Abstract
An enterostomy bandage comprising a skin-protective plate (11) of an adhesive material suitable to be attached directly against the body. According to the invention, the enterostomy bandage is characterized in that a stiffening oblong section (12) is integrated with the central portion of the skin-protective plate, which section (12) is intended to be inserted under and support an intestine loop drawn forth out of the body.
Description
Enterostomy bandage This invention relates to an enterostomy bandage.
At several surgical operations against diseases of the intestine, the surgeon is forced to expose an artificial of the intestine on the abodomen, a so-called colostomy .when the colon is concerned or an ileostomy when the ileum is concerned.
A stomy can be the transversely cut-off remaining end of the intestine when, for example, the rectum has been removed by operation because of tumor. In this case only one intestine opening opens in the abdominal skin. It is. however, necessary to temporarily relieve an intestine section in order to render possible a healing process which is undisturbed by the passing of feces, for example of an operation wound in the rectum opening or at inflammatory intestinal processes. At these occasions a loop, mostly of the right-hand portion of the colon transversum is drawn forth to the outside of the abdominal skin and retained there by a supporting rod of varying design which is inserted one decimeter under the loop.
This supporting rod has the object to prevent the intestinal loop from being retracted into the abdominal cavity, which would result in peritonitis.
At present the support usually consists of a round or flat rod of rigid plastic material with or without movable parts. The support mostly is sutured to the skin. After the top of the exposed intestinal loop has been cut through, two intestine openings are created, one in the oral and one in the anal direction. This condition is defined as double-leg colostomy or "loop-colostomy" . Analogous thereto the transversely cut-off and exposed intestine portion is defined as one-leg colostomy.
What is said hereinafter applies both to colostomy and ileostomy, but the colostomy conditions have been chosen for exemplifying the invention.
It is apparent that the operation methods here dealt with imply considerable bandaging problems. The feces coming forth from the intestine must be collected in a bag attached expediently and fastened in the skin about the colostomy. Gas and excrements must be prevented from leaking out. In practice this is effected in such a way, that in a plate of varying material, for example caraya rubber or different adherent synthetic or natural substances, a round opening is cut open which has the same size as the intestine opening. The body heat causes the incision to fuse with the plate. The plate is pasted on the skin, and a bag is pasted on the plate. Though fat rolls and folds in the abdomen skin may render bandaging difficult , the colostomy bandage normally acts satisfactorily at one-leg colostomy.
There are, however, great difficulties involved in bringing about adequate bandaging in the initial period after double-leg colostomy. This is due above all to the problems of causing the bandage to seal about the supporting rod, which is removed after about one week. These substantial problems have been the reason of countless attempts to vary the bandaging in the post-operative phase.
The present invention eliminates these difficulties in respect of bandaging and double-leg colostomy.
The present invention, thus, relates to a colostomy bandage comprising a skin-protecting plate of an adhesive material suitable to be attached directly to the body, and is characterized in that a stiffening oblong section is integrated with the central portion of the skin-protecting plate, which section is intended to be inserted under and support an intestine loop drawn forth out of the body.
The invention is described in greater detail in the following, with reference to an embodiment thereof shown in the accompanying drawing, in which Fig. 1 shows an example of a conventional colostomy bandage, Fig. 2 is a section along the line B-B in Fig. 1, Fig. 3 shows a colostomy bandage according to the invention, Fig. 4 is a section along the line C-C in Fig. 3, Fig. 5 is a section along the line A-A in Fig. 3, and Fig. 6 is a perspective view of a colostomy bandage according to the invention.
In Fig. 1 an example of a conventional colostomy bandage is shown and, there-fore, "PRIOR ART" is indicated in the Figure.
Such a colostomy bandage comprises a skin-protective plate 1, for example of a material marketed under the registered trade name STOMAHESIVE by Squibb Surgicare Ltd. Also other materials, however, may exist. These materials have in common good adherence to the body. Above the skin-protective plate a plastic film is provided, on which a colostomy bag (not shown) can be pasted. In Fig. 1 a plastic ring 2 is shown for connecting a bag.
In Fig. 1 two intestine openings 3,4 are shown which have been drawn forth and sutured as described above. A supporting rod 5, which comprises a foldable cross-bar 6, has been inserted under the intestine loop 3.4 and abuts the body 10 about the operation wound 7. The skin-protective plate 1 at the beginning is unbroken. A hole 8 corresponding to the intestine loop is cut in the skinprotective plate 1 whereafter the plate is attached directly on the body 10. The skin-protective plate either covers the supporting rod 5. or the skin-protective plate is applied under the supporting rod as shown in Fig. 1. In Fig. 2 a section along the line B-B is shown, from which appears that a space 9 is formed to the sides of the
supporting rod 5. Feces from the intestine opening 3,4 flow into this space, for example as indicated by the arrow 27 , and thus into the operation wound.
The afore-described method belongs to prior art. The present invention, more precisely, has the object to eliminate both the problem that feces penetrate into the operation wound, and a problem implying that feces also penetrate under the skin-protective plate at its connection to the supporting rod, especially when the skin-protective plate is located over the supporting rod. This latter problem causes the skin-protective plate to partially loosen, so that feces can leak out outside the bandage, which gives rise to sore skin and pains.
According to the present invention, the stomy bandage comprises a skin-protective plate of an expedient materual, for example STOMAHESIVE, which plate has substantially the same size and thickness as the conventional plates.
The skin-protective plate 11 further comprises a stiffened section 12, which is stiffened in that a stiffening oblong bar 13 is attached in, or rigidly mounted on the surface of the skin-protective plate (shown by dash-dotted lines in Fig. 3). The bar 13 has such a thickness and is so attached in or at the skin-protective plate 11 that its thickness at said section only slightly exceeds or substantially is equal to the thickness of the remaining skin-protective plate.
The invention, thus, briefly implies to integrate a stiffening bar 13 or corresponding member in the skin-protective plate 11.
According to a preferred embodiment, the skin-protective plate is perforated along two lines 14,15 extending from one edge 16 of the skin-protective plate 11 past and on both sides of the bar 13.
According to a further preferred embodiment, the skin- protective plate 11 is provided with two semicircular perforations 17,18 in the place for the stiffened section 12, which perforations have an area corresponding to the size of the intestine loop 3,4 on both sides of said section.
After an intestine loop has been taken out, opened and sutured, the two semicircular areas 19, 20 defined by the perforations 17, 18 are pressed out. Also the perforations 14,15 for the stiffened section 12 are broken. Thereafter the stiffened section 12 is folded down so that the skin -protective plate assumes in principle the appearance shown in Fig. 6, whereby said section 12 is inserted under the intestine loop to a position corresponding to the position of the supporting rod 5 in Fig. 1. Thereafter the areas 21,22 of the skin-protective plate are folded down which enclose said section 12 against the skin. After a while the joints marked in Fig. 6 by 23,24,25,26 are joined together by action of body heat. Due to the fact that the stiffening section is thin and enclosed by an adhesive material, the intestine loop sealingly abuts the section 12, see Fig. 4. Therefore, such spaces 9 as formed by utilizing a supporting rod 5 are not formed, and feces are prevented from running into the operation wound. By said joining together, furthermore, a tight bandage is established about the operation wound, whereby feces are prevented from leaking in under the skin-protective plate to a greater degree than in the case when a hole 8 is cut and a supporting rod 5 is utilized.
Furthermore, the inconveniences of wear and discomfort arising when a supporting rod 5 at movements of the body wears against the skin-protective plate 1 and skin are prevented, which inconveniences are involved at convent
ional stomy bandage, especially when the supporting rod is applied under the skin-protective plate.
For information can be mentioned, that STOMAHESIVE contains gelatine, pectin, sodium carboxymethyl cellulose and polyisobutylene.
The bar 13 consists of an elastic, but stiff material, and preferably the bar is a plastic bar, but it can be manufactured of any suitable material.
The bar preferably has a length of 50-100 mm and a width of 2-7 mm.
The bar 13 preferably is provided with a hole 27 to be utilized for facilitating bandage exchange.
When no perforations 14, 15,17,18 are provided, the colostomy bandage is cut open prior to its application. In this case the colostomy bandage can be provided with marking lines, along which the cutting is to be made.
On the outside of the skin-protective plate 11 described a collecting bag (not shown) can be attached directly.
The present invention must not be regarded restricted to the embodiment set forth above, but can in respect of dimensions and material choice be varied within the scope of the attached claims.
Claims
1. An enterostomy bandage comprising a skin-protective plate of an adhesive material suitable to be attached directly against the body, c h a r a c t e r i z e d i n that a stiffening oblong section (12) is integrated with the central portion of the skin-protective plate, which section (12) is intended to be inserted under and support an intestine loop (3,4) drawn forth out of the body (10).
2. An enterostomy bandage as defined in claim 1, c h a r a c t e r i z e d i n that said section (12) comprises a bar (13) of an elastic, but stiff material, preferably a plastic bar.
3. An enterostomy bandage as defined in claim 2, c h a r a c t e r i z e d i n that said bar (13) is located within the skin-protective plate (11).
4. An enterostomy bandage as defined in claim 2, c h a r a c t e r i z e d i n that said bar (13) is rigidly attached to the skin-protective plate (11).
5. An enterostomy bandage as defined in claim 1,2,3 or 4, c h a r a c t e r i z e d i n that the skin-protective plate (11) is provided with perforations along two lines (14,15) extending from one edge (16) of the skin-protective plate past and on both sides of the bar (13).
6. An enterostomy bandage as defined in claim 1,2,3, 4 or 5, c h a r a c t e r i z e d i n that the skin-protective plate (11) is provided with two semicircular perforations (17,18) at the place for said section (12), which perforations (17,18) indicate an area, which corresponds to the size of an intestine loop (3,4) on the sides of said section (12).
