Description

The present utility model is related to the medical field, more specifically it treats the present model of an applicator of a device for dilatation of the endocervical canal (internal canal of the cervix).

[002] Stretching, that is, narrowing of the cervical canal (endocervical) is the most important complication after cervical surgery. The clinical repercussions may vary from menstrual cramps, followed by hematometrial (blood accumulation inside the uterus), infertility and difficulty in the early detection of recurrence and / or recurrence of pre-mensign lesion. Studies show the stenosis index of the cervical canal varying from 1, 3 to 19.0% when performed with high frequency loop surgery.

[004] O procedimento cirúrgico com aplicação do presente dispositivo dilatador evita então a retenção de sangue no interior do útero durante o período menstruai, mitigando às cólicas, que se acentuam quando o canai endocervical permanece fechado, [003] Mainly because of these post-surgical complications, an applicator was developed to assist in the introduction of a medical device for endocervical canal dilation. The applicator takes the more agile surgical procedure, reducing the operative time, thus remaining an anesthetized patient for a shorter period. The surgical procedure with application of the present dilator device then avoids the retention of blood within the uterus during the menstrual period, mitigating colic, which is accentuated when the endocervical cannula remains closed,

[005] Another advantage of the application of the device is to avoid the patient's infertility, since the closure of the endocervical canal contributes to infertility.

[008] Another advantage is visualization of the inner part of the endocervical canal or squamous cell junction (JEC) in all women (in the fertile period or not) during a follow-up examination, called coiposcopy. When the scam-cell junction (JEC) is not visualized, the examination is incomplete. This examination is performed on the patient's follow-up for the early detection of recurrence or recurrence of cancer or pre-invasive cervical lesion.

A introdução do dispositivo dilatador ocorre peio canal vaginal até o correto posicionamento no colo do útero peio orifício externo adentrando no canai endocervical Thus, the application of the dilator device is indicated in patients with pre-invasive cervical lesions who will undergo conization (CAF) or patients with initial cervical tumors (less than 2 cm) with indication of surgery fertility preserver also called radical trachelectomy In order to facilitate the introduction of the device for dilatation of the endocervical canal, the present applicator device has been developed which facilitates correct positioning of the device during the surgical procedure thus avoiding false paths of the device (called a fistula). The introduction of the dilator device occurs through the vaginal canal until the correct positioning in the cervix of the uterus by the external orifice entering the endocervical canal

O documento americano US5582594 mostra um desses apiicadores que possui uma curvatura para facilitar a penetração do aplicador no canal endocervical, Various techniques for treating cervical diseases, such as radiation source primers for the treatment of cancer by brachytherapy, are found in the state of the art. US5582594 shows one such implement which has a curvature to facilitate penetration of the applicator into the endocervical canal,

[010] The applicator of the prior art is provided with a fiange which has the function of marking the position of the cervical region and, as such. prevent perforation of the uterine wall from the tip of the applicator.

O aplicador objeto do presente modelo de utilidade não tem objetivo de introduzir fontes de radiação para o tratamento do câncer e sim posicionar durante o procedimento cirúrgico um dispositivo que procura evitar o fechamento do canal endocervícai, US4244357 also discloses an apparatus for treating cervical cancer for the introduction of a linear source in order to achieve homogeneity in the radiation dose. The applicator object of the present utility model has no objective of introducing radiation sources for the treatment of cancer and yes to position during the surgical procedure a device that seeks to avoid the closure of the endocervical canal,

[012] O presente modelo de utilidade possui três porções; distai, medial e próxima! . [012] The present utility model has three portions; distal, medial and next! .

The medial portion of the applicator is provided with a recess which functions as a screen for the dilator device, said dilator being advantageously positioned in the distal region of the applicator, which facilitates the introduction of the device into the patient's vaginal canal and its correct positioning in the endocervical canal, The use of the applicator taught here makes the surgery faster and more accurate.

[014] The proximal region is provided with a cable for the surgeon to handle the applicator. This cable preferably has grooves to promote friction with the surgeon's sleeve. The cable is advantageously further provided with a chamfer which marks the correct position of curvature of the distal portion in order to guide the surgeon upon introduction of the applicator into the patient. That is, the applicator can be provided with a cable with a chamfer extending in the axial direction of the cable, the cable being further provided with a plurality of transverse grooves.

[015] Thus, the present disclosure discloses an applicator for introducing a dilator device into the endocervical canal, wherein the said dilator device comprises a disk having a center hole through which a duct extends.

The applicator is comprised of a distal portion having a bulged tip, a curved potion and a straight portion.

The dilator device is inserted into the applicator by the introduction of the tip into the lower orifice of the dilator device. The distal portion of the applicator passes through the duct until the dilator device disc encounters the recess, the dilator device positioned in the straight portion.

[018] The medial portion has a larger diameter than the distal portion and is slightly longer than the vaginal canal. The medial portion connects to a nearby portion! which serves as a handle for the surgeon's handling.

[019] As figuras são sumariamente descritas conforme mostrado a seguir: [019] The figures are briefly described as follows:

Figura 1 - mostra a vista lateral do aplicador; Figure 1 shows the side view of the applicator;

Figura 2 ~~ vista superior do aplicador; Figure 2 is a top view of the applicator;

Figura 3 - vista em perspectiva do aplicador; Figure 3 - perspective view of the applicator;

Figura 4 - perspectiva isométrica do dispositivo dilatador; Figure 4 - isometric view of the dilator device;

Figura 5 ~ perspectiva inferior do dispositivo dilatador; Figura 6 - vista frontal do dispositivo dilatador; Figura 7 - vista inferior do dispositivo diiatador; Figura 8™ vista superior do dispositivo dilatador; Figure 5 is a bottom view of the dilator device; Figure 6 - front view of the dilating device; Figure 7 - lower view of the dialing device; Figure 8 is a top view of the dilator device;

Figura 9 - mostra a vista lateral do dispositivo diiatador inserido no aplicador; Figure 9 shows the side view of the dial device inserted in the applicator;

Figura 10 - mostra a vista em perspectiva do dispositivo dilatador inserido no apiicador. Figure 10 shows the perspective view of the dilator device inserted in the applicator.

[020] The dialing device (40) must be inserted into the applicator. The tip 11 is inserted into the bore 42 of the implement, the curvature 12, length, and duct diameter 41 being suitable for passage through the distal portion 10 easily. The distal portion 10, being thinner and produced from plastic material, has some flexibility which favors the introduction of the device 40. Such flexibility is also necessary to favor introduction of the distal portion (10) into the narrowed endocervical canal.

O disco (43) do dispositivo (40) dilatador possui ainda uma pluralidade de orifícios (44) para a sutura, The dilator device (40) is introduced into the distal region (10) of the applicator until the radial recess (21) is encountered. The outside of the disc (43) rests on the applicator not allowing continuity of displacement of the applicator device (40). Thus, the device (40) is positioned in the straight portion (13) for introduction into the patient. The disc (43) of the The dilator device (40) further has a plurality of sutures (44) for suturing,

The present applicator is then provided with a distal portion (10) of smaller diameter than the proximal portion (20), advantageously to form a radial undercut (21).

[023] The bulged tip (11) is first inserted into the vaginal canal of the patient until the cervical region is found. The tip (11) of the applicator is inserted into the sequence in the narrowed endocervical canal. In this way, this tip is advantageously bulged so that it can be introduced into the narrowed and narrowed cervical canal.

[024] As soon as the surgeon is able to insert the tip (11) into the endocervical canal, it continues to introduce the applicator until the end (45) of the duct (41) is introduced into the endocervical canal. By continuing to insert the applicator into the inner portion of the disc (43) you will find the ectocervix. At that time, the surgeon may simply withdraw the applicator, the device (40) being correctly positioned for completion of the surgery by means of stitch attachment of the dilator device at 4 cardinal points in the cervix (3, 9, 9, and 12 h) .

[025] The applicator is further provided with a proximal portion (30) which functions as a handle for the surgeon's handling. This proximal portion (30) is preferably larger in diameter than the portion (20) medial. The medial portion 20 has a length compatible with the vaginal canal, and its introduction helps the surgeon to orient himself to mail positioning in the dilator device 40 in most patients.

A porção (30) próxima! [026] Next portion (30)! is further provided with a cable provided with a bevel (31) which serves for the surgeon to support the finger and at the same time mark the direction of curvature of the distal portion (10), guiding the surgeon during the surgical procedure. Without the bevel (31), the surgeon would not easily know the position of the curvature (12). The chamfer (31) further promotes support to prevent rotation of the applicator. The next portion (30)! is further provided with a plurality of preferably transverse grooves (32) which favor friction between the distal portion and the hand of the surgeon further promoting firmness.
