Definitions
- the invention relates to a surgical clip applicator in accordance with the preamble of Claim 1.
- Such clip applicators are used, for example, in laparoscopic surgery, but can also be used for open surgery.
- the clip applicator is introduced through a trocar sleeve, e.g. in the abdominal region.
- a conventional clip applicator With a conventional clip applicator a plurality of metal clips are fed from a clip magazine to a jaw-like head, which is situated at the end of a hollow tube.
- the applicator head is actuated by an actuating mechanism and a slide element slidable longitudinally in the tube. After introducing the foremost clip from the magazine into the jaw element and after closing of the clip, the next clip is brought into position by the action of a spring.
- the object of the invention is to create a surgical clip applicator for clips made of plastic, which is re-sterilisable.
- Claim 1 The features of Claim 1 serve to resolve this problem.
- the invention provides in a beneficial manner for the clip magazine to be interchangeably integrated in the shaft tube and to contain all the mechanical components, except for the push rod drive, for individual clip transportation to the jaw element.
- the clip applicator after taking off the clip magazine comprises only a few mechanical parts, which can be sterilised without problems and the emptied clip magazine discarded.
- After sterilisation of the clip applicator a new sterile clip magazine is installed, allowing the clip applicator to be reused again.
- the invention provides for the components unsuitable for sterilisation to be comprised in the disposable clip magazine. Because only the clip magazine has to be discarded and not the entire clip applicator, a considerable cost reduction can be accomplished when using clip applicators.
- the invention also provides, in a beneficial manner, for the push rod to push along a transportation slider for the clips, pretensioned under the tension of a spring in distal direction, during initial movement in proximal direction after actuation of the jaw element and to effect the release upon reaching a predetermined position.
- a new clip is fed after each actuation of the clip applicator from the clip magazine to the jaw element and held there in position by the transportation slider. Since the transportation slider can only be pushed along after complete closure of the jaw element, security is thus provided in ensuring that each actuating operation is completed, prior to the next clip being loaded into the jaw element with the aid of the transportation slider.
- the clip magazine a plurality of clips arranged one behind the other are guided in a clip channel.
- the clip magazine can normally hold an adequate number of clips for one operation. If, however, the number of clips is insufficient for an operation, there is the facility for removing an empty clip magazine and installing a replacement clip magazine during the operation.
- the transportation slider introduces the foremost clip situated in the clip magazine into the jaw element during movement in distal direction, in other words after disengagement of the push rod, whilst other clips situated in the clip magazine are moved forwards by one position towards the distal end of the clip magazine.
- the transportation slider can be slidably guided in the clip channel.
- the transportation slider is situated together with the clips in the clip channel in a beneficial manner, and can convey the said clips individually in the said clip channel.
- the clips can be guided in the clip channel in spaced manner.
- the clips are thus held by the transportation slider at a predetermined distance/pitch.
- the clips feature projections on both shanks (legs), directed outwards, which due to pretension of the flexible shanks of the clips, engage recesses arranged laterally in the clip channel.
- the said clips can engage with their projections in the lateral recesses after movement by one position in distal direction, whereby the clips are held stationary during the movement of the transportation slider in proximal direction and not dragged along by the transportation slider.
- the transportation slider features a flexible slide element for each clip, which deflects upon passing a clip which is proximally situated in the clip channel, when the transportation slider moves towards the proximal end.
- the transportation slider presses against the proximal end of a clip during movement in distal direction and slides the said clip on further by one position in the clip magazine.
- the flexible slide element can deflect, thus preventing any return conveyance of the clips.
- the transportation slider can be formed from a flat spring part, e.g. a spring metal sheet.
- the transportation mechanism in accordance with the invention is particularly beneficial for clips made of plastic, e.g. clips made of material capable of resorption by the human body.
- the shaft tube can have an inspection window, which permits establishment during an operation whether there are sufficient clips provided in the clip magazine.
- the clip magazine can feature a partition generally extending over the entire length of the clip magazine, which forms the base of the clip channel.
- a fork-shaped guide piece at the distal end of the push rod moves in distal direction, which acts upon guide faces of the jaw element and closes the jaw element with the clip contained therein.
- the shaft tube is preferably mounted in a rotatable manner in the handle grip and thus enables the jaw element to be rotated through 360° during use. It is intended that the jaw cheeks of the jaw element are inclined in relation to the longitudinal axis of the shaft tube by approx. 10° to 20°, preferably 12°. The inclination of the jaw cheeks assists manipulation of the applicator head during use.
- the actuating mechanism comprises a release lever located on the handle grip, which acts upon the push rod.
- the release lever is a component of the push rod mechanism with which both the jaw element during movement of the push rod in distal direction as well as the transportation slider during movement of the push rod in proximal direction are actuated.
- the release lever can be loaded by the force of a leaf spring against the return force of the push rod, which forms a slidable stop for the release lever. On moving the release lever in contra-direction to the normal direction of actuation the stop formed by the leaf spring can be pushed back until the push rod is released, allowing removal from the actuating mechanism.
- the clip magazine features a casing of basically semi- cylindrical cross section, which can be inserted into the shaft tube and which, with the shaft tube section recessed out for the clip magazine, completes a circular-shaped cross section conforming to the diameter of the shaft tube.
- a clip magazine therefore does not increase the diameter of the shaft tube, so that clip applicators having a shaft tube diameter of 10 to 15 mm, preferably of 12 mm, can be manuf ctured, which can be used in conjunction with conventional trocar sleeves.

- Fig. 16A the transportation of the clips by means of the to 16D transportation slider
- Fig. 17A transportation and release of the to 17D transportation slider in a plurality of phases.
- the clip applicator 1 depicted in Fig. 1 features a handle grip 2, in which is located an actuating mechanism 4, basically comprising a release lever 18 and a push rod 8.
- an actuating mechanism 4 basically comprising a release lever 18 and a push rod 8.
- the push rod 8 With the release lever 18 the push rod 8 can be pushed in the direction towards the distal end 16 of the clip applicator 1.
- the push rod 8 is pressed back again into the initial position with the aid of a return spring 5.
- the push rod 8 lies with a plunger 9 against the lever arm 21 of the release lever 18, upon which a counter-torque is imparted by a leaf spring 15 on the lever arm 21.
- the leaf spring 15 forms a stop.
- the leaf spring 15 can be rotated against the normal direction of actuation, whereupon the lever arm 21 shown on the drawing continues to be displaced towards the right, allowing the push rod 8 to be removed from the shaft tube 6 for sterilisation.
- the shaft tube 6 is rotatably mounted in the casing 3 of the handle grip 2 and can be rotated through 360° with the aid of a grip ring 7.
- the push rod 8, which is designed rotationally symmetrical at its proximal end, is also rotated with the shaft tube.
- the shaft tube 6 houses a clip magazine 12 in its distal region 16, comprising a magazine upper part 17, a transportation slider 24 and a partition 40, which, shown on the drawings in a downwards direction, partitions off a clip channel 26.
- the shaft tube 6 houses the clip magazine 12 as an interchangeable unit in a recess 38.
- the clip magazine 12 is pushed inwards to hook underneath at the distal end of the shaft tube 6, and engages the shaft tube 6 at the proximal end of the recess 38 with the aid of a spring-actuated pawl.
- the distal end 16 of the shaft tube additionally houses a jaw element 14, which can be extracted, but is statically located in its position in the shaft tube.
- the shaft tube 6 is connected to the grip ring 7 in a manner which prevents rotation and is coupled via the grip ring 7 to the casing 3.
- the push rod 8 with its circular cross section passes through the grip ring 7 and into the casing 3, the push rod 8 comprising a plunger 9 at its proximal end which presses against the lever arm 21 situated in the casing 3.
- the return spring 5 consists of a helical spring, which surrounds the proximal end of the push rod 8 and which lies with its one end against the end face of the plunger 9 lying opposite the lever arm 21 and which supports itself by the other end against a washer ring 29 slidable on the push rod 8, which in its turn supports itself against the detachable clip magazine 12.
- the plunger 9 is, for example, screwed on the proximal end of the push rod 8.
- the clip applicator 1 can be completely dismantled with little manipulative effort. As already stated, the clip magazine 12 and the jaw element 14 and the push rod 8 can be removed. After loosening the screws, the shaft tube 6 can be separated from the casing 3.
- the clip magazine 12 is made preferably from plastic and is an item for single use only (not reusable), which cannot be sterilised.
- the remaining components of the clip applicator 1 can be dismantled and are of simple structure, so that re-sterilisation is feasible without any problem. Consequently, apart from the clip magazine 12, the clip applicator 1 is made preferably of metal.
- an inspection window 39 On the side in the clip magazine 12 at the distal end 16 of the shaft tube 6 is situated an inspection window 39, which permits visual monitoring of any clips 10 still situated in the clip magazine 12.
- the shaft tube features, in cross section, a circular-shaped form having a diameter of maximum 15 mm, preferably 12 mm.
- the clip magazine 12 is inserted into a recess 38 and, together with the remaining cross section of the shaft tube 6, restores once again a circular-shaped cross section with the diameter of the shaft tube 6.
- the push rod 8 is guided longitudinally in the shaft tube 6 and extends beneath the clip magazine 12 in the distal region 16.
- the fork-shaped distal end 52 of the push rod 8 is used to close the jaw element 14 upon further forward movement of the push rod 8, in order to close for its part clip 10 situated in the jaw element.
- Figs. 8 and 9 show the distal end 52 of the push rod 8 in plan view prior to, and during actuation of the push rod.
- the distal end 52 of the push rod 8 is designed as a fork and has two cheeks 58, which cooperate with guide faces 55 of the jaw element 14.
- the push rod 8 extends with its distal end 52 beneath the jaw element 14, the cheeks 58 projecting in the direction towards the jaw element 14.
- the guide faces 55 of the jaw element 14 open outwards so that the cheeks 58 close the jaw element 14 when the push rod 8 is moved in distal direction.
- the jaw element 14 is inclined by approx. 10° to 20°, preferably 12°, on its distal end, which accomodates the clip guide channel 54.
- the clip guide channel 54 terminates in front of the distal end of the jaw cheeks 59, so that a stop position is created for the clip introduced into the jaw element 14. At least one of the jaw cheeks 59 has a recess 60 in which the projection 13 of a shank of the clip 10 can engage.
- the jaw element 14 is introduced through an end face opening 61 into the shaft tube 6 and fixed by a thread facility 62 and a screw 57 to the shaft tube.
- the thread facility 62 is arranged in a slot 64 of the push rod 8 and guides the push rod 8 in the distal region.
- the push rod 8 transfers the actuation force of the release lever 18 to the jaw element 14 with a clip 10 situated therein, in order to close same, whereupon the clipping operation is concluded. In so doing, push rod 8 is moved forwards towards the distal end of the shaft tube.
- a projection 27 of the push rod 8 projecting in the direction towards the transportation slider 24 can carry along a projecting hook-shaped end 25 of the transportation slider 24, projecting in the direction towards the projection 27, in proximal direction, until at a predetermined position a projection 28, projecting from the magazine upper part 17 in the direction towards the transportation slider 24, breaks the link between the transportation slider 24 and push rod 8.
- the transportation slider 24 rushes forward in distal direction and results in pushing the clips 10 situated in the clip magazine 12 forwards by one position, whereupon the foremost clip 10 situated in the clip magazine is moved forwards into the jaw element 14 up against a stop.
- the transportation slider 24 features, as best depicted by Figs. 6 and 7, tongue-shaped slide elements 20, which, as can be seen in Figs. 4A and 5A, act upon the proximal end of each clip situated in the clip magazine 12, thus transporting the clip away towards the distal end.
- the slide elements 20 are manufactured in one piece as part of the transportation slider 24, which is produced from an elastic material. According to Fig.
- the slide elements 20 project downwards in the clip channel, wherein the transportation slider 24 above the clips 10 is arranged in unison with the said clips in the clip channel 26. Due to the form of the clips 10, the clips being made of a material capable of resorption by the human body and depicted in the drawings, the slide elements 20, as best depicted by Fig. 7, are arranged on the side of the transportation slider 24. In the case of other clip forms, the slide elements can be arranged accordingly at another position, in order to enable optimum transportation of the clips.
- the clip channel 26 features, as best depicted by Fig. 5A, lateral recesses 19 on both sides, in which projections 13 on - li ⁇
- the shanks 10a, 10b, projecting outwards can engage under spring force effect and can secure the clips at that point from moving in proximal direction.
- the transportation slider 24 presses the clips in distal direction under pretension. After actuation of the jaw element the push rod moves in proximal direction, whereupon the slide elements 20, as indicated in Fig. 6, spring upwards so as to arrive behind the clip lying in proximal direction. At the same time, the clips 10 are held securely with the aid of their projections in the recesses 19, so that the clips remain in their position during return transportation of the transportation slider 24. upon release of the transportation slider 24 the said transportation slider rushes forward, whereupon all clips 10 of the clip magazine are pushed forwards by one position.
- the recesses 19 are arranged laterally in a saw tooth-like manner in the clip channel 26, wherein the width of the channel of the clip channel 26 is reduced conically in distal direction in front of each recess 19.
- the clip shanks 10A, 10B are therefore pressed slightly together against the spring tension of the shanks prior to reaching the recesses 19, so that upon reaching the recesses 19 they can engage these by their projections 13 under spring tension.
- Fig. 10 shows a cross section along the line X-X in Fig. 4A.
- the clip channel features a groove 30 on the side of the slide elements 20 shown extending upwards on the drawing, which forms the necessary clearance, during deflection of slide element 20 during the passing over of a clip 10, for a slider 31 extending orthogonally in relation to the transportation slider 24 and transversely in relation to the clip channel 26.
- Fig. 12 shows the slider 31 of the slide element 20 in its deflected position during the movement of the transportation slider 24 in proximal direction and during the passing over of a clip 10 held in its position in the clip channel 26.
- Fig. 11 shows a section along line XI-XI in Fig. 7.
- the compression spring 23 supports itself at its end against the magazine upper part 17 and presses against the transportation slider 24 with its distal end and pretensions the said transportation slider 24 in distal direction.
- the compression spring 23 thus surrounds a narrower section 32 of the transportation slider.
- the transportation slider 24 widens out once more to the width within the clip channel 26, whereby a stop 22 is formed in conjunction with the magazine upper part 17 in distal direction, which is adapted to the position of the foremost clip 10 in the jaw element 14.
- Fig. 13 shows a section along the line XIII-XIII in Fig. 4B. It can be seen that projection 27 is formed by two wedge-shaped ramps, wherein the hook 25 of the transportation slider 24 formed, for example, by a scroll of the spring material can hook at the end face 33 facing the proximal end of the push rod 8. The ramp-like form of the projection 27 thus facilitates the lifting of the hook 25.
- Figs. 14 and 15 show corresponding cross sections along the lines XIV-XIV and XV-XV in Fig. 4B.
- Figs. 16A - 16D show the conveyance of clips 10 in various phases in the clip magazine 12.
- push rod 8 moves in distal direction, whilst the transportation slider is pressed into its stop position under the effect of compression spring 23 in distal direction, wherein a clip 10 is situated in the jaw element 14 and the stop 22 lies against the magazine upper part.
- Push rod 8 moves in Fig. 16B in proximal direction, wherein, as depicted in Fig. 17B, the hook 25 of the transportation slider 24 is carried along by the end face 33 of the projection 27 in proximal direction.
- the slide element 20 springs upwards, the slider 31 moving into the groove 30 seen in Fig. 10 of the clip channel 26.
- Figs. 16C and 17C depict the moment in which the link between the transportation slider 24 and the push rod 8 is broken. The push rod is thus moved onwards in proximal direction.
- Figs. 16D and 17D show the phase in which the transportation slider 24 rushes forward and pushes forward all clips 10 situated in the clip magazine 12 by one position.
