Definitions
- the invention relates to a polyaxial alignable stabilizing element for endoprostheses.
- the setting and the spatial stabilization of endoprostheses is done a) by pressing into a prepared implant bearing or b) by cementing in a slightly enlarged implant bed.
- sufficient implant stability primary stability
- secondary stability ossification in the implant-implant boundary area
- the simplest stabilizing elements used are self-tapping bone screws for cortex or cancellous bone, which by correspondingly shaped implant openings in the bone be screwed and secure a firm connection between bone and implant at least for a period of time.
- a second group of stabilizing elements are tabs.
- Tabs can either be part of the implant itself or can be attached to the implant. They are fixed by the described bone screws on the viable bone. The shape of these tabs must be adapted to the anatomical requirements.
- a third group of additional elements are pins which are dowel-like driven into suitable bone substance and give the implant further fit by positive engagement. This group of additional elements has been used sparingly so far because the solutions used were both difficult to handle and limited in their effect.
- All previously used cones are either a) integral part of the implant or b) rigidly connected to the implant or c) releasably connected to the implant, but afflicted with a disadvantageous whilsinvarianz after its attachment.
- German patent DE 19714050 C2 which describes a hip socket in which the socket body carries at least one stabilizing pin which can be driven into the acetabulum. To improve the seat of the stabilizing pin in the acetabulum, this has a conically widening to the cup body outer contour. At the stabilizing pin a sawtooth-shaped profile is further formed, the steep flanks facing the socket body.
- German Patent DE 10113614 B4 is an elongated
- Fixation element described which can be coupled to the socket in the range of spherically bordered holes.
- the element is fixed by a screw with polyaxial head, which is set from the inside through the relevant hole in the socket.
- the surgeon places one or more fixation elements in appropriately prepared cutouts.
- the cup body is then placed in the acetabulum and screwed with threaded bolts so that a permanent connection between the socket and fixation element is made.
- the main disadvantage is that the fixation elements must be introduced before setting the implant socket in the pelvic bone and the pan must then be used accurately to the screw holes. This results in very great principle difficulties in setting the implant and also a correction of the socket seat after setting is not really possible without restricting the fixation element in its effect.
- the described angle variability of the element is limited because the available spherical pressing surfaces must not affect the contour of the implant socket.
- Stabilizing element for endoprostheses which is suitable for a necessary subsequent alignment of an implant.
- the structure consists of a hemispherical guide tip, a guide portion, which adjoins the guide tip, consists of driving surfaces, which are designed conically polyhedral, a subsequent osteointegrative region, which has a functionally modified surface, a final expansion region, which dowel similar constructed with spherical surfaces is and an area with partial spherical surface in the expansion area.
- the object of the invention is to develop a polyaxial alignable, pin-shaped stabilizing element for endoprostheses, which is preferably mounted after the optimal positioning of the implant and which is characterized by an adapted form and functional functional areas.
- the polyaxial alignable stabilizing element with a
- Guide tip, a guide portion and an osteointegrative region according to the invention has a final attachment portion which is spherical shell-shaped provided with spherical faces, wherein the polyaxial orientation of the stabilizing pin is made possible by a spherical area in the mounting area, and a recessed thread for retraction of the stabilizing pin is used.
- the spherical faces of the stabilizing pin are in
- connection area of the implant by a set nut, which has a spherical surface with a conical transition region to the thread, with the spherical surface of the journal bearing on the own spherical inner surface positively pressed.
- the surface of the osteointegrative region is greatly enlarged by coating with titanium powder and is coated with a biomimetic, electrodeposited calcium phosphate layer (for example BONIT®).
- a biomimetic, electrodeposited calcium phosphate layer for example BONIT®
- One or more stabilization pegs of various sizes may be placed during an operation.
- the insertion of the pins is done by pressing in undersized
- Figure 4 stabilizing pin with setting screw with polar angle WP positioned in the pin receptacle.
- stabilizing pin 1 are used to increase the primary stability.
- an inventive, polyaxial alignable stabilizing element is shown. It consists of a stabilizing pin 1, which is divided into different functional areas. Furthermore, the journal bearing 3, which is located in a pan base body, and the setting nut 2, which serves to set the stabilizing pin 1, is shown.
- the polyaxial alignable peg-shaped stabilizing element for endoprostheses has several functional areas.
- the first functional area is a hemispherical area and serves as Guide tip 1a for guiding the stabilizing pin 1 during the depression in the undersized hole in the appropriate bone area.
- This is followed by a guide area, which consists of drive surfaces 1 b, the conical polyhedral (eg four or star surfaces) are executed and serve to prevent rotation and compression of spongious bone during insertion of the pin.
- An osteointegrative region 1c (holding region) ensures the mechanical stability of the stabilizing pin 1 in the bone by the preferred bonding with the bone.
- a structured surface which is osteoinductively coated in a further step.
- the structuring is carried out indirectly by roughening by means of abrasive particle beam and subsequent surface enlargement by applying titanium particles in a plasma-assisted coating process.
- the osteoinductive coating is accomplished by electrochemical deposition of a biomimetic calcium phosphate layer (e.g., BONIT®).
- the journal bearing 3 is surrounded by a spherical surface 3b with a free
- Opening angle 3d determined.
- the thread 3b is inserted for the setting nut 2.
- Stabilizing pin 1 positively inserted with the ball part 3b of the implant in the implant and screwed with the setting nut 2 with positive engagement with the spherical pin inner surface 1f, see. Fig. 2, Fig. 3.
- the polyaxial orientation of the stabilizing pin 1 is possible by sliding in the mating on the spherical surface 4 with dissolved setting nut 2, the necessary clearance angle FW is shown in Fig. 2. According to the operational circumstances can
- Stabilizing pin 1 can be used in different sizes.
- the simple preparation of the seat of the stabilizing pin 1 is carried out by a directional bore with a reduced diameter by means of guide sleeves through the implant body.
- the stabilization pins 1 are pressed into the bone by the journal bearing 3 in the implant base body and firmly screwed into the journal bearings 3 of the implant.
- Their direction is polyaxially adjustable in a predetermined range. The blind-setting of the stabilizing pin 1 in the direction of drilling is possible.
- FIG. 4 shows the possible polyaxial orientation with a polar angle WP due to the partial spherical surface 1f.
- the stabilizing pin 1 is made of titanium material in the required grade.
