Claims (6)
- 医療システムであって、
患者の心臓にペーシングパルスを送出するための1つまたは複数の電極と、
前記1つまたは複数の電極に結合されるエネルギー送出および検知ユニットと、
肺動脈圧測定を行うことが可能な肺動脈圧センサと、
少なくとも前記肺動脈圧測定結果を格納するよう構成されたメモリと、
前記メモリ、前記肺動脈圧センサ、ならびに前記エネルギー送出および検知ユニットに結合されたコントローラとを備え、前記コントローラは、
短期バーストプロトコルのそれぞれの反復について、前記短期バーストプロトコル中に前記エネルギー送出および検知ユニットによって前記患者の心臓に適用されるペーシングのパラメータの値を変更し、
前記短期バーストプロトコルの各反復の間に、ペーシング無し、または、前もって確立された内因性値でのペーシングを含む検知の介在期間を提供し、
前記短期バーストプロトコルの反復中および前記検知の介在期間に行われた前記肺動脈圧測定結果を格納し、
前記格納された肺動脈圧測定結果によって確定される最適心室前負荷に基づいて選択される前記パラメータの値を使用してペーシング治療を提供するよう構成される、医療システム。 - ペーシングの前記パラメータは、房室遅延、心房内遅延、及び2心室遅延のうちの1つ以上を含む、請求項1に記載の医療システム。
- ペーシングの前記パラメータは、前記電極のペーシング部位を含む、請求項1に記載の医療システム。
- 前記コントローラは、ベースラインの肺動脈圧を確定するようさらに構成され、前記最適心室前負荷は、前記ベースライン肺動脈圧を、前記短期バーストプロトコルの反復中に格納された前記肺動脈圧測定結果と比較することによって確定される、請求項1に記載の医療システム。
- 前記肺動脈圧測定結果は、肺動脈拡張期圧測定結果を含む、請求項1に記載の医療システム。
- 患者の圧反射系が前記短期バーストプロトコルの反復中に、ペーシングの前記変更されたパラメータに対して順応しないように、前記短期バーストプロトコルの反復の長さが選択される、請求項1に記載の医療システム。
