Description

用於植入脊椎體內之支撐件及使用此支撐件之植入系統
本新型係有關於一種用於植入脊椎體內之支撐件及將支撐件植入脊椎體內之植入系統,尤指一種可在脊椎體內進行組合之支撐件及其植入系統。
骨質疏鬆症的病人容易因骨質流失而併發脊椎壓迫性骨折,大多數病人的脊椎壓迫性骨折會在數個月內癒合,然而在恢復期間內,病人常常會因持續的腰背疼痛、神經壓迫而活動不便、臥病在床,甚至產生褥瘡、肺炎、尿路感染等併發症。部分罹患脊椎壓迫性骨折的病人會發生骨折不癒合而持續長久疼痛,進而導致脊椎整體穩固性不足與駝背變形,使得相鄰脊椎節也發生骨牌效應式脊椎壓迫性骨折。
「開放性後側脊椎固定」與「骨融合手術」是採用螺釘鑽入脊椎中以作為支撐,但常因患者本身骨質疏鬆而使得螺釘容易鬆脫,只好加大傷口而使用更多螺釘來固定更多節脊椎,如此手術時間長與流血大量易使年老病人產生更多併發症,例如心肺功能衰竭、感染等。
「微創經皮穿刺椎體成形術」是使用骨穿刺針器具,在X光影像導引下,將骨水泥經由小傷口穿過椎莖突灌入骨折的脊椎體內,藉以支撐崩塌陷落之脊椎體,以達到止痛效果。然而此方法無法矯正變形的椎體。另一種所謂「微創經皮穿刺椎體擴張成形術」,則利用一可擴張之球體來恢復椎體的高度,隨後移出球體,再灌入骨水泥於因球體擴張所製造的空腔內,藉此矯正部分的脊椎體崩塌變形。
以填充物經由椎莖而植入脊椎體是目前一種新的加速脊椎壓迫性骨折復原之方法,然而當所需的填充物尺寸過大時,在穿過椎莖時可能會傷害周圍神經,但若植入尺寸不足的填充物,則無支撐脊椎體的效果。
鑒於習知技術在治療脊椎壓迫性骨折的不足及治療方法上的缺失,本新型提供一種用於脊椎體內之組合式支撐結構,包含:一中空體具有一上表面及一下表面相對該上表面;一第一導引部形成於該上表面;一第二導引部對應該第一導引部形成於該下表面;一第一扣合部形成於該上表面;以及一第二扣合部對應該第一扣合部形成於該下表面,其中第二導引部係由中空體一側向外延伸出一延伸部,以提供另一支撐件之第一導引部滑動結合,另一支撐件藉由延伸部之引導滑動至支撐件之下表面,兩支撐件分別之第一、二導引部彼此滑接且第一、二扣合部彼此扣合。依此概念,單個支撐件可植入個體的脊椎體內,或二個或二個以上的支撐件可依序導入個體的脊椎體內並在其內進行組合。
醫療人員可視需求將單一支撐件植入個體之脊椎體內,或將複數支撐件(二件或二件以上的支撐件)先後植入個體之脊椎體內,每一支撐件可製為方便植入脊椎體之小尺寸,再於植入脊椎體內時扣合於先行入的支撐件。藉此以改進習知技術中以單一填充物植入脊椎體所產生容易傷害椎莖周圍神經或尺寸不合而無支撐效果之缺點。
本新型另提供一種植入系統,應用於包含一椎莖突(pedicle)及一脊椎體(vertebra)之個體脊柱(spine),所述植入系統包含:一外導管穿透椎莖突並進入脊椎體;一第一支撐件,可由外導管進入脊椎體內;以及一可視需要的第二支撐件,可由外導管進入脊椎體內,該第一支撐件該第二支撐件可相互扣合;其中第一、二支撐件分別包含一上表面及一下表面,各上表面分別形成有一第一導引部以及一第一扣合部,各下表面分別形成有一第二導引部及一第二扣合部;其中第一導引部係對應第二導引部;其中第一扣合部係對應第二扣合部;其中第一支撐件之第二導引部係延伸出一延伸部於外導管中,以供第二支撐件之第一導引部在外導管內滑動結合,第二支撐件藉由延伸部之導引滑動至脊椎體中,令第二支撐件之第二扣合部扣合於第一支撐件之第一扣合部。
本新型之詳細技術及較佳具體實施例,配合所附圖式描述如後,使本領域之技藝人士可理解所請新型之特徵。
除非另行定義,所有在本文中所使用的技術及科學術語具有如同本新型所屬技術領域中之技藝人士一般所瞭解的意義。
本文中所使用的「一」乙詞,如未特別指明,係指至少一個(一個或一個以上)之數量。
請參閱圖1及圖2,分別為本新型用於脊椎體內之支撐件一立體示意圖及一剖面示意圖,其中圖2係由圖1沿剖面線AA’之剖面示意圖。支撐件100包含一上表面100a及一下表面100b。一第一導引部102例如為一凹槽形成於上表面100a,一第二導引部104例如為一凸條可配合於第一導引部102之凹槽形成於下表面100b。一第一扣合部106可包含複數鋸齒形成於上表面100a,第一扣合部106可沿第一導引部102兩側形成,每一鋸齒可包含一第一斜坡106a及一第二斜坡106b斜率大於第一斜坡106a。一第二扣合部108可包含複數鋸齒形成於下表面100b,第二扣合部108可沿第二導引部104兩側形成,每一鋸齒可包含一第三斜坡108a及一第四斜坡108b斜率大於第三斜坡108a。支撐件100可包含一中空體,具有螺孔形成於一側表面位於上下表面間,且具有複數開口112形成於一或多側表面位於上下表面間,所述複數開口112係連通於支撐件100之中空體內。支撐件100之中空體可填充自體骨、移植骨或人工代用骨,支撐件100植入人體之脊椎體後可與周圍骨質融合而促進骨折癒合。
第二導引部104之凸條可向支撐件100之中空體外延伸一延伸軸104b,一小凸條104a可形成在第二導引部104之凸條及延伸軸104b之一側表面其較遠離支撐件100之中空體。第一導引部102之凹槽包含一開放端102a由支撐件100一側表面連通於外部以及一封閉端102b以定義第二導引部104之凸條的移動極限。一小凹槽102c對應第二導引部104之凸條的小凸條104a可形成於導引部102之凹槽中,小凹槽102c係水平嵌入支撐件100對應封閉端102b中以形成一固定凹部,小凸條104a則可略突出於第二導引部104之凸條的一端而形成一固定凸部進入固定凹部中,而當一支撐件100之小凸條104a進入另一支撐件100之固定凹部時,一支撐件100之第一斜坡106a係抵接於另一支撐件之第三斜坡108a,一支撐件100之第二斜坡106b係抵接於另一支撐件之第四斜坡108b。
實際應用時,複數支撐件100可逐一地藉由導引設備植入人體的脊椎體中,並於脊椎體中組合。當第一個支撐件100植入後,第二導引部104之延伸軸104b因向外延伸,因此可將隨後植入的支撐件100之第一導引部102的凹槽滑動結合於第一個支撐件100之第二導引部104的延伸軸104b並朝向第一個支撐件100滑動,藉此在脊椎體中準確的與第一個支撐件100組合。當兩支撐件100確實組合完成後,可將第一個支撐件100多餘的第二導引部104之延伸軸104b剪除,再以相同方式將第三、四及更多支撐件100植入。
支撐件100之材料可為金屬、高分子塑膠、陶瓷材料、或生物可降解材料。金屬可例如為不銹鋼、鈷鉻鉬合金、鈦或鈦合金。高分子塑膠可例如為聚醚醚酮(polyetheretherketone,PEEK)、超高分子量聚乙烯(Ultra High Molecular Weight PolyEthylene,UHMWPE)、聚甲基丙烯酸甲脂(Polymethylmethacrylate,PMMA)或矽橡膠。陶瓷材料可例如為氧化鋁、磷酸鈣鹽類、生醫玻璃等)等。生物可降解材料材料可例如為聚二惡烷酮(polydioxanone)、聚己內酯(poly(ε-caprolactone))、聚酸酐(polyanhydride)、聚原酸酯(poly(ortho ester))、共聚醚酯(copoly(ether-ester))、聚酰胺(polyamide)、聚乳酸(polylactone)、聚富馬酸丙二醇酯(poly(propylene fumarate)、聚乳酸(poly(lactic acid))、聚甘醇酸(poly(glycolyic acid))、共聚乳酸-甘醇酸(poly(lactide-co-glycolide))或其組合。

如圖4所示,一外導管200穿過一人體之椎莖突300以進入塌陷之脊椎體400內。將外導管200導入脊椎體400之方法係先以X光影像導引一中空之骨穿刺針(圖未示)鑿開椎莖突而進入塌陷的脊椎體400,再將一導引鋼針(圖未示)沿著骨穿刺針器內置入脊椎體400內,接著拔除骨穿刺針,將外導管200順著導引鋼針進入脊椎體400。安裝好外導管200後,將一第一支撐件100c以其螺孔(圖未示)結合一內導管201。第一支撐件100c醫療人員可藉由內導管201推動第一支撐件100c由外導管200內滑入脊椎體400中。接著可將內導管201自第一支撐件100c卸除。第一支撐件100c的第二導引部104之凸條所延伸出的延伸軸104b此時係位於外導管200中。
如圖5所示,將一第二支撐件100d以其螺孔(圖未示)結合一內導管201後,利用第二支撐件100d之第一導引部(圖未示)先滑動結合於第一支撐件100c之第二導引部104的延伸,由外導管200內滑入脊椎體400中,同時稍微擠壓第一支撐件100c向下移動(或向上移動)。第二支撐件100d向脊椎體400內移動時,第二支撐件100d之第二扣合部108的鋸齒結合於第一支撐件100c所具第一扣合部106之鋸齒,且由前述對於鋸齒結構的描述可知,第二支撐件100d可順暢的向脊椎體400內推,但因受制於鋸齒的結構而無法外拉。是以,第二支撐件100d的第二扣合部108隨著第二導引部104滑接於第一支撐件100c之第一導引部(圖未示)而扣合於第一支撐件100c的第一扣合部106。當第二支撐件100d與第一支撐件100c組合至定位後,可以一中空之剪除器具順著第一支撐件100c的第二導引部104抵達第二導引部104延伸出第一支撐件100c之端面,將多餘的第二導引部104剪除並抽離。接著可將內導管201自第二支撐件100d卸除。第二支撐件100d的第二導引部104之凸條的延伸此時係位於外導管200中。在一具體實施態樣中,當一支撐件植入脊椎體內時,可使塌陷之脊椎體達局部復位,可藉由重覆加入另一支撐件,達到所需的支撐高度,直到完全復位為止,優點是僅需微創小傷口即可將輕薄短小的單一支撐件送入脊椎體內,在加入另一支撐件的過程,無需加大傷口,而可達到完整支撐脊椎體之目的。
如圖6所示,重複以上步驟以將一第三支撐件100e植入脊椎體400中。第一支撐件100c、第二支撐件100d及第三支撐件100e可形成本新型之組合式支撐結構以支持塌陷之脊椎體400。然而本新型並不以包含三個支持件的支撐結構為限,可視需求將適當數量的支持件植入塌陷的脊椎體400中。
本新型用於脊椎體內之支撐件,可先後植入人體之脊椎體中。複數支撐件於脊椎體中進行組合,因此單一支撐件可具有小尺寸以方便植入脊椎體,醫療人員可視需求植入足夠數量的支撐件於病人之脊椎體中。本新型之植入系統,係利用支撐件之延伸軸,使後植入脊椎體的支撐件可沿著前一植入支撐件的延伸軸進入脊椎體中並與前一植入之支撐件準確的扣合,向前推進時,可達鎖定方向之效果,只能前進,不可後退。由此可知,本新型用於脊椎體內之支撐件及植入系統可避免習知技術中以單一填充物植入脊椎體所產生容易傷害椎莖周圍神經或尺寸不合而無支撐效果之缺點。
綜上,本創作之支撐件及其系統至少具有以下優點: 一、單個支撐件可作成輕薄短小的單體,可安全穿越椎莖而植入脊椎體內,不會破壞椎莖或損傷周圍神經。 二、可在有限狹小之脊椎體內,將複數個支撐件組合在一起,進而堅實復位塌陷脊椎體,所需植入的支撐件數量,視所需復位之脊椎體之高度而定,理論上可無限組建至填充滿實整個脊椎體為止。 三、組合複數個支撐件時,是採單方向鎖定咬合,不會鬆脫。 四、支撐件的中空體設計可讓骨質密集地向內生長。 五、支撐件的中材質可為金屬、高分子塑膠、或生物可降解材料,提供抗壓、抗拉及抗扭應力等需要的機械性質。

Claims (15)

  1. 一種用於植入脊椎體內之可組合式之支撐件,包含:一中空體具有一上表面及一下表面相對該上表面;一第一導引部形成於該上表面;一第二導引部對應該第一導引部形成於該下表面;一第一扣合部形成於該上表面;以及一第二扣合部對應該第一扣合部形成於該下表面;其中該第二導引部係由該中空體一側向外延伸出一延伸部,以提供另一支撐件之滑動結合。
  2. 如請求項1的支撐件,其中該中空體於其側表面包含至少一開口。
  3. 如請求項1的支撐件,其中該支撐件之一側表面具有一螺孔。
  4. 如請求項1的支撐件,其中該第一導引部包含一凹槽,以及該第二導引部包含一凸條,且該凸條係由該支撐件之中空體一側表面向外延伸出一延伸軸。
  5. 如請求項1的支撐件,其中該第二導引部之凹槽包含一開放端由該支撐件之一側表面連通於外部以及一封閉端。
  6. 如請求項1的支撐件,其中該第一扣合部位於沿著該第一導引部兩側形成,且該第二扣合部位於沿著該第二導引部兩側形成。
  7. 如請求項1的支撐件,其中該第一扣合部包含複數鋸齒,該各鋸齒包含一第一斜面及一第二斜面其斜率大於該第一斜面,該第二扣合部包含複數鋸齒,該各鋸齒包含一第三斜面及一第四斜面其斜率大於該第三斜面。
  8. 如請求項1的支撐件,其中該支撐件之材料包含金屬、高分子塑膠或生物可降解材料。
  9. 一種植入系統,應用於包含一椎莖突及一脊椎體之個體脊椎,該植入系統包含:一外導管,供穿透該椎莖突並進入該脊椎體;一第一支撐件,可由該外導管進入該脊椎體內;以及一可視需要的第二支撐件,可由該外導管進入該脊椎體內,該第一支撐件與該第二支撐件可相互扣合;其中該第一、二支撐件分別包含一上表面及一下表面,該各上表面分別形成有一第一導引部以及一第一扣合部,該各下表面分別形成有一第二導引部及一第二扣合部;其中該第一導引部係對應該第二導引部;其中該第一扣合部係對應該第二扣合部;其中該第一支撐件之該第二導引部係延伸出一延伸部於該外導管中,以供該第二支撐件之該第一導引部在該外導管內滑動結合,該第二支撐件藉由該延伸部之導引滑動至該脊椎體中,令該第二支撐件之該第二扣合部扣合於該第一支撐件之該第一扣合部。
  10. 如請求項9的植入系統,其中該第一支撐件及該第二支撐件係分別為中空體且於其側表面包含至少一開口,使該脊椎體內之骨質可長入該第一、二支撐件內。
  11. 如請求項10的植入系統,其中該第一支撐件及該第二支撐件分別之該中空體內係填充有該脊椎體之病人自體骨、一移植骨或一人工代用骨。
  12. 如請求項9的植入系統,其中該第一支撐件及該第二支撐件之一側表面具有一螺孔,以供結合一內導管用以推動該第一支撐件及該第二支撐件於該外導管中滑動。
  13. 如請求項9的植入系統,其中該第一導引部包含一凹槽,該第二導引部包含一凸條,該延伸部係該凸條所延伸出之一延伸軸。
  14. 如請求項13的植入系統,其中該第一導引部之凹槽包含一開放端供該第二導引部之凸條進入,以及一封閉端止擋該第二導引部之凸條。
  15. 如請求項9的植入系統,其中該第一扣合部包含複數鋸齒,該各鋸齒包含一第一斜面及一第二斜面其斜率大於該第一斜面,該第二扣合部包含複數鋸齒,該各鋸齒包含一第三斜面及一第四斜面其斜率大於該第三斜面。
