Definitions
- the invention relates to a method and an arrangement for the optical stimulation of the human eye by means of the projection of light patterns onto the inside of a perimeter hemisphere.
- Optical stimulation systems which use a spatially fixed coordinate system are known from the prior art. It is assumed that the viewer is looking at a defined and clearly visible point in this coordinate system. Under this condition, light stimuli are applied to a flat or spherical surface in a predefined spatial relationship to the coordinate system, i.e. with existing fixation to the direction of view, projected or activated in the case of active light sources embedded in the surface.
- a disadvantage of the previous methods is that the defined spatial assignment of the light stimuli to the retinal projection is no longer guaranteed when leaving the tolerance range of the viewing direction from the predetermined position. As a result, the measurement data cannot be used and the examination is delayed.
- the reasons for leaving the tolerance range of the required line of sight can be subjective or objective in nature.
- a typical subjective reason is the simulation of a disease or damage by the examined person. Aggravation is less common - the patient tries to hide damage to the visual field.
- the object of the invention is therefore to provide a method and an arrangement with which it is possible to follow the current gaze direction of the examined person with the coordinates of the optical system in such a way that the intended retinal areas can be optically stimulated independently of the eye movement.
- the light patterns are imaged on the perimeter hemisphere with the aid of a light source, a diaphragm arrangement, a lens arrangement and a light distributor, and the position of the light pattern the perimeter hemisphere follows the direction of the eye.
- the light patterns are generated in a computer-controlled manner and their brightness, color, geometry and also can be changed over time.
- the coordinate system of the perimeter optics is constantly tracked to the current viewing direction of the viewer, so that the intended areas on the retina can always be optically stimulated regardless of his or her will. NEN. In this way, an interruption of the perimetric examination due to incorrect fixation is no longer necessary, as with conventional methods.
- the invention further relates to an arrangement for stimulating the human eye by projecting light patterns onto the inside of a perimeter hemisphere, in which means are provided for determining the direction of view of the examined eye during the projection of the light patterns onto the perimeter hemisphere.
- An infrared light source and an infrared camera with image evaluation are preferably provided as means for determining the viewing direction.
- the light distributor can be designed as a multi-mirror device or as a D-ILA arrangement or as a rotation scanner or as a galvanomirror scanner or in a combination of these principles.
- a flat or curved surface is positioned in the field of view of the viewer, in which optical stimuli are produced at predetermined locations with suitable light sources in such a way that they meet the intended areas in their optical projection on the retina.
- the eye movements are recorded with a camera and the current viewing direction is determined from the image information. Based on the current viewing direction, the optical stimulus points on the projection surface are then adjusted so that the projection location on the retina does not change.
- the advantage here is that due to the constant monitoring of the viewing direction and the tracking of the optical system to the change in viewing direction, the desired retinal areas are always stimulated even when the eye moves. Even with cooperative patients who are willing to maintain gaze fixation, but who cannot always do this for physiological reasons, this is an advantage in terms of measurement methods, particularly with regard to the reliability and objectivity of the method. The most important contribution is to be seen in the fact that functional diagnosis of the visual field is still possible in the case of objectively impossible or subjectively unwanted cooperation of the examined person. All previous methods of topological functional diagnostics, which start from the fixed eye, fail in this point.
- the patient sits in front of a perimeter hemisphere 1, into which a stimulus pattern generated by the computer 6 is projected by an illumination system 2.
- the illuminating pattern causes typical stimulus responses in the patient's EEG.
- the EEG is detected from the patient's head surface with the aid of an EEG electrode system 3, amplified in an amplifier 4 and digitized with an analog-to-digital converter 5.
- the EEG is analyzed and evaluated in digital form in computer 6.
- the examined person must look at a fixation point BR so that the light stimuli emanating from the stimulation points SP and SP hit the planned area on the retina. If the gaze on BR is not fixed and the examined person changes the gaze direction, the stimulation points SP ] and SP 2 of the current gaze direction (fixation point BR 2 ) must be tracked to grant the desired stimulation parameters.
- the eye is illuminated with an infrared light source 7 so that its image recorded with an infrared camera 8 is sufficiently rich in contrast for evaluation by means of image processing.
- an image digitizer 9 After digitization in an image digitizer 9, the image is evaluated in the computer 6 and the current viewing direction is determined.
- the fixation point is then moved from the original (BR) to a new position (BR) and the stimulation points SP, and SP are spatially shifted accordingly (SP / and SP 2 ' ). In this way it can be ensured that the planned retinal areas are always stimulated even with eye movements.
