一種結構化醫療表單設計系統,係供編輯產出複數個結構化醫療表單而包含一表單狀態管理模組、一設計元件管理模組及一結構化連動模組,且該表單狀態管理模組電訊連接該設計元件管理模組及該結構化連動模組,該設計元件管理模組電訊連接該結構化連動模組,該結構化連動模組至少電訊連接外部一健康系統伺服器,該健康系統伺服器內存有分別具有一介接碼之複數個健康資料,其特徵在於:該表單狀態管理模組係設有一代碼管理單元,該設計元件管理模組設有複數個素材元件及一單張設計元件,且該等素材元件包含複數個系統連動素材及圖片,各該系統連動素材分別內設介接該健康系統伺服器之該介接碼,以供透過該介接碼截取並呈現對應之該健康資料;該表單狀態管理模組於接收一表單創建指令時,該代碼管理單元產生一表單唯一碼並驅動該設計元件管理模組,使該單張設計元件顯示一編輯表單,以供拖曳該等素材元件之其中至少一者至該編輯表單中置放及編輯;其中,為圖片之該素材元件係設有一區域標示屬性,以供於圖片中標示複數個需求區域並輸入對應各該需求區域之一區域欄位值,且該等需求區域允許與另一該素材元件設置連結關係;當該表單狀態管理模組接收一儲存指令並檢核獲知該編輯表單之表單名稱非為預設名稱時,該設計元件管理模組打包該編輯表單形成一表單數據包,且該表單狀態管理模組利用該表單唯一碼轉換該表單數據包生成具一唯一連動碼之該結構化醫療表單並傳輸至該結構化連動模組,以供透過該唯一連動碼自該結構化連動模組讀取對應之該結構化醫療表單而進行問卷調查; 該結構化連動模組自反饋之該結構化醫療表單讀取獲得複數個回覆資訊,以據此彙整、計算且分析生成一問卷報告,並回饋至該健康系統伺服器中;其中,當該結構化醫療表單中一該素材元件設有該區域標示屬性並連結另一該素材元件時,填單而點選各該需求區域即顯示對應之該區域欄位值於另一該素材元件中對應位置處並允許編輯,且各該需求區域供點選新增至少一註釋,以供該問卷報告顯示該區域欄位值及該註釋之其中至少一者。 A structured medical form design system is provided for editing and generating a plurality of structured medical forms and comprises a form state management module, a design element management module and a structured linkage module, wherein the form state management module is connected to the design element management module and the structured linkage module by telecommunication, the design element management module is connected to the structured linkage module by telecommunication, and the structured linkage module is connected to at least an external health system server by telecommunication, wherein the health system server stores a plurality of health data each having an access code.
