The dash-dot-dash lines showing the ovals in FIG. 1 illustrate printed indicia that form part of the claimed design. The dash-dot line showing a horizontal line at the center of FIG. 2 illustrates a perforation that does not form part of the claim design. The dashed lines showing circles in FIGS. 1 and 2 depict portions of the wristband form with extender that form no part of the claimed design.
