Abstract
Description
非動物性安定化ヒアルロン酸のゲル粒子の調製
例えば米国特許第5,827,937号に既に例示されているように、連鎖球菌(Streptococcus)の発酵によって調製したヒアルロン酸10gを1%NaOH、pH>9 100ml中に溶解した。0.2%濃度に1,4−ブタンジオールジグリシジルエーテルの形の架橋剤を加えた。この溶液を40℃で4時間インキュベートした。
頬及び顎の増大
材料
生理的食塩水中に分散させた非動物性安定化ヒアルロン酸(20mg/ml)からなる透明で無色の粘弾性ゲル。このゲルは、例えば実施例1の方法によって得ることができる。滅菌した試験材料(2ml)を3mlガラス製注入器中に供給し、先端が丸くなった滅菌16G×7又は9cm Coleman浸潤カニューレ(Byron Medical Inc.、米国アリゾナ州Tucson)を用いて皮下及び/又は骨膜上に注入した。
美的な目的で頬及び/又は顎の増大治療を求めているどちらか一方の性別の成人外来患者(>18歳)。試験に含まれることとして、試験の継続期間中、他の美容手法(例えば他の増大治療、ボツリヌス毒素注入、レーザー又は化学薬品による皮膚の再生(skin resurfacing)又は美容整形手段)を控えることへの承諾を求めた。先の6ヶ月以内に顔面組織増大治療又はレーザー/ケミカルピーリング手法或いは先の12ヶ月以内に美的顔面手術を受けた患者は、この試験から除外した。さらに、対象とする処置部位に影響を及ぼしている進行性の皮膚病又は炎症を示す患者、局所麻酔薬に対して既知のアレルギー/過敏症がある又はNASHAに対して以前から副作用がある患者、並びに現在抗凝血薬又は血小板凝集阻害薬を摂取している患者は、参加させなかった。抗凝血薬、アスピリン及び非ステロイド系抗炎症薬の使用は、注入部位が完全に治癒するまで禁止した。
処置部位を消毒液で洗浄し、局所麻酔を必要とした場合、リドカイン(0.5又は1.0%)/アドレナリン溶液を、計画した切開部位に注入した。必要に応じて、局所神経ブロック又はリドカイン/アドレナリンの皮下注入によって提案した埋入部位にさらに麻酔を行った。皮下、筋肉下又は骨膜上の脂肪組織中にゲルを投与するための先端の丸い16Gカニューレの経皮挿入を容易にするために、長さ1〜2mmの皮膚切開をメス(11ブレード)又は鋭い注入針で行った。トンネル技術を用いて、各注入後にカニューレを操作して異なる管に入れることにより、増大を必要とする部位全体に、単回の大量投与ではなく分量に小分けしてゲルを注入した。最大3つの別々の解剖学的部位(顎及び両頬)に、各処置セッションにおいてゲルを最大10ml(注入器5本)投与した。注入を完了してすぐに、処置部位をマッサージして周囲組織の外形に適合させ、必要に応じて、氷を短い間当てて任意の腫れを抑えた。
持続期間がより長い非動物性安定化ヒアルロン酸のゲル粒子の調製
連鎖球菌の発酵によって調製したヒアルロン酸10gを1%NaOH、pH>9 100ml中に溶解した。0.2%濃度に1,4−ブタンジオールジグリシジルエーテルの形の架橋剤を加えた。この溶液を40℃で4時間インキュベートした。
胸部組織増大
胸の小さい女性に、例えば実施例3の方法によって得られるゲルを注入した。先が丸い12G針を用いて、各乳房の腺部下、ちょうど胸筋上にゲル100mlを小分けして埋入した。天然組織を妨げないように注意した。埋入の12ヶ月後、胸は、薄い結節状インプラントを含み引き続き良好な形であった。インプラントによってマンモグラフィ分析がぼけることはなかった。
Claims (25)
- 生理的食塩水にさらした時のサイズが1〜5mmの範囲の注入用ゲル粒子である、粘弾性媒質の粒子。
- 前記サイズが1〜2.5mmの範囲である、請求項1に記載の粒子。
- 前記サイズが2.5〜5mmの範囲である、請求項1に記載の粒子。
- 前記粘弾性媒質が多糖類及びその誘導体からなる群から選択される、請求項1から3までのいずれか一項に記載の粒子。
- 前記粘弾性媒質が安定化グリコサミノグリカン及びその誘導体から選択される、請求項1から3までのいずれか一項に記載の粒子。
- 前記粘弾性媒質が安定化ヒアルロン酸、安定化コンドロイチン硫酸、安定化ヘパリン、及びその誘導体からなる群から選択される、請求項1から3までのいずれか一項に記載の粒子。
- 前記粘弾性媒質が架橋ヒアルロン酸及びその誘導体からなる群から選択される、請求項1から3までのいずれか一項に記載の粒子。
- 生理的食塩水にさらした時の前記ゲル粒子中の前記粘弾性媒質の濃度が5〜100mg/mlの範囲である、請求項7に記載の粒子。
- 15〜50Nの圧力を加えることにより20ゲージ以上の針によって注入可能である、前記請求項のいずれか一項に記載の粒子。
- (i)所望の濃度の粘弾性媒質を含むゲルを製造するステップと、
(ii)生理的食塩水にさらした時のサイズが1〜5mmの範囲のゲル粒子になるよう前記ゲルを機械的に破壊するステップ
を含む、粘弾性媒質の注入用ゲル粒子を生成する方法。 - 粘弾性媒質の粒子を含む軟組織増大インプラントであって、大部分の前記粒子が、生理的食塩水にさらした時のサイズが1〜5mmの範囲の注入用ゲル粒子である、軟組織増大インプラント。
- 前記サイズが1〜2.5mmの範囲である、請求項11に記載の軟組織増大インプラント。
- 前記サイズが2.5〜5mmの範囲である、請求項11に記載の軟組織増大インプラント。
- ヒトを含む哺乳動物における軟組織増大の方法であって、粘弾性媒質の注入用ゲル粒子を含むインプラントを前記哺乳動物における軟組織増大を所望する部位に表皮下投与することを含み、大部分の前記粒子の生理的食塩水にさらした時のサイズが1〜5mmの範囲である方法。
- 前記投与が皮下投与である、請求項14に記載の方法。
- 前記投与が筋肉下投与である、請求項14に記載の方法。
- 前記投与が骨膜上投与である、請求項14に記載の方法。
- 前記サイズが1〜2.5mmの範囲である、請求項14から17までのいずれか一項に記載の方法。
- 前記サイズが2.5〜5mmの範囲である、請求項14から17までのいずれか一項に記載の方法。
- 前記軟組織増大の部位が、顔面組織と露出皮膚によって覆われた他の組織とから選択される、請求項18に記載の方法。
- 前記投与が単回投与である、請求項14から20までのいずれか一項に記載の方法。
- 前記投与が多層投与である、請求項14から20までのいずれか一項に記載の方法。
- 医薬品として使用するための、請求項1から9までのいずれか一項に記載の注入用ゲル粒子。
- 医薬品として使用するための、請求項1から9までのいずれか一項に記載の注入用ゲル粒子を含む注入用軟組織増大インプラント。
- ヒトを含む哺乳動物における治療的軟組織増大のための医薬品を製造することを目的とする、請求項1から9までのいずれか一項に記載の粘弾性媒質の注入用ゲル粒子の使用であって、大部分の前記粒子の生理的食塩水にさらした時の平均サイズが1〜5mmの範囲であり、前記医薬品は、前記哺乳動物における治療的軟組織増大を所望する部位に請求項14から22までのいずれか一項に従って表皮下投与するのに適している使用。
