Definitions

  • the invention consists of a thoracoscopic ringed surgical clamp of small dimensions (for example, 4 cm) , preferably made of disposable plastic material, which can be positioned and released within the thorax thanks to a purposely provided clamp- applicator - or with other suitable surgical clamp - and subsequently pulled to facilitate exposure of the part concerned during surgical operations of thoracoscopic pulmonary resection thanks to a metal (multipurpose) stemlike instrument of adequate length (25 cm) and small diameter (3 mm) , which, once inserted through an accessory surgical incision, captures the rear end of the clamp, shaped like a rounded hook, via a small loop made of synthetic thread set at the end of the stem.
  • a thoracoscopic ringed surgical clamp of small dimensions (for example, 4 cm) , preferably made of disposable plastic material, which can be positioned and released within the thorax thanks to a purposely provided clamp- applicator - or with other suitable surgical clamp - and subsequently pulled to facilitate exposure of the part concerned
  • the loop enables release of the end of the instrument and its subsequent extraction via the clamp-applicator.
  • the ringed mini-clamp ensures parenchymal gripping thanks to a reversible clamping mechanism with opposed teeth.
  • One of the main advantages of mini-invasive surgery of the thorax is represented by the possibility of limiting the extension of the surgical incision and the parietal trauma. This is normally achieved with three incisions of 15-20 mm through which the optics and the various dedicated instruments that enable execution of the surgical intervention are inserted.
  • Recently, in an attempt to minimize further the surgical insult there have been developed surgical thoracoscopic techniques with two accesses or even with a single access. Approaches of this sort entail simultaneous passage of a number of instruments through a single access, which renders the manoeuvres ergonomically disadvantageous and more problematical .
  • the ringed mini-clamp according to the present invention which can be released within the thorax but can be manoeuvred from outside, enables optimal anatomical exposure, preventing bunching of a number of instruments passing through one and the same incision, thus rendering surgical manoeuvres much simpler and faster. This moreover enables execution, through a single thoracoscopic access, of manoeuvres that could otherwise be performed only with multiple accesses .
  • the instrument is also aimed at use with robotic techniques, which can benefit a great deal from the aid of this innovative technology.
  • the ringed surgical thoracoscopic mini-clamp MP has a length of 4 cm. It is made up of a body constituted by a semi-elliptical rear segment 1 designed to ensure the elastic force necessary for opening the clamp and maintaining aligned juxtaposition of the rings 2. It terminates with an end having the shape of a double rounded hook C, the function of which is to enable engagement thereof with a manoeuvring stem S.
  • the body of the mini-clamp has at the two sides two hemispherical heads B that are to interface with the hollow hemispherical ends Cl of the clamp- applicator PA for positioning/release/removal of the mini-clamp MP itself.
  • the mechanism for clamping/release of the mini-clamp equipped with opposed self-locking teeth A.
  • a moderate pressure applied on the lateral hemispherical heads B interconnects the opposed teeth A, blocking the mini-clamp MP in the gripping position, whilst a further pressure on the lateral heads B releases the teeth A, displacing them laterally in a direction opposite to one another, thus enabling opening of the mini-clamp.
  • the aforesaid opposed teeth A are wedge- shaped to enable said lateral displacement.
  • the distal end of the mini-clamp MP is constituted by the two elastic arms that terminate with the rings 2 for parenchymal gripping, which are preferably oval and toothed.
  • the mini-clamp MP according to the invention can be positioned/released/extracted via a clamp- applicator PA having a length of 25 cm, constituted by an articulatable handle of a surgical type and two articulated curved distal jaws Bl, which terminate with hollow metal hemispheres CI.
  • This clamp- applicator PA requires just one 3-cm incision for intrathoracic introduction and release of the mini- clamp MP.
  • the ringed mini- clamp MP is closed by tightening the clamp-applicator PA, which is then extracted.
  • the mini-clamp MP can then be pulled and displaced as desired in the course of the manoeuvres via a metal stem S, preferably straight with a round cross section, having a length of 25 cm and a diameter of 3 mm, with a small hole at the distal end, where a loop L made of synthetic thread passes, which is to grip the clamp, thus rendering it manoeuvrable from outside via the stem itself.
  • the stem S is inserted through a small incision of approximately 2 mm made in the skin. Connection is made by passing the loop around the end, shaped like a rounded double hook C, of the mini-clamp MP. Engagement may be easily reversed by detaining the loop L and releasing it from the nontraumatic double hook C referred to above.
  • the mini- clamp MP according to the present invention may also be used with a surgical clamp of some other type, provided that this is suitable for the purpose.
