Abstract

L'invention se rapporte à un instrument de chirurgie qui sert à pratiquer l'ostéotomie précise de la bosse nasale (système de bistouri à destination nasion, dit DNCS). Ce système DNCS est constitué par un instrument en métal inoxydable composé d'un guide-lame (1), d'une lame (2) et d'un manche (3). Ces trois éléments sont montés lors de l'utilisation et démontés pour permettre leur nettoyage et leur stérilisation. Le guide-lame (1) permet, en dépit du manque de visibilité du champ opératoire, au chirurgien de localiser plus facilement avec précision le nasion (N), qui constitue le point de destination de l'ostéotomie. Le guide-lame (1) comporte une ouverture (1.1) permettant l'introduction de la lame (2). L'antinasion (1.7) de sa barre (1.2) vient se placer sur le nasion (N). La lame se déplace sur l'appui glissant (1.6) du guide-lame et entre les ailes (1.4) et également entre les cadres de glissement (1.5). Le guide-lame comprend également une douille (1.3) permettant son assemblage avec le manche (3). La lame (2), qui est l'élément coupant du système (DNCS) comporte une partie distale étroite (2.4) avec un biseau (2.6) et un bord tranchant (2.3). Cette partie de la lame s'adapte dans le guide-lame (1). La lame (2) comprend une partie proximale large (2.1) avec deux butées (2.2) et un pôle (2.5). Le manche (3) est assemblé par sa languette (3.1) au guide-lame (1). En outre, la position du manche (3) dans cet espace sert de guide pour pratiquer une ostéotomie à hauteur constante des os du nez. Ainsi, un tel système DNCS sert d'instrument grâce auquel le chirurgien-plasticien élimine le problème du manque de visibilité du champ opératoire pendant la rhinoplastie, en particulier pendant l'ostéotomie de la bosse nasale, et peut effectuer une telle opération, à l'aide de ce système DNCS, avec une haute précision et sans danger.
