A fascinating Deep Dive session at the #FacultyofPharmaceuticalMedicine Annual Symposium on inclusion and women's health. We were well set up by inspiring talks from Annalisa Jenkins and Freda Lewis-Hall, as keynote speakers. Chaired artfully by Bu Siakpere - with fellow speakers Jennifer Logue, Lisa Campbell and Ngawai Moss, all bringing different and complementary perspectives. I have to admit it made me feel angry. 💠 In many large and significant trials women are under-represented, sometimes such that the percentage of women is too small for subgroup analysis - and therefore the results only really relate to men. The legacy of the male dominated Framingham study is in part responsible for the under diagnosis of myocardial injury in women. 💠 And the Thalidomide scandal set back studies in pregnancy and breast feeding for years. Even as late as 1993 women were routinely excluded from trials until the NIH Revitalisation Act mandated their participation 💠 Preclinical studies have been carried out entirely on male mice - even in obesity studies where female mice are known to respond differently to diet. 💠 80 percent of autoimmune disease patients are women - yet this is not reflected in many trials in these conditions. 💠 Stories of a lack of preconception advice for women with long-term conditions, having to navigate pregnancy and breast feeding alone - often staying off effective medicines for prolonged periods with consequences for long-term health and co-morbidities. It seems there is a lack of will to answer questions about the safety of medicines in pregnancy and breast feeding for drugs widely used. As Jennifer Logue suggested - we value endpoints in men over outcomes in women. So we ended with a call to arms for the FPM - supported by Dr Sheuli Porkess and Dr Emma Harvey . We need to educate the public and professionals to better interrogate trial results and see bias and whether conclusions can be extrapolated to women. And if not, why not. The 'Friends and Family Test' - FFT for short - has been an NHS fixture for more than a decade now and is steadily accumulating feedback at a rate of around 2,000,000 responses per month. But is it a useful way of measuring and improving people's experiences? This new report led by Picker colleagues Olli Potter & Jenny King looks at the history, use, and effectiveness of the FFT. Drawing together evidence from a literature review, stakeholder feedback, and wider learning, it shows that: ▪️The FFT has raised interest in and awareness of patient experience, 'raising the floor' by ensuring that all providers have routine access to feedback. ▪️Quantitative data from the FFT lacks comparability and shows little variation over time, producing consistently positive results that may not reflect changes in people's experiences. ▪️Qualitative feedback - written comments - are a rich and powerful source of insight - but they are not collated nationally and providers can struggle to make the most of them. Based on the findings, we advocate for reform of the FFT to improve its effectiveness and value for money - and we set out recommendations to ensure that changes can be co-produced with patients and the public and useful to NHS organisations both locally and nationally. To rethink how the system values new treatments, it isn’t enough to show that your innovation works – you need a 'why now' that compels action - payors willing to pay for the innovation, HCPs wanting to prescribe it and patients committed to taking it. For me, that starts with an actionable insight: naming the very specific moment that matters in real life – the consultation, the scan result, the prescribing decision, the conversation at home – where a shift in behaviour is both possible and would unlock the full value of your asset ie its ability to transform patient outcomes.​ An insight linked to that moment, inspires ideas and drives a sense of urgency for ‘why your innovation’ at that moment. Any ‘why now’ ideas need to be sharp, believable, and emotionally resonant and stress tested through three lenses: Would a sceptical HCP/Payor change anything because of this? Is it genuinely different to the value that is created by what they do today? And can every function see how to act on it this quarter? Teams that nail their 'why now' don’t just have a better story; they give therapies the best possible chance of reaching the right patients, in the right way, at the right time
