Abstract

Die Erfindung betrifft ein Verfahren und Vorrichtung zum Ermitteln der Endpositionen beim Implantieren von Endoprothesen, insbesondere auch beim Austauschen von Teilen eines Hüftendoprothesen-Systems bei Hüftgelenkrevisionen, wobei ein Probeadapter mit exzentrischem Innenkonus und am Umfang vorhandener Gradskala auf den Konusschaft der eingesetzten Endoprothese gebracht wird, auf den Probeadapter eine entsprechende Probekugel gesetzt wird, die Kugel mit der Pfanne in Eingriff gebracht wird, der Operateur die Endposition durch Drehen des Probeadapters die Einstellung des CCD-Winkels und des Halslängenausgleiches ermittelt, diese Position des Probeadapters durch Übertragen einer Markierung von der Gradskala auf den Schaft der Endoprothese durchführt, Probeadapter mit Probekugel entfernt, die zum Verbleib bestimmten Kugel und Adapter mit gleicher Gradskala am Umfang so einsetzt, daß eine Übereinstimmung einer bestimmten Markierung des Adapters mit der Markierung auf dem Schaft gegeben ist.

Description

Die Erfindung betrifft ein Verfahren und Vorrichtung zum Ermitteln der Endpositionen beim Implantieren von Endoprothesen, insbesondere auch beim Austauschen von Teilen eines Hüftendoprothesen-System bei Hüftgelenkrevisionen, wobei ein Probeadapter mit exzentrischem Innenkonus und am Umfang vorhandener Gradskala auf den Konusschaftes der eingesetzten Endoprothese gebracht wird, auf den Probeadapter eine entsprechende Probekugel gesetzt wird, die Kugel mit der Pfanne in Eingriff gebracht wird, der Operateur die Endposition durch Drehen des Probeadapters die Einstellung des CCD-Winkels und des Halslängenausgleiches ermittelt, diese Position des Probeadapters durch Übertragen einer Markierung von der Gradskala auf den Schaft der Endoprothese durchführt, Probeadapter mit Probekugel entfernt, die zum Verbleib bestimmten Kugel und Adapter mit gleicher Gradskala am Umfang so einsetzt, das eine Übereinstimmung einer bestimmten Markierung des Adapters mit der Markierung auf dem Schaft gegeben ist.
Bei Hüftgelenkrevisionen wird insbesondere der Austausch der vorhanden Erstversorgung notwendig, wobei der Steckkopf oder Kugel vom Metallkonus eines implantierten Schaftes einer Endoprothese abgezogen und ein anschließender Neuaufbau mit einer gleichzeitigen Korrektureinstellung des CCD-Winkels und des Halslängenausgleiches erfolgten muss. Dabei wird ein Adapter mit exzentrischem Innenkonus auf den Metallkonus des Schaftes der verbleibenden Endoprothese gebracht und auf den Außenkonus des Adapters wird ein neuer Steckkopf, meist eine Keramikkugel, mit gleichem Innenkonus wie der Aussenkonus des Adapter ist, gesetzt. Dabei darf auf keinen Fall der Adapter mit der Kugel bereits eine feste Konusverbindung mit dem Schaft der Endoprothese eingehen. Durch drehen des Adapters ermittelt der Operateur die erforderliche und gewünschte Korrektureinstellung des CCD-Winkels. Dabei wird häufig, durch das Einbringen der Kugel in die Pfanne, bereits eine sehr intensive aber unerwünschte Verbindung der Konen untereinander festgestellt, so dass nur mit viel Aufwand und Mühe der Operateur ein Verdrehen und Ermitteln des besten CCD-Winkels bewerkstelligen kann.
Es ist ein Endoprothesen-System bekannt, bei dem der obere Rand des Schaftes der Endoprothese eine Verzahnung aufweist, über welchen ein Adapter, mit der Gelenkkugel, mit einer Innenverzahnung verbringbar ist. Wenn der Adapter nunmehr verdreht wird, so ergibt sich eine bestimmte gerastete Position von Adapter und darauf befestigtem Kugelkopf. Nachteilig ist hierbei, das der Operateur nur gegen den Widerstand und die Verzahnungshöhe den Adapter verdrehen kann, so dass er in Richtung des Halslängenausgleichs arbeiten muss. Auch eine Kennzeichnung der Stellung des Adapters zum Schaft der Endoprothese ist hier nicht vorhanden.
Durch die EP 0423 303 ist eine modulare Hüftgelenkprothese mit justierbarem Gelenkkopf bekannt, wobei zum endgültigen einzementieren des Gelenkkopfes der Femurschaft zunächst befestigt wird, dieser eine erste konische Aufnahme aufweist, in die ein Zapfen mit einer Markierung steckbar ist, der dazu dient, die spätere Stellung des Gelenkkopfes zunächst vorläufig zu ermitteln, durch eine sektorale Skala an der Halsauflage des Gelenkkopfes. Damit wird auch gleichzeitig die Stellung des Fräser ermittelt, der die Markhöhle zur Aufnahme des nach unter erweiterten Gelenkkopfes ausräumt. Eine stufenlose Ermittlung des CCD-Winkels und ein exzentrischer Adapter zur Aufnahme der Gelenk­ kugel werden aber nicht dargestellt oder sind hier einbezogen.
Die Erfindung hat sich dementsprechend die Aufgabe gestellt, ein Verfahren und entsprechende Mittel zum einfachen ermitteln der Stellung der Kugelköpfe auf dem CCD-Winkel bei Hüftgelenkoperationen anzugeben, um dem Operateur behilflich zu sein, mit Hilfe bekannter Schwenkhülsen oder exzentrischer Adapter die richtige Valgus-/Varus- oder Antetorsion-/Retro­ torsionsstellung zu finden.
Zur Lösung dieser Aufgabe wird ein Verfahren und eine Vorrichtung zum Ermitteln der Endpositionen von Kugelköpfen beim Implantieren von Endoprothesen vorgeschlagen, wobei ein Probeadapter mit exzentrischem Innenkonus und am Umfang vorhandener Gradskala auf den Konusschaft der eingesetzten Endoprothese gebracht wird, auf den Probeadapter eine entsprechende Probekugel gesetzt wird, die Kugel mit der Pfanne in Eingriff gebracht wird, der Operateur die Endposition durch Drehen des Probeadapters die Einstellung des CCD-Winkels und des Halslängenausgleiches ermittelt, diese Position des Probeadapters durch Übertragen einer Markierung von der Gradskala auf den Schaft der Endoprothese durchführt, Probeadapter mit Probekugel entfernt, die zum Verbleib bestimmten Kugel und Adapter mit gleicher Gradskala am Umfang so einsetzt, das eine Übereinstimmung einer bestimmten Markierung des Adapters mit der Markierung auf dem Schaft gegeben ist.
Dazu werden dem Operateur in der Herstellung preiswerte Probeadapter der entsprechenden Größe und ebensolche Probekugelköpfe gegeben, wobei der Probeadapter ein im unteren Bereich des Umfangs eingravierte Skala mit Zahlen Buchstaben oder anderen Darstellungen aufweist, so das der Operateur nach der Ermittlung der endgültigen Stellung des Kugelkopfes nach belieben eine Markierung des Adapters auf den Schaft der Endoprothese überträgt.
Anschließend werden Probeadapter und Probekugel entfernt und durch die zum dauerhaften Verbleib bestimmten Teile ersetzt.
Insbesondere bei Revisionen ist dieses Verfahren nunmehr sehr einfach und vorteilhaft anzuwenden.
Weitere Vorteile ergeben sich aus der nachfolgenden Beschreibung eines Ausführungsbeispiels und werden anhand der Zeichnung näher erläutert.
Es zeigt:
Fig. 1 einen Schaft einer Endoprothese mit einer perspektivischen Darstellung von Probeadapter und angedeutetem Kugelkopf,
Fig. 2 den endgültigen exzentrischen Adapter mit angedeutetem Kugelkopf.
Fig. 1 zeigt zunächst den proximalen Teil einer möglicherweiser implantierten Endoprothese 1, was nicht näher dargestellt zu werden braucht. Am Schaft 2 sitzt der Aufnahmekonus 3, bereit zur Aufnahme eines Kugelkopfes, einer Geradadapters oder eines exzentrischen Adapters. Dargestellt ist weiter ein Probeadapter 5 mit einem exzentrischen Innenkonus 6. Während der Innen­ konus 6 entlang der Achse 10, also in Richtung des CCD-Winkels verläuft, ist die eigentliche Mittelachse 11 des Probeadapters 5 in maximaler Stellung Valgus dargestellt. Auf dem unteren Rand des Probeadapters 5 ist eine Skala 7 mit Zahlen eingraviert oder anderweitig aufgebracht, vorhanden. Es können auch Buchstaben oder andere unterschiedliche Symbole verwendet werden.
Auf dem Probeadapter 5 ist ein Probekugelkopf 20 angedeutet strichpunktiert gezeichnet.
Der Operateur hat beispielsweise bei einer Revision einer bereits vorhandenen Hüftgelenkendoprothese den bisherige Kugelkopf entfernt und hat festgestellt, dass er sowohl die Stellung des Kugelkopfes im CCD-Winkel als auch einen größeren Abstand, also einen Halslängenausgleich vornehmen muss. So bestimmt er einen Adapter mit exzentrischem Innenkonus und einer bestimmten Länge für den Halslängenausgleich. Um nun die endgültige Stellung des Kugelkopfes zu bestimmen nimmt er zu dem gewählten Adapter einer bestimmten Grösse und Kennung den entsprechenden Probeadapter 5 mit Probekugelkopf 20 und setzt diese auf den Konus 3. Nach dem verbinden mit der Pfanne kann der Operateur den Probeadapter soweit verdrehen, bis er gewünschte und erforderliche Stellung des Kugelkopfes feststellt. In dieser Stellung wird auf dem Schaft 2 eine Markierung 8 zu einer bestimmten Zahl, beispielsweise der Zahl 12 auf dem Probeadapter vorgenommen, falls die vorhandene eingesetzte Endoprothese 1 noch keine feste Markierung aufweist. Es liegt jedenfalls im Rahmen dieser Erfindung, dass auf einer Endoprothese unterhalb des Konus 3 zwei gegenüberliegende Markierungen 8 vorhanden sind. Diese lassen sich durch Ätzen oder Farbauftrag auf den Hals direkt bei der Fertigung der Endoprothesen auf oder anbringen.
Anschließend werden Probeadapter 5 und Kugelkopf 20 entfernt und der endgültige Adapter 14, gemäß Fig. 2, wird zusammen mit dem zu implantierenden Kugelkopf 30 auf den Konus 3 gesetzt, wobei jetzt die in völliger Übereinstimmung mit der Skala des Probeadapters identische Zahl "12" nunmehr auf der Skala 15 des Adapters 14 mit der Markierung 8 auf dem Schaft 2 zur Deckung gebracht wird.
Es stimmen also die beiden Innenkonen 6 der Probeadapter 5 und des zu implantierenden Adapters 14 mit den jeweiligen Skalen 7 und 15 überein.
Bei dem zu implantierenden Adapter 14, meist aus einem Implantatwerkstoff wie Titan gefertigt, ist die Skala 15 an der Stirnseite um den Eingang des Innenkonus 6 angebracht um die gesetzte Markierung 8 exakter treffen zu können.
Die Skalen 7 und 15 sind hier vorzugsweise wie eine Uhr mit Zahlen von 1 bis 12 versehen, wobei eine feinere Unterteilung ebenso möglich ist. Der Probeadapter 5 und der Probekugelkopf 20 sind vorzugsweise aus einem klinisch unbedenklichen Material, beispielsweise Kunststoff, hergestellt und in einer Warnfarbe, beispielsweise Rot gehalten. Außerdem sind am Umfang des Probeadapters 5 und auf dem Probekugelkopf 20 noch Warnhinweise 9 mehrmals am Umfang, z. B. "NO IMPLANT" vorhanden.

Claims (10)

1. Verfahren zum Ermitteln der Endpositionen beim Implantieren oder Revisionen von Endoprothesen, dadurch gekennzeichnet, dass eine Probeeinheit (40) bestehend aus Probeadapter (5) mit vorhandenen Markierungen (7) und Probesteckkugel (20) auf den Konusschaft (3) der eingesetzten Endoprothese (1) gebracht wird, der Operateur die Endposition der Probeeinheit (40) durch Drehen des Probeadapter (5) die erforderliche Position der Probeeinheit (40) zur Einstellung des CCD- Winkels ermittelt, diese Position der Probeeinheit (40) durch Übertragen einer der Markierungen der Gradskala (7) als Markierung (8) auf dem Schaft (2) der Endoprothese (1) durchführt, die Probeeinheit (40) entfernt, die zum Verbleib bestimmten Steckkugel (30) und Adapter (14) mit gleicher Gradskala (15) am Umfang so einsetzt, das eine Übereinstimmung mit der vorher ermittelten Markierung auf dem Adapter (7) mit der Markierung (8) auf dem Schaft (2) gegeben ist.
2. Vorrichtung zum Ermitteln der Endpositionen beim Implantieren oder Revisionen von Endoprothesen, dadurch gekennzeichnet, dass Adapter (14), Probeadapter (5) und die Endoprothese (1) Markierungen (7, 8, 15) tragen.
3. Vorrichtung nach Anspruch 2, dadurch gekennzeichnet, dass der Adapter (14) und der Probeadapter (5) identische Markierungen (7, 15) tragen, wobei die Markierungen (7, 15) immer eine bestimmte Position zur Stellung des Innenkonus (6) einnehmen.
4. Vorrichtung nach Anspruch 2, dadurch gekennzeichnet, dass die Markierungen (7, 15) am Adapter (14) und Probeadapter (5) Skalen mit Strichen und Zahlen sind.
5. Vorrichtung nach Anspruch 2, dadurch gekennzeichnet, dass die Skalen (7, 15) aus Strichen mit Buchstaben oder andere Symbole bestehen.
6. Vorrichtung nach Anspruch 2, dadurch gekennzeichnet, dass der Adapter (14) eine Skala (15) an seiner Stirn- oder Unterseite trägt.
7. Vorrichtung nach Anspruch 2, dadurch gekennzeichnet, dass der Probeadapter (5) eine Skala (7) am Umfang des Aussenkonus trägt.
8. Vorrichtung nach Anspruch 2, dadurch gekennzeichnet, dass die Probeeinheit (40) bestehend aus Probeadapter (5) und Probesteckkugel (20) aus Kunststoff in einer Warnfarbe gefertigt sind.
9. Vorrichtung nach Anspruch 2, dadurch gekennzeichnet, dass auf dem Probeadapter (5) und Probesteckkugel (20) Warnhinweise (9) angebracht sind.
10. Vorrichtung nach Anspruch 2, dadurch gekennzeichnet, dass die Endoprothese (1) mindestens zwei gegenüberliegende Markierungen (8) trägt.
