A segunda camada (4) aplica-se sobre a primeira (6) como uma tampa (4) e suporta a linha (5) de distribuição de pressão negativa. A primeira camada (6) de espuma de poliuretano deverá ter sob toda a sua extensão inferior uma tela (7) de polipropileno ou material similar, vaselinada, para proteção da pele (9) ao redor da ferida (8). Como opção, pode-se também colocar uma tela vaselinada sob a segunda camada de espuma e para a proteção do enxerto de gordura (3). Ao final, sobre todas as camadas (4, 6) de espumas de poliuretano aplica-se um plástico filme adesivo (2) para vedação do ambiente ao redor da ferida (8), para que se evitem vazamentos e consiga-se a manutenção da aplicação de TFPN de maneira constante sobre a ferida. A espuma com cavidade central poderá permanecer por até sete dias corridos sobre a ferida.
