Definitions
- This invention relates to a device for intubating a patient .
- an arched metal laryngoscope of variable si ze , with an L- shaped cross-section and equipped with a light positioned at the handle , is introduced into the oral cavity .
- the laryngoscope Once the laryngoscope has been advanced to the correct location with a li fting and pulling motion toward the operator, it enables visualisation of the trachea and vocal cords . At this point , a ventilation tube is inserted into the laryngoscope , the tube being equipped with an anchor balloon or cuf f at its end .
- the functions o f the anchor balloon are , in addition to stably positioning the ventilation tube in the oral cavity, to prevent saliva from entering the trachea and to ensure that all air mechanical ly introduced into the ventilation tube reaches the lungs .
- the manoeuvre can be traumatic for the anatomical structures through which the laryngoscope passes , such as the tongue , palate , and teeth, as the laryngoscope is made of metallic material ;
- Figure 1 is a side view of a device for intubating a patient according to this invention.
- FIG. 2 is a cross-section view of the tube of the device in Figure 1 .
- Figure 1 shows a side view of a device 1 for intubating a patient according to this invention .
- the device 1 comprises a tube 2 , described in detail below, inside of which a reinforced cable 4 is housed .
- the reinforced cable 4 is made of semi-rigid mouldable material so as to be flexible and able to be manipulated according to desired shapes .
- This material is , preferably, a known, biologically compatible material .
- an anchoring sleeve 8 is fixed that is designed to anchor the device 1 in the oral cavity of the patient to be intubated .
- the reinforced cable 4 is provided, at said proximal end 6 of the tube 2 , with a video camera 12 designed to leave said proximal end 6 of the tube 2 , so as to acquire images of the oral cavity of the patient following the insertion of the device 1 of the oral cavity itsel f .
- the device 1 also comprises , near one distal end 14 of the tube 2 , opposite the proximal end 6 , a valve 16 for controlling a flow of air in the anchoring sleeve 8 , and a valve 18 for controlling a flow of air inside the wall of the tube 2 , as described in detail below .
- the valve 16 is connected to the anchoring sleeve 8 via a f irst connecting tube 16a ; the valve
- Figure 2 shows a cross-section view of the tube 2 .
- the tube 2 has an external wall 20 defining a through hole 22 .
- the reinforced cable 4 and at least one optical fibre 24 for lighting the through hole 22 are inserted.
- the external wall 20 is a hollow wall.
- the external wall 20 is folded like a packet in order to assume an undulating shape forming a number of pockets 26 arranged side-by-side, i.e. a sequence of peaks and valleys.
- the ends radially internal to the pockets 26 lie on the same first circumference 26a that delimits the through hole 22, while the ends radially external to the pockets 26 (i.e. the peaks) lie on the same second circumference 26b concentric and external to said first circumference 26a, shown with the dashed line in Figure 2.
- the external wall 20 is made of thermoplastic material known in and of itself, which is rubbery, semi-rigid, and absolutely biologically tolerable .
- This material has thermoplastic properties at temperatures above 35 ° C so that , once inserted in the oral cavity of the patient , may be modelled according to the natural curves of the oral cavity itsel f .
- this material preferably has an elastic hysteresis absent at temperatures above 37 ° C, so that the tube 2 does not return to its original shape after having been deformed and having, thus , assumed a di f ferent shape to the original one .
- a connector 30 at the distal end 14 o f the tube 2 for connecting the tube 2 to a mechanical ventilator of a known type .
- the reinforced cable 4 is designed to be connected to a remote control device 32 that , preferably, comprises a display screen 34 designed to show the advancing of the tube ( and of the reinforced cable 4 arranged to move integrally with the tube 2 ) in the oral cavity of the patient .
- the remote control device 32 is advantageously provided with an adj usting member 36 for adj usting the intensity of the light of the optical fibre 22 (which is also connected to the remote control device 32) , an adjusting member 38 for adjusting the optical focus of the video camera 12, and an adjusting member 40 for adjusting the magnification of the displayed image.
- Said adjusting members 36, 38, and 40 are, for example, shaped like knobs.
- the operation of the device 1 is as follows.
- the tube 2 is inserted in the oral cavity, guided using the optical video camera 12.
- the device is inserted by intubation 1 (and, thus, the tube 2) , in the Guedel tube.
- the tube 2 is anchored via the anchoring sleeve 8, blowing air into the anchoring sleeve 8 through the valve 16.
- the reinforced cable 4 is removed from the tube 2 and the tube 2 is expanded by blowing air inside the external wall 20, which expands, thus increasing the final diameter of the tube 2 inside the oral cavity.
- the tube 2 will no longer have the external wall 20 folded as described above but is completely expanded, and the diameter of the through hole 22 will be greater than it was before the air is introduced .
- an intubation device it i s possible to advance a guidable tube that has a smaller diameter than the prior art laryngoscope and the ventilation tube in the oral cavity .
- the advancing of the device for intubation is less traumatic for the oral cavity .
- the visual control of the intubation operation is continuous , thanks to the presence of the optical fibre 24 , and it is also possible to control it remotely . Finally, thanks to the fact that the tube is guidable , it is not necessary to hyperextend the head of the patient to carry out its intubation .
- the removal of the reinforced cable 4 occurs when the tube 2 is already in place and anchored, so in the correct positioning .
