一の物質が、前記デバイスが患者の外の送信器から信号を受信したときに前記少なくとも一のリザーバから放出されることを特徴とするデバイス。
- 請求項36に記載のデバイスにおいて、前記少なくとも一のリザーバが胃腸消化管内にデバイスがある間に補充されるように構成されていることを特徴とするデバイス。
- 請求項39に記載のデバイスにおいて、前記リザーバが患者の食道を介して胃に通されたカテーテルデバイスを介して補充されるように構成されていることを特徴とするデバイス。
- 請求項1に記載のデバイスにおいて、前記少なくとも一のアクチュエータが、胃の中でスペースを占有して患者の満腹感を強化する少なくとも一のスペース占有部材を具えることを特徴とするデバイス。
- 請求項41に記載のデバイスにおいて、前記スペース占有部材が、前記係留部材の拡大した部分を具えることを特徴とするデバイス。
- 請求項41に記載のデバイスにおいて、前記スペース占有部材が、繋索を介して前記係留部材に連結されていることを特徴とするデバイス。
- 請求項1に記載のデバイスにおいて、前記少なくとも一のアクチュエータが、生物学的反応を誘発するように構成された一またはそれ以上のトリガを具えることを特徴とするデバイス。
- 請求項44に記載のデバイスにおいて、前記トリガが、満腹感の反応を誘発するように構成された表面被覆を具えることを特徴とするデバイス。
- 請求項45に記載のデバイスにおいて、前記表面被覆が、十二指腸内の生物学的脂質または脂肪センサと相互作用するように構成されていることを特徴とするデバイス。
- 請求項45に記載のデバイスにおいて、前記表面被覆が、時間の経過に従って前記デバイスから溶出して、前記満腹感の反応を誘発するように構成されていることを特徴とするデバイス。
- 請求項47に記載のデバイスにおいて、前記表面被覆が少なくとも、脂肪、脂質、炭水化物、およびたんぱく質の誘導体のうちの少なくとも一つを具えることを特徴とするデバイス。
- 請求項44に記載のデバイスにおいて、前記トリガが、満腹感反応を誘発するように構成された機械的刺激を具えることを特徴とするデバイス。
- 請求項1に記載のデバイスにおいて、前記少なくとも一のアクチュエータが、少なくとも一の撮像デバイスを具えることを特徴とするデバイス。
- 請求項50に記載のデバイスにおいて、前記撮像デバイスが、光ファイバデバイス、超音波デバイス、レーザ撮像デバイス、内視鏡デバイス、カメラ、およびX線撮像デバイスからなる群から選択されることを特徴とするデバイス。
- 請求項1に記載のデバイスにおいて、前記少なくとも一のアクチュエータが、グローバルポジショニングシステムに使用する位置信号を送信する信号送信器を具えることを特徴とするデバイス。
- 請求項1に記載のデバイスにおいて、前記少なくとも一のアクチュエータが、データ保存デバイスを具えることを特徴とするデバイス。
- 請求項53に記載のデバイスにおいて、前記データ保存デバイスが、患者の医療記録を保存するように構成されていることを特徴とするデバイス。
- 請求項1に記載のデバイスが更に、胃腸消化管内の一又はそれ以上の特徴を検出する係留部材に連結した少なくとも一のセンサを具えることを特徴とするデバイス。
- 請求項55に記載のデバイスにおいて、前記少なくとも一のセンサが、pH、温度、胆汁含有率、栄養分含有率、脂肪、糖、アルコール、オピエート、薬物、分析物、電解質、及びヘモグロビンのうちの少なくとも一つを検出するように構成されていることを特徴とするデバイス。
- 請求項55に記載のデバイスが更に、前記検出した信号に関連するデータを処理し、この処理したデータを前記少なくとも一のアクチュエータに提供するように構成したプロセッサを具えることを特徴とするデバイス。
- 請求項1に記載のデバイスが更に、前記患者の外に配置したあるいは前記患者内にインプラントした一又はそれ以上の送信器からの信号を受信する少なくとも一の受信器を具えることを特徴とするデバイス。
- 請求項58に記載のデバイスが更に、前記受信した信号を処理して、この処理した信号を前記少なくとも一のアクチュエータに提供するように構成したプロセッサを具えることを特徴とするデバイス。
