First, the basics: these aligners are manufactured in Lahore, Pakistan, then shipped to customers in the US. The company keeps a small office in Hicksville, New York, but that's not where anything actually happens — it's essentially just a US-facing address to make the company look local. All real operations, manufacturing, and decision-making happen out of Pakistan. From what I saw, there's no real oversight or reporting on sanitation and safety standards during production.
The part that bothered me most was the "free" video consultations. These are marketed like a real dental screening, but in practice, nearly everyone who does one is told they're a good candidate for treatment — regardless of what their teeth actually look like. I watched people get approved for aligners who had no business getting them. The consultation isn't there to help you; it's there to close a sale.
The agents answering emails and calls — the people customers are trusting for guidance about their teeth — don't have dental training. They also work under pseudo names, not their real identities.
If treatment doesn't work, don't expect a refund. And if a customer posts a negative review on Trustpilot, I saw the company respond by offering discounts or payouts specifically to get that review taken down — not to fix the actual problem, just to protect the rating.
This isn't limited to one brand, either. The same operation runs Caspersmile in the UK, Aligner32 for the US and international customers, and SmilePath in Australia — all owned and operated by Digital Billing Services in Pakistan.
I used to work at Digital Billing Services, the Pakistan-based company behind these aligner brands. I'm writing this because I saw firsthand how customers were treated, and I don't think people deserve to be misled the way they are.
I'm not saying this to be dramatic. I'm saying it because I was inside the company and saw how the sales process worked, and I don't think customers are getting the full picture before they hand over their money and their smile.
My advice: don't trust the video consultation, don't expect a refund, and think twice before ordering from any of these brands.

As a general rule, I do not write negative reviews. But my experience with AlignerCo has been an absolute nightmare from start to finish- and I’m not even halfway through my “treatment plan”.
I ordered my kit over 6 months ago, and I have only progressed to Step 5. Each set of aligners is to be worn for 10 days. Allow me to show you the math- 10 days x 10 steps is 100 days or about 3.5 months. This delay alone should be a huge red flag.
Additionally, the other day I was contacted about my “ill-fitting” aligners and asked to pay a small shipping fee (only $100!) to have an impression kit sent to me so I can RESTART. Despite the fact that I have been documenting and attempting communication at every step, I was only informed of the poor fit just yesterday (6 months in). And ONLY this after I contacted the company multiple times to send me my next set of aligners. What is odd is that I wore my 1st set for over a month waiting for the fit to be approved for steps 2-5. They were only fabricated after the fit was assessed and APPROVED. And now suddenly the fit is off? So weird..
The whole thing is an absolute scam. Take your money elsewhere.

I ordered a night guard (I have been using night guards for years for bruxism) and the impression process went smoothly. However, when my night guard arrived it is so tight that it was stuck in my mouth for nearly 30 minutes while I tried everything I could think of to get it out. I was genuinely concerned I was going to damage my teeth/gums. Upon contacting customer service they told me that it is supposed to fit snugly (I have never had an oral appliance fit this tight before). Though they mention a satisfaction guarentee on the website no refund was determined to be possible so now I have an unwearable appliance.

I wanted night time aligners for some small adjustments. Had braces as a kid and retainer afterwards but didn’t wear the retainer in my 20s. Now about 40 and teeth aren’t bad but don’t want them to get worse. I ordered and they sent the impression kit. I did the impressions (not easy btw) and sent them in. Weeks later and no word. Post office says delivered but Alignerco says post office may have lost it. Sales person calls me and tells me that not only did they find my impressions but they were approved and good to go. Let’s buy today! I agreed. No news on when the aligners were coming. I keep checking. I find out that the sales person lied to me and they did not have the aligners. Still lost. I ask them to send me another kit even though I was mad about the sales person lying. Another week goes by and nothing. I ask for a tracking number. They tell me that they found the impressions but they aren’t approved. The other ones are on the way right?? Nope. We will send the new kit. After they said they sent it about 10 days before. Now we are nearly 8 weeks into the process of getting the impression kit to even start the process to get alingers. I ask for the second time to please cancel my order because I’ve had enough. The request goes without response. Finally I had to dispute the transaction for the aligners. I had paid for them but never received them. Transaction disputed. Awful customer service and sales people that lie to you. I don’t have experience with any other companies for aligners but please don’t use this one. 9 weeks I went back and forth and they even had my money for about 5 weeks and I had no aligners.

Very poorly run company and absolutely terrible customer service! I'm beginning to wonder if it's a SCAM. Don't waste your money or time with this company, just spend a few dollars more and work with a local orthodontist. It's been 10 weeks since we placed an order for our son's aligners, and 6 weeks since the company has been in receipt of his molds we sent back, and we're still receiving emails stating "Once your fabrication request has been sent to the lab your aligners will take approximately 3 weeks to fabricate". At this rate it will be 13-14 weeks from the time we placed the initial order to "MAYBE" receiving an actual product. If the customer service is this awful, what confidence can a customer have in the actual product being any better?
Communication is seriously lacking. I originally used this company to fix the crowding on my bottom teeth, but unfortunately, the process ended up messing up my top teeth as well. They created a huge gap between my side teeth, and now my front teeth are uneven. Before treatment, my top teeth were at least straight across.
The final straw was when my aligners got destroyed. It took them over a week just to get to my request for replacement aligners because of a backlog. In the meantime, my teeth have shifted, and I’m now left dealing with the consequences of a delay that was completely out of my control.
And don’t put too much faith in the “guarantee.” Apparently, if your retainers are considered “ill-fitting,” you can lose the guarantee to have your teeth corrected. I had no idea my retainers were considered ill-fitting because they don’t check in with you or monitor your progress.
Overall, I’m extremely disappointed with the results, the lack of communication, and the lack of follow-up throughout the process. I went into this hoping to save money, but instead I’m now unhappy with my teeth and may have to spend even more money to fix the problems that resulted from treatment.
I definitely learned my lesson: Sometimes trying to save money with a cheaper company isn’t worth it. I wish I had spent more upfront for a company that provided better communication, monitoring, and accountability.
