Abstract
Description
Implantationsvorrichtung für selbst-expandierende Stents Implantation device for self-expanding stents
Beschreibungdescription
Die Erfindung betrifft eine Implantationsvorrichtung für selbst-expandierende Stents gemäß Anspruch 1.The invention relates to an implantation device for self-expanding stents according to claim 1.
Different forms of stents are known from the prior art. These form a vascular prosthesis, which consists of body-compatible material. Stents are generally used to expand hollow vessels such as blood vessels or body openings and to keep them in an expanded state. For this purpose, the stent is normally positioned in a narrowed hollow vessel in the patient's body in an unexpanded state and subsequently expanded. The stent body usually consists of a web structure, the web structure having a plurality of adjacent cells, each delimited by webs. When expanding, the individual web areas of the stent are deformed so that it remains permanently in the expanded form.
There are designs for expanding the stent in which the stent is previously applied to an expandable balloon catheter and is transferred to its implantation position in the body opening by expanding the balloon at the site of the implantation. An alternative is self-expanding stents that can be implanted without the need for an expandable balloon catheter. Due to their design and material, these stents are able to expand on their own at the site of implantation. Special implantation devices are required for this.
It is therefore an object of the present invention to provide an implantation device for self-expanding stents which is simple in construction and easy to use.
This object is achieved by the features of claim 1.
By providing a cover tube, it is possible to transfer the stent applied to a catheter from the non-expanded position, which makes it possible to insert it into the body opening, simply by pulling back the cover tube into the expanded position, since when the cover device is withdrawn, the stent due to its the expansion forces which make self-expansion possible assume its implantation position in the body opening without the application of further forces.
Die Unteransprüche haben vorteilhafte Weiterbildungen der Erfindung zum Inhalt. The dependent claims contain advantageous developments of the invention.
By providing an actuating lever and a cable pull that is operatively connected to it and is attached to the cover tube, a translation of the trigger path and thus a reduction in the force to be applied is made possible. For this purpose, the cable is attached to the lever and the cable or the cable and the cover tube attached to it are retracted by a tilting or swiveling movement of the actuating lever. The gear ratio, the force and the distance to be traveled can vary due to the lever length become. Furthermore, a controlled withdrawal of the cover tube is made possible by the tilting movement or the pivoting movement of the actuating lever.
Further details, features and advantages of the invention result from the following description of an exemplary embodiment with reference to the drawing.
The only figure in the drawing shows a schematically simplified illustration of an implantation device according to the invention.
The implantation device 1 for a self-expanding stent 2 shown in the drawing is shown in a state in which the stent 2 is placed on a catheter 3. A cover tube 4 is arranged above the stent, which surrounds the stent over its entire length hm and thus also surrounds the part of the catheter 3 on which the stent 2 is arranged. The figure shows that this position is close to the catheter tip 9.
The figure also illustrates the provision of an actuating device 5 which is operatively connected to retract the cover tube 4 along the catheter axis m symbolized by the arrow A.
In the particularly preferred embodiment shown, the actuating device 5 has an elongated actuating lever 6. The actuating lever 6 is attached to a bearing area 10 of the catheter 3 by means of a pivot bearing 11 such that it can be pivoted or pivoted. An attachment point 13 is provided between the pivot bearing 11 and a handle part 12 of the actuating lever 6, to which one end 14 of a cable 7 is attached. The fastening point 13 can be, for example, a pin which engages the end 14 of the cable 7, which is designed in the manner of a loop, for example. from Fastening point 13 extends from the rope 7 m to a guide tube 9 which is arranged on a part of the catheter 3 lying outside the body of the patient. Within this guide tube 9, the cable 7 runs to a connecting part 8, which is also always attached to an area of the catheter 3 that lies outside the body during the implantation process. The connecting part 8 can be, for example, a ring or a sleeve, on which the free end of the cable 7 is fastened in a suitable manner.
The cover tube 4 surrounding the stent in turn extends up to the connection part 8, that is to say to an area of the catheter which lies outside the body during the implantation process.
The construction mentioned creates a connection between the actuating lever 6 of the actuating device 5 and the cover tube 4. For implantation, the self-expanding stent 2 is first applied to the catheter 3 in the vicinity of the catheter tip 9 and held in its implantation position with the aid of the cover tube 4, that is to say a position in which the insertion of the catheter with the stent attached thereon is a body opening such as a blood vessel. Once the position has been reached, such as a stenosis in a blood vessel on which the stent 2 is to be implanted, the cover tube 4 can be withdrawn away from the catheter tip 9 along the catheter axis A by means of the actuating lever 6 of the actuating device 5 remaining outside the patient's body . For this purpose, the actuating lever 6 must be tilted or pivoted around the pivot bearing 11, as is illustrated by the various positions 6 (Ri), 6 (R 2 ), 6 (R 3 ) and 6 (R 4 ) m shown in the figure in the figure is. The actuating lever 6 is thus pivoted continuously from its starting position 6 (R), which it remains during the implantation process up to the stenosis, m its end position 6 (R 4 ), and thus pulls the cover tube 4 back via the cable 7. If As a result of this actuation process, the stent 2 is released entirely from the cover tube 4, and due to the self-expansion forces, it is transferred into its implantation position, in which it can, for example, permanently widen a stenosis in a blood vessel.
