Abstract
Un instrumento quirúrgico, que comprende: un mango (14) que tiene un miembro tubular alargado (16) que se extiende en dirección distal desde el mango; un efector final (12) previsto en un extremo distal del miembro tubular alargado; una varilla de accionamiento (18) montada para movimiento con relación al miembro tubular alargado, para operar el efector final; un mecanismo de almacenamiento de energía (20) para almacenar y proporcionar energía para mover la varilla de accionamiento; y un mecanismo de actuación (74); en que el mecanismo de actuación incluye un sistema de amortiguación operable en el mecanismo de almacenamiento de energía para controlar el régimen de liberación de la energía almacenada en el mecanismo de almacenamiento de energía (54, 160, 254, 354, 360, 460, 470); el mecanismo de almacenamiento de energía incluye: un resorte (24) y un émbolo (28) que puede operar sobre el resorte; un vástago de émbolo (30) fijado al émbolo y susceptible de aplicación con la varilla de accionamiento; y caracterizado porque el mecanismo de almacenamiento de energía incluye: una estructura de rueda dentada (42, 44, 46) en el vástago de émbolo susceptible de operar con la estructura de rueda dentada (52) en la varilla de accionamiento para mover la varilla de accionamiento en respuesta al movimiento del vástago de émbolo.
Description
En ciertos dispositivos, tales como los instrumentos quirúrgicos, es deseable almacenar energía dentro del instrumento para facilitar el disparo del instrumento. Uno de tales dispositivos es un aplicador de grapas descrito en la Patente de EE.UU. Número 4.611.595. Este aplicador de grapas almacena 5 energía en un resorte, el cual está retenido por un mecanismo de cerrojo. El movimiento de un mango desaplica el cerrojo dando por resultado la rápida e incontrolada liberación de la energía almacenada, para accionar el aplicador de grapas. Sin embargo, en algunos instrumentos sería deseable tener una estructura para permitir una liberación controlada de la energía almacenada, para controlar el régimen de actuación del instrumento. 10
Además, muchos instrumentos quirúrgicos, tales como las grapadoras de 60 mm usadas en tejido grueso, requieren más energía, que puede ser desarrollada cómodamente mediante un simple apretón con una mano humana. Actualmente, los cirujanos pueden usar las dos manos para disparar, o usar los instrumentos que requieran múltiples aplicaciones de esfuerzos más bajos para dispararlos. Por consiguiente, sería también deseable disponer de un instrumento quirúrgico que pueda ser previamente 15 activado mediante bombeo, al menos una vez y posiblemente varias veces, para almacenar energía potencial en un resorte. Este preactivado puede ser hecho por una enfermera quirúrgica u otro ayudante. Esto es particularmente importante durante los procedimientos largos en los que la fatiga, ocasionada por los múltiples disparos, puede ser incomoda para el cirujano.
En el documento FR 2701832 se describe un mango para un instrumento quirúrgico que tiene 20 una disposición de diente y cremallera.
En el documento US 6.443.973 se describe un instrumento quirúrgico de grapar que tiene un motor para accionar un eje de accionamiento para hacer que actúe el instrumento de grapar.
Se proporciona un instrumento quirúrgico para uso para hacer que actúen varios efectores 25 finales. El instrumento quirúrgico incluye generalmente un mango que tiene un miembro tubular alargado que se extiende distalmente desde el mango. Los varios efectores finales pueden ser previstos en el extremo distal del miembro tubular alargado y ser accionados mediante un accionador, tal como por ejemplo, una varilla de accionamiento, que vaya montada para movimiento con relación al miembro alargado. El instrumento quirúrgico incluye también un mecanismo de almacenamiento de energía para 30 almacenar y proporcionar energía para mover la varilla de accionamiento y por consiguiente hacer actuar los efectores finales. Se han descrito varias realizaciones de mecanismos de actuador que son operables en el mecanismo de almacenamiento de energía para refrenar y controlar el régimen de liberación de la energía almacenada en el mecanismo de almacenamiento de energía.
El mecanismo de almacenamiento de energía incluye generalmente un cilindro que tiene un 35 émbolo cargado por resorte situado en el mismo. Un vástago de émbolo está unido al émbolo y se extiende a través del cilindro para aplicar la estructura de engranaje asociada con la varilla de accionamiento. El mango está previsto en un segundo extremo del vástago de émbolo para comprimir el resorte y el émbolo dentro del cilindro. La liberación de la energía almacenada en el resorte fuerza a moverse al vástago de émbolo en una dirección en que hace actuar a la estructura de engranaje para 40 accionar la varilla de accionamiento, y, por consiguiente, hacer actuar los efectores finales.
En una primera realización, un sistema de fluido está asociado con el cilindro y el émbolo para refrenar y amortiguar el movimiento del émbolo. Se ha previsto un sistema de transferencia para transferir el fluido desde un lado del émbolo al otro lado del émbolo, dentro del cilindro. El sistema de transferencia puede incluir, además, un mecanismo de derivación que permite al cirujano comprimir el resorte y el 45 vástago y, por consiguiente, transferir el fluido dentro del cilindro.
Un sistema de actuación de una realización alternativa utiliza un mecanismo de freno operable sobre la estructura de engranaje que conecta el vástago de émbolo con la varilla de accionamiento. La depresión de un disparador libera la presión del freno sobre la estructura de engranaje, permitiendo que el instrumento sea disparado. 50
Análogamente, en otra realización se utiliza un mecanismo de freno similar para controlar el régimen de liberación de la energía almacenada en el mecanismo de almacenamiento de energía. Esta realización utiliza una transmisión articulada, particularmente útil para proporcionar una ventaja mecánica y por consiguiente una mayor presión sobre el mecanismo de engranaje en situaciones en las que el resorte sea de una fuerza particularmente alta. 55
También se describen realizaciones adicionales del instrumento quirúrgico en las que se utiliza un volante para proporcionar un efecto de amortiguación sobre el mecanismo de engranaje al ser accionado el mecanismo de engranaje por el vástago de émbolo. En una realización, una rueda de engranaje loca está unida al volante, y se ha previsto una superficie de leva en un disparador para aplicación de fricción al volante y, por consiguiente, controlar el régimen de liberación de energía 5 almacenada en el resorte. En una realización alternativa, una estructura de múltiples engranajes está asociada con el volante para proporcionar mayores ventajas mecánicas, tales como las que aquí se exponen.
DESCRIPCIÓN DE LOS DIBUJOS

DESCRIPCIÓN DETALLADA
Se exponen varias realizaciones de un conjunto de mango en el cual la energía es almacenada en un resorte por una o más acciones manuales de entrada y es refrenada por varias estructuras de actuación. Cuando el cirujano acciona un disparador, la energía es liberada a regímenes controlables, 25 permitiendo con ello que la energía almacenada en el resorte sea entregada al mecanismo de trabajo para efectuar la función deseada.
En el sistema van incorporados unos medios de amortiguación, para controlar el régimen de liberación de energía para una acción deliberadamente suave. La amortiguación puede ser hidráulica, neumática, mecánica o por otros medios adecuados. Si se desea, puede efectuarse la liberación 30 controlable por el cirujano a través del uso de una válvula, un freno, un volante u otros medios de control adecuados, para así permitirle actuar más rápidamente, más lentamente, o incluso hacer una pausa en la acción de liberación.
Con referencia a la FIG. 1, se describe un instrumento 10 para accionar varios efectores finales 12. El instrumento 10 incluye generalmente un cuerpo de mango 14, un miembro tubular alargado 16 que 35 se extiende en dirección distal desde el cuerpo de mango 14 y una varilla de accionamiento 18 movible longitudinalmente en el mismo para accionar el efector final 12. Aunque el miembro tubular alargado 16 se ha ilustrado como fijado al cuerpo 14 del mango, el miembro tubular alargado 16 puede ser montado, como alternativa, para rotación con respecto al cuerpo 14 del mango.
Se ha previsto un sistema de almacenamiento de energía 20 para almacenar energía y 40 proporcionarla a la varilla de accionamiento 18 al ser liberada. El sistema de almacenamiento de energía 20 incluye generalmente un cilindro 22 que tiene un resorte compresible 24, tal como, por ejemplo, un resorte de estampa, montado en el mismo. El resorte 24 está situado entre una tapa inferior 26 del cilindro 22 y un émbolo 28 montado a deslizamiento en el cilindro 22. Un vástago de émbolo 30 se extiende a través del cilindro 22 y está fijado al émbolo 28. Un mango de activación 32 está montado en un punto A 45 de pivote en un soporte 34 en el cuerpo 14 del mango. Un extremo inferior 36 del vástago de émbolo 30 está montado a pivotamiento en el mango 32 de activar en un punto de pivote B y se desplaza en la ranura E del mango 32 de activación. Un extremo superior 38 del vástago de émbolo 30 está conectado a una cremallera en L 40 que tiene dientes 42 de la cremallera en L. Una rueda dentada grande 44 y una rueda dentada pequeña 46 están fijadas cada una a la otra y están montadas para rotación en el cuerpo 50 14 del mango. Los dientes 48 de la rueda dentada pequeña 46 engranan con los dientes 42 en L de la cremallera 40 en L, mientras que los dientes 50 de la rueda dentada grande 44 engranan con los dientes de accionamiento 52 de la varilla de accionamiento 18. Las configuraciones de las relaciones de engranajes son tales que un movimiento en una distancia d1 del vástago de émbolo 30 da por resultado un movimiento en una distancia d2 del vástago de accionamiento. En una realización útil, un movimiento 55
en una distancia d1=25 mm, da por resultado un movimiento en una distancia d2=66 mm aproximadamente.
Girando el mango de activación 32 alrededor del pivote A, en la dirección de la flecha F, se tira del vástago de émbolo 30 haciendo que comprima al resorte 24 entre el émbolo 28 y la tapa inferior 26. Al tirar del vástago de émbolo 30 se tira de la cremallera 40 en L, haciendo que los dientes 42 de la 5 cremallera en L hagan girar a las ruedas dentadas grande y pequeña 44 y 48 en sentido a derechas, por aplicación con los dientes 48 de la rueda dentada pequeña 46. Al girar la rueda dentada grande 44 en sentido a derechas, los dientes 50 tiran de la varilla de accionamiento 18 en dirección proximal por aplicación con los dientes de accionamiento 52. Este movimiento lleva la varilla de accionamiento 18 a una posición proximal. 10
Como se ha indicado en lo que antecede, se han previsto varias realizaciones de un mecanismo de actuación para refrenar el resorte comprimido 24 y permitir una liberación controlada de la energía almacenada en el mismo. Siguiendo con referencia a la FIG. 1, se ha provisto una primera realización de un mecanismo actuador que tiene un sistema hidráulico 54 para controlar la liberación del resorte 24 y proporcionar un mayor control de la actuación de los efectores finales 12 del instrumento. El cilindro 22 15 está sellado mediante una tapa superior 56 y una tapa inferior 26, y está provisto de un fluido hidráulico incompresible 58 a cada lado del émbolo 28. En una realización, el émbolo 28 está provisto de un aro tórico 60 para aislar el fluido 58 a uno y otro lado del émbolo 28, y permitir un movimiento suave del émbolo 28 dentro del cilindro 22.
Se ha previsto un sistema de transferencia 62 entre una lumbrera superior 64 y una lumbrera 20 inferior 66 en las tapas superior e inferior 56, 26, para mover el fluido hidráulico 58 de un lado del émbolo 28 al otro lado, dentro del cilindro 22, al ser comprimido el resorte de estampa 24. El sistema de transferencia 62 incluye tubos superior e inferior 68, 70 que están en comunicación de fluido con las lumbreras superior e inferior 64, 66. El movimiento del émbolo 28 dentro del cilindro 22 fuerza al fluido hidráulico 58 desde un lado del émbolo 28 al otro a través de los tubos superior e inferior 68, 70. 25
El sistema de actuación incluye también una válvula 72 situada entre los tubos superior e inferior 68, 70 respectivamente, y un disparador de válvula 74 para accionar la válvula 72. La depresión del disparador de válvula 74 abre gradualmente la válvula 72 para permitir el flujo de fluido hidráulico 58. Cuando no está oprimido el disparador de la válvula 74, la válvula 72 está cerrada y no puede circular fluido 58 a su través. 30
A fin de permitir el movimiento de fluido 58 desde un lado del émbolo 28 al otro durante la compresión del resorte 24, se ha dispuesto un sistema de derivación 76 que tiene una válvula 78 de retención en un solo sentido, situada alrededor de la válvula 72 y entre los tubos superior e inferior 68, 70, respectivamente. La válvula 78 de retención en un solo sentido actúa como un cerrojo o mecanismo de retención que solamente permitirá en flujo de fluido 58 en la dirección desde el tubo inferior 70 al tubo 35 superior 68 durante la compresión del resorte 24.
Para operar, se acciona el mango 32 de activación para comprimir el resorte 24 y tirar de la varilla de accionamiento 18 en dirección proximal, como se ha descrito aquí en lo que antecede. Se cierra la válvula 72, y la válvula 78 de retención permite que el fluido fluya desde el tubo inferior 70 al tubo superior 68. Una vez que el resorte 24 está totalmente comprimido, la presión del fluido 58 en una cámara 40 superior 80 del cilindro 22 mantiene al resorte 24 en compresión. El fluido 58 no puede fluir de vuelta a través del sistema de derivación 78 y la válvula 72 está cerrada. Para liberar de modo controlable la presión del resorte 24 y por consiguiente accionar el instrumento, se oprime el disparador 74 de la válvula para permitir que el fluido 58 fluya desde el tubo superior 68 al tubo inferior 70 y disminuya la presión en la cámara superior 80 al ser transferido el fluido a una cámara inferior 82. Al liberar el resorte 24, el 45 vástago de émbolo 30 se mueve hacia arriba, haciendo girar a las ruedas de engranaje grande y pequeña 44 y 46 en sentido de rotación a izquierdas e impulsando la varilla de accionamiento 18 en dirección distal para accionar al efector final 12. La liberación de la presión, y por consiguiente la actuación del instrumento, pueden controlarse con precisión por parte del operador. Manipulando el disparador 74 de la válvula, la actuación puede ser instantánea y rápida, lenta y gradual, o incluso intermitente, mediante una 50 pequeña depresión repetida del disparador 74 de la válvula.
Otra función del sistema hidráulico 54 es la de amortiguar el movimiento del émbolo 28 dentro del cilindro 22 para proporcionar una liberación suave y controlada de la presión del resorte 24 y, por consiguiente, una actuación muy suave del instrumento.
Con referencia ahora a la FIG. 2, se ha descrito una segunda realización de un instrumento 55 quirúrgico que tiene un sistema de almacenamiento de energía. La segunda realización incorpora un sistema de freno para liberar de modo controlable la presión del resorte. Es de hacer notar que los elementos comunes de la realización aquí descrita son sustancialmente similares a los de la primera
realización. El instrumento 110 está diseñado para proporcionar actuación de los efectores finales 112 asociados.
El instrumento 110 incluye generalmente un cuerpo 114 de mango que tiene un miembro tubular alargado 116 que se extiende en dirección distal desde el mismo. Una varilla de accionamiento 118 está montada de modo movible en el cuerpo 114 del mango y en el miembro tubular alargado 116 y es 5 alimentado de energía por un sistema de almacenamiento de energía 120, de la manera que se ha descrito en lo que antecede con respecto al sistema de almacenamiento de energía 20. El sistema de almacenamiento de energía 120 incluye un cilindro 122 que tiene un resorte 124, tal como un resorte de estampa, situado entre una tapa inferior 126 y un émbolo 128. Un vástago de émbolo 130 está fijado al émbolo 128. Un mango de activación 132 está montado a pivotamiento alrededor del punto A para 10 manipular el cuerpo 114 en el soporte 134. Un extremo inferior 136 del vástago de émbolo 130 está fijado al mando de activación 132 y pivota en el punto B y se desplaza en una ranura E en el mango de activación 132. El sistema de almacenamiento de energía 120 incluye también el extremo superior de la varilla de émbolo 138 unido a una cremallera en L 140 que tiene dientes de cremallera en L 142. Las ruedas de engranaje grande y pequeña 144, 146 que tienen dientes de engranaje 150 y 148, 15 respectivamente, funcionan juntamente con los dientes de accionamiento 152 en la varilla de accionamiento 118 de la manera que se ha descrito en lo que antecede para alimentar de energía a la varilla de accionamiento 118 en respuesta a la liberación de la energía almacenada en el resorte 124.
En la realización de la FIG. 2, el aire llena el cilindro 122 y las holguras superior e inferior 154, 156 formadas en el cilindro 122 alrededor del vástago de émbolo 130 permiten que fluya el aire 20 libremente entrando y saliendo del cilindro 122. Con objeto de refrenar el resorte 124 en el estado de comprimido y permitir una liberación controlada de la presión del resorte 124 y por consiguiente la actuación del instrumento, se ha previsto un sistema de freno 158 el cual actúa por fricción sobre la rueda dentada grande 144. El sistema de freno 158 incluye una zapata de freno 160 que se aplica a fricción a la rueda dentada grande 144. Se ha previsto un resorte de carga 162 para cargar a la zapata de freno 160 a 25 aplicación con la rueda dentada grande 144 con una fuerza suficiente como para impedir la rotación de la rueda dentada grande 144 e impedir la liberación del resorte comprimido 124.
Un disparador 164 está montado a pivotamiento en el alojamiento en el pivote C y conectado a la zapata de freno 160 por una barra de transferencia 166. La depresión del disparador 164 mueve la zapata de freno 160 gradualmente fuera de aplicación con la rueda dentada grande 144 y contra la carga del 30 resorte de carga 162. Esto permite la liberación controlada de la presión del resorte en el resorte comprimido 124 y por consiguiente la actuación controlable del instrumento.
Con referencia ahora a la FIG. 3, se describe una realización alternativa de un instrumento quirúrgico que tiene otro tipo de conjunto de freno para la liberación controlada del resorte de estampa y la actuación del instrumento. El instrumento 210 incluye generalmente un cuerpo de mango 214 que tiene 35 un miembro tubular alargado 216 que se extiende en dirección distal desde el mismo. Una varilla de accionamiento 218 está montada de modo movible en el cuerpo de mango 214 y en el miembro tubular alargado 216 y está alimentada de energía por un sistema de almacenamiento de energía 220 de la manera descrita en lo que antecede con respecto al sistema de almacenamiento de energía 20. El sistema de almacenamiento de energía 220 incluye un cilindro 222 que tiene un resorte 224, tal como un 40 resorte de estampa, situado entre una tapa inferior 226 y un émbolo 228. Un vástago de émbolo 230 está fijado al émbolo 228. Un mango de activación 232 está montado a pivotamiento alrededor del punto A para manejar el cuerpo 214 en el soporte 234. Un extremo inferior 236 del vástago de émbolo 230 está fijado al mando de activación 232 y pivota en el punto B y se desplaza en la ranura E en el mango de activación 232. El sistema de almacenamiento de energía 220 incluye también el extremo superior 238 del 45 vástago de émbolo unido a una cremallera en L 240 que tiene dientes de cremallera en L 242. Las ruedas dentadas grande y pequeña 244, 246 que tienen dientes de engranaje 250 y 248, respectivamente, funcionan juntamente con los dientes de accionamiento 252 en la varilla de accionamiento 218 de la manera que se ha descrito en lo que antecede para alimentar de energía a la varilla de accionamiento 218 en respuesta a la liberación de la energía almacenada en el resorte 224. 50
Como se ha indicado en lo que antecede, se ha previsto un sistema de actuación para controlar el régimen de liberación del resorte 224. Se ha previsto un conjunto de actuación o de freno 254 similar al de la FIG. 2. El conjunto de freno 254 incluye un freno 256 que está previsto para aplicarse a pivotamiento a la rueda dentada grande 244 para controlar el movimiento de la misma. Un disparador 258 está montado a pivotamiento en el cuerpo 214 del mango en un punto fijo en general central. El disparador 258 55 es relativamente largo para proporcionar una acción de palanca adicional y reducir la presión en el disparador 258 necesaria para accionar el instrumento. Un primer extremo 260 del disparador 258 se extiende fuera del cuerpo 214 del mango, y un segundo extremo 262 está conectado al freno 256 por una transmisión articulada 264. Un resorte de carga 266 dispuesto en el cuerpo de mango 214 actúa sobre el segundo extremo 262 para cargar al freno 258 a aplicación con la rueda dentada grande 244. 60
De una manera similar a la de la realización de la FIG. 2, la depresión del disparador 258 eleva el freno 256 separándolo de la rueda dentada grande 244 para liberar de modo controlable la energía almacenada en el resorte 224 y accionar el instrumento.
Con referencia ahora a la FIG. 4, se ha descrito un sistema de actuación que tiene un mecanismo de volante para liberación controlada de la energía almacenada en el resorte y por 5 consiguiente la actuación del instrumento. El instrumento 310, como para las anteriores realizaciones, está diseñado para proporcionar la actuación del instrumento asociado 310. El instrumento 310 incluye en general un cuerpo de mango 314 que tiene un miembro tubular alargado 316 que se extiende en dirección distal desde el mismo. Es de hacer notar que en todas las realizaciones aquí descritas, el miembro tubular alargado puede estar montado para rotación sobre el cuerpo 314 del mango. Una varilla de accionamiento 10 318 está montada de modo movible en el cuerpo 214 del mango y en el miembro tubular alargado 316 y está alimentada de energía por un sistema de almacenamiento de energía 320. El sistema de almacenamiento de energía 320 incluye un cilindro 322 que tiene un resorte 324 tal como un resorte de estampa, situado entre una tapa inferior 326 y un émbolo 328. Un vástago de émbolo 330 está fijado al émbolo 328. Un mango de activación 332 está montado a pivotamiento alrededor del punto A en el cuerpo 15 314 del mango en el soporte 334. Un extremo inferior 336 del vástago de émbolo 330 está montado a pivotamiento en el mango de activación 332 y pivota en el punto B y se desplaza en una ranura E en el mango de activación 232, como se ha ilustrado. El sistema de almacenamiento de energía 320 incluye también el extremo superior 328 del vástago de émbolo unido a una cremallera en L 340 que tiene dientes de cremallera en L 342. Las ruedas dentadas grande y pequeña 344 y 346, que tienen dientes de 20 engranaje 350 y 348, respectivamente, funcionan juntamente con los dientes de accionamiento 352 en la varilla de accionamiento 318 de la manera descrita en lo que antecede. Para alimentar de energía la varilla de accionamiento 318 en respuesta a la liberación de la energía almacenada en el resorte 324.
Se ha previsto un sistema de actuación alternativo para controlar la liberación del resorte 324 mediante la implementación de una estructura de volante para suavizar la actuación del instrumento 310. 25 Un volante 354 de diámetro relativamente grande está montado para rotación en el alojamiento y conectado a la rueda dentada grande 344 por una rueda dentada loca 356 de pequeño diámetro. Un disparador 338 está montado a pivotamiento en el cuerpo 314 del mango e incluye una superficie de leva 360 susceptible de aplicación con el volante 354. Un resorte de disparador 362 carga al disparador 338 de tal modo que la superficie de leva 360 en el disparador 338 se aplica firmemente al volante 354 para 30 frenar al resorte comprimido 324. La diferencia de diámetros entre el volante 354 y la rueda dentada loca 358 proporciona una ventaja mecánica, con lo cual se reduce la presión sobre el disparador 338 que se necesita para accionar el instrumento.
La actuación del disparador 338 contra la carga del resorte de disparador 362 lleva a la superficie de leva 360 fuera de aplicación de fricción con el volante 354. Esto permite la liberación controlada de la 35 energía almacenada en el resorte 324 y la actuación controlada del instrumento quirúrgico 310. Es importante señalar que la inercia del volante 354 ayuda a retardar la iniciación de la rotación y por consiguiente la actuación del instrumento 310. Esta aceleración retardada de la liberación de la presión del resorte permite una actuación más suave y más lenta de la varilla de accionamiento 318.
Con referencia ahora a la FIG. 5, el instrumento 410, al igual que en las anteriores realizaciones, 40 está diseñado para proporcionar la actuación del instrumento asociado 410. El instrumento 410 incluye en general un cuerpo de mango 414 que tiene un miembro tubular alargado 416 que se extiende en dirección distal desde el mismo. Es de hacer notar que en todas las realizaciones aquí descritas, el miembro tubular alargado puede estar montado para rotación sobre el cuerpo 114 del mango. Una varilla de accionamiento 418 está montada de modo movible en el cuerpo 414 del mango y en el miembro tubular alargado 416 y 45 es alimentada de energía por un sistema de almacenamiento de energía 420. El sistema de almacenamiento de energía 420 incluye un cilindro 422 que tiene un resorte 424, tal como un resorte de estampa, situado entre una tapa inferior 426 y un émbolo 428. Un vástago de émbolo 430 está fijado al émbolo 428. Un mango de activación 432 está montado a pivotamiento alrededor del punto A para manejar el cuerpo 414 en el soporte 434. Un extremo inferior 438 con el vástago de émbolo 430 está 50 fijado al mango de activación 432 y pivota en el punto B y se desplaza en una ranura E en el mango de activación 432, como se ha ilustrado. El sistema de almacenamiento de energía 420 incluye también el extremo superior 428 del vástago de émbolo unido a una cremallera en L 440 que tiene dientes de cremallera en L 442. Las ruedas dentadas grande y pequeña 444 y 446, que tienen dientes de engranaje 450 y 448, respectivamente, funcionan juntamente con los dientes de accionamiento 452 en la varilla de 55 accionamiento 418, de la manera descrita en lo que antecede, para alimentar de energía a la varilla de accionamiento 418 en respuesta a la liberación de la energía almacenada en el resorte 424.
Se ha previsto un sistema de actuación alternativo para controlar la liberación del resorte 424 mediante la implementación de una estructura de volante de múltiples ruedas dentadas para suavizar la actuación del instrumento 410. 60
Una rueda dentada intermedia 454 está fijada a la rueda dentada loca 456, de tal modo que los dientes de la rueda dentada loca 458 engranan con los dientes de la rueda dentada grande 450. Un volante 460 que tiene una rueda dentada de accionamiento 462 fijada al mismo, está montado para rotación en el cuerpo del mango 414. Los dientes 464 de la rueda dentada de accionamiento de la rueda dentada 462 de accionamiento son susceptibles de engranar con los dientes 466 de la rueda dentada 5 intermedia 454 para hacer girar al volante 460. Un disparador 468 está montado a pivotamiento en el cuerpo 414 del mango e incluye un mecanismo de freno similar al de las realizaciones anteriormente descritas, que incluye una superficie de leva 470 susceptible de aplicación con el volante 460. Las superficies de contacto de la superficie de leva 470 y el volante 460 son relativamente lisas, para proporcionar una acción de frenado suave y progresivo entre ellas. Un resorte de disparador 472 carga al 10 disparador 468 a aplicación con el volante 460.
La operación es similar a la de las realizaciones anteriormente descritas, por cuanto el instrumento 410 es activado haciendo girar el mango de activación 432 para comprimir el resorte 424. Una aplicación de fricción de la superficie de leva 470 sobre el disparador 468 contra el volante 460 impide la liberación de la energía. Para hacer que actúe el instrumento 410, se deprime el disparador 468, 15 disminuyendo la aplicación de la superficie de leva 470 con el volante 460. Variando el grado de presión en el disparador 468 se puede variar la cantidad de contacto entre la superficie de leva 470 y el volante 460 para variar con ello el régimen de activación del instrumento 410. Además, se obtienen ciertas ventajas mecánicas mediante el uso de mecanismos de múltiples ruedas dentadas. Los mecanismos de múltiples ruedas dentadas permiten que sea aplicada una fuerza relativamente grande para controlar el 20 régimen de liberación de energía almacenada en el resorte, en respuesta a una fuerza relativamente pequeña aplicada a los disparadores. Además, el uso de múltiples ruedas dentadas que tengan diferentes diámetros permite la multiplicación de la fuerza aplicada a la varilla de accionamiento y por consiguiente a los efectores finales.
Está además contemplado que la acción de entrada para almacenar energía pueda ser 25 efectuada por un motor, de tal modo que la energía pueda comenzar por ser transferida al resorte durante el período de tiempo durante el que haya de ser remplazado un cartucho o una unidad de carga desechable, o incluso antes, inmediatamente después del disparo anterior. Puesto que se ha ampliado el período de tiempo de transferencia de energía desde el motor al resorte, se pueden usar un motor y una batería (o suministro de energía) más ligeros. 30
Se ha contemplado además que, aunque en las realizaciones aquí representadas se utiliza una empuñadura del tipo de pistola, se pueden usar también otros tipos de empuñadura tales como, por ejemplo, recta, descentrada, etc., con los sistemas de almacenamiento de energía y de actuación descritos. Además, se pueden emplear otros dispositivos de prearmado para el mango de activación para pivotamiento, tales como, por ejemplo, una leva giratoria, una palanca compuesta de tornillo de avance, 35 una palanca acodada, un cable de tracción, etc.
Aunque se han representado en los dibujos varias realizaciones de lo que se describe, no se ha pretendido que la exposición quede limitada a ellas, puesto que se pretende que la exposición sea de alcance tan amplio como lo permita la técnica y que la memoria descriptiva sea entendida de igual modo. Por lo tanto, la anterior descripción no debe ser entendida como limitadora, sino simplemente como 40 ejemplos de realizaciones preferidas. Quienes sean expertos en la técnica contemplarán otras modificaciones dentro del alcance de esta exposición.
Claims (10)
- REIVINDICACIONES1. Un instrumento quirúrgico, que comprende:un mango (14) que tiene un miembro tubular alargado (16) que se extiende en dirección distal desde el mango;un efector final (12) previsto en un extremo distal del miembro tubular alargado; 5una varilla de accionamiento (18) montada para movimiento con relación al miembro tubular alargado, para operar el efector final;un mecanismo de almacenamiento de energía (20) para almacenar y proporcionar energía para mover la varilla de accionamiento; yun mecanismo de actuación (74); 10en que el mecanismo de actuación incluye un sistema de amortiguación operable en el mecanismo de almacenamiento de energía para controlar el régimen de liberación de la energía almacenada en el mecanismo de almacenamiento de energía (54, 160, 254, 354, 360, 460, 470);el mecanismo de almacenamiento de energía incluye:un resorte (24) y un émbolo (28) que puede operar sobre el resorte; 15un vástago de émbolo (30) fijado al émbolo y susceptible de aplicación con la varilla de accionamiento; ycaracterizado porque el mecanismo de almacenamiento de energía incluye:una estructura de rueda dentada (42, 44, 46) en el vástago de émbolo susceptible de operar con la estructura de rueda dentada (52) en la varilla de accionamiento para mover la varilla de 20 accionamiento en respuesta al movimiento del vástago de émbolo.
- 2. El instrumento quirúrgico según la reivindicación 1, en el que el mecanismo de almacenamiento de energía incluye un mango de activación (32) montado en el mango y conectado al vástago de émbolo, de tal modo que el movimiento del mango de activación comprime al resorte.
- 3. El instrumento quirúrgico según la reivindicación 1 ó 2, en el que el mecanismo de actuación 25 incluye un fluido (58) para contener y liberar la energía almacenada en el mecanismo de almacenamiento de energía.
- 4. El instrumento quirúrgico según la reivindicación 3, en el que el mecanismo de actuación incluye una válvula (72) para controlar el flujo del fluido.30
- 5. El instrumento quirúrgico según la reivindicación 4, en el que el mecanismo de actuación incluye una válvula de derivación (78) para permitir el movimiento del fluido cuando la válvula está cerrada.
- 6. El instrumento quirúrgico según la reivindicación 1 ó 2, en el que el mecanismo de actuación incluye un sistema de freno (254) operable sobre el mecanismo de almacenamiento de energía para 35 refrenar y liberar la energía almacenada en el mecanismo de almacenamiento de energía.
- 7. El instrumento quirúrgico según la reivindicación 1, 2 ó 6, en el que el mecanismo de actuación incluye un volante (354) que puede operar sobre el mecanismo de almacenamiento de energía.
- 8. El instrumento quirúrgico según la reivindicación 3, en el que el émbolo y el resorte están montados en un cilindro estanco a los fluidos (22) previsto en el mango, y el fluido se proporciona al 40 cilindro y es movible desde un lado del émbolo al otro lado del émbolo dentro del cilindro, en respuesta a la operación del mecanismo de actuación.
- 9. El instrumento quirúrgico según una cualquiera de las reivindicaciones precedentes, en el que el mecanismo de actuación refrena la energía contenida en el mecanismo de almacenamiento.
- 10. El instrumento quirúrgico según una cualquiera de las reivindicaciones precedentes, en el que 45 el mango tiene una forma de empuñadura de pistola.
