Definitions
- the present invention relates to a nail for external fixation devices.
- External fixation is a method for treating fractures which is based on the application of an external fixation device, constituted by a central body and by a variable number of metallic nails, without surgically affecting the fracture.
- the central body of the fixation device ensures stability of the reduction of the fracture and in some cases allows to act on the fracture, appropriately modifying its alignment axes, in order to improve reduction of the axial misalignment of the fracture heads.
- the central body is connected to the bone segments above and below the fracture by the nails.
- Said nails are rods with a threaded end which is inserted in the bone through the skin and the various soft tissues and grips said bone; the other end is instead connected to the body of the fixation device by means of clamps.
- One of the main problems of external fixation is constituted by the progressive decrease of the grip of the nails in the bone.
- the firmness of the grip of the nails in fact decreases in the course of time, and obviously if the functioning of the nails is altered, the entire treatment of the fracture can be compromised.
- This decrease in mechanical grip is caused by many factors, such as the initial damage produced in the bone as a consequence of the insertion of the nail, the constant forming of fibrous tissue around the metallic nail, the forming of infections, and the effect of the load.
- a principal object of the present invention is to solve the above problem by providing a nail which has significantly better grip in the bone than conventional ones.
- a further object of the present invention is to provide a nail wherein improved performance does not negatively affect its low cost.
- a nail which is characterized in that at least the portion for engaging in the bone is coated with an applied material which has bone regeneration-aiding properties.
- figure 1 is a view of a nail according to the invention.
- the nail comprises a stem 1 which ends with a threaded portion 2 which is meant to engage in the bone.
- the numeral 3 designates the layer of applied bone regeneration-aiding material which covers the threaded portion 2 and also affects the surface of the stem which remains in contact with the soft tissues after the nail has been fixed.
- Hydroxyapatite or other materials having similar performance are preferably used as the material having bone regeneration-aiding properties. Examples of these other materials are: — hydroxyapatite bound with halogens such as hydroxyfluoroapatite and related mixtures, including hydroxyapatite;
- the applied material may also be of the absorbable type, particularly when its absorption occurs in a time considered to be shorter than that necessary to perform the task for which it is intended.
- the coating of bone regeneration-aiding material can also extend to the surface of the nail which is in contact with soft tissues.
- the thickness of the applied material may be any, the coating of the nail has a thickness on the order of 30-80 microns.
- the performance of the coating can be improved by using suitable binding agents between the surface of the nail and the applied material.
- the applied material may be deposited with any known deposition technology, particularly by plasma spray.
- the bone regeneration-aiding treatment may also be constituted not by a coating but by an impregnation or diffusion, with known methods, of ions or compound elements that aid bone regeneration in the structure of the nail.
- Nails treated in the above described manner in fact have a significantly greater grip in bone tissue than uncoated ones; it has furthermore been demonstrated that this advantage persists for a long time after in vivo implanting.
- the basic reason for this behavior is that the applied material, interposed between the bone and the metal, leads to higher values of the initial extraction force, and that the direct contact which occurs between the surface coating of bone-regeneration aiding material and the bone ensures higher values of extraction force after some time.
- nails coated with bone-regeneration aiding material provide excellent contact between the bone and the surface of the nail.
- the cortical bone in fact become denser and expands, growing directly in contact with the bone-regeneration aiding material.
- another significant advantage of coated nails according to the invention is constituted by a reduction in the frequency of infections.
- the stem can be metallic or composite and may have any configuration.
- the device can in fact be used successfully also with cylindrical, conical, frustum-shaped and similar stems.
