本申請案通常關於膜(例如聚合物膜)及製造方法,且在至少一些具體例中係關於穿孔膜及用於醫療用途之製造方法。

在整形外科或外傷手術中,感染是一大挑戰。雖有類似無菌處理及消毒處理之預防措施,手術位置仍是局部病原之接近位置以致變為致命且引起感染。
塗覆含有藥物(諸如抗生素)之可植入裝置對於降低感染已是有效的。然而,若有大的數量、尺寸、及形狀的植入體及其他醫療裝置,則對每一裝置提供塗料的規律的、經濟的、及後勤方面的負擔是龐大的。若考慮在塗料中使用另外的藥物諸如止痛劑、抗腫瘤劑及生長促進物質,則該問題被放大。
本發明之具體例係關於改良穿孔的聚合物膜及彼之新穎澆鑄製造方法。在一些具體例中,雖然該膜可用在任何應用中,該膜係與可植入之醫療裝置一同使用。
現存之形成穿孔膜的商用方法通常包含形成固態膜作為第一步驟,然後在該膜中鑽孔或割孔洞作為第二步驟。至少一些在本文中所述之具體例的優點是:在膜形成的同時,也形成該膜之孔洞或穿孔。此方法可以是有用的,當所形成之聚合物膜是極薄且因隨後之處置或加工有受損危機時,或當該膜之厚度及/或強度使其難以藉由傳統方法鑽孔或割開而不損壞該膜時。此一方法也可以是有利的,

參考圖1A之具體例,膜10(例如聚合物膜)是一種具有第一表面10a及相對之第二表面10b的可撓物體。
在一具體例中,可由單一的生物可相容材料薄片形成膜10。在一具體例中,膜10係由二或更多材料片配置。在一較佳具體例中,該生物可相容材料是生物可吸收的。在與醫療裝置12(參見圖2)同用之具體例中,膜10在一些具體例中當植入活體內時將隨著時間溶離出且被病患所吸收以致僅留下醫療裝置12(諸如若醫療裝置12並非由生物可吸收材料製成)。醫療裝置12在其他具體例中也可由生物可吸收材料製成,在此情況中,醫療裝置12及膜10二者最後皆溶解。在一些具體例中,膜10可被配

在一些具體例中,生物可吸收膜10比不可吸收之篩網更具優點,該篩網例如可利用緻密之纖維組織包裝或包埋於其中,或呈現其他與長期外來物體暴露相關的議題。在一些具體例中,膜10是僅部分可生物吸收的。
可使用生物可吸收聚合物以提供具有限定終點之藥物(諸如抗生素)之經控制的釋出。抗生素之連續長期的存在常是非所欲的,因為這可產抗拒抗生素之細菌發展的環境。在一具體例中,膜10之完全分解確保:該藥物將在預定及/或可選擇時間內完全釋出。在一具體例中,即使在該膜10未完全被吸收的情況中,該藥物可被完全釋出或基本上完全釋出。
膜10之吸收作用也可影響及/或控制抗生素在連續釋出期中的釋出。隨著該膜降解,例如該膜之滲透性可增加,且更多藥物可被釋出。在一些具體例中,所用之聚合物對於可撓之結構而言必須具有相對高之抗張強度,且能藉由溶液澆鑄被加工。在依具體例中,膜10係由包括以下四種單體之一或多種的共聚物配置:乙交酯、乳交酯、己內酯、及碳酸伸丙酯。乙交酯可被包括且可具有使膜10之降解加速的效果。乳交酯也可被包括且可具有增加膜10之機械強度的效果。己內酯及碳酸伸丙酯可被使用且可具有增加膜10之可撓性的效果。
在一具體例中,該生物可吸收之聚合物包括PLA、

另一面(內側至外側),以例如避免在膜10與醫療裝置12間產生“死角(dead space)”。另外,穿孔14比一個無此種穿孔之袖體可有利地將藥物或生物製劑更均勻地分配至鄰近組織及骨質,當該材料由該聚合物滲出時。
可配置任何尺寸及形狀之孔14。在一具體例中,孔14被基本上為圓柱體形之側壁所限定。在一些具體例中,孔14所具有之側壁具有向內凸之表面的片段。在一些具體例中,該向內凸之表面基本上是拋物線狀的。孔14之橫截面不需是完美的圓,且在一些具體例中可以是卵圓形、橢圓形、星形或菱形。在一些具體例中,孔14延伸至一或多個頂點。在一具體例中,此種頂點在使用期間促進膜10中之撕裂(例如藉由該穿孔產生弱區)。在一具體例中,孔14由內表面10b完全延伸過片12至外表面10a(參見圖4C)。在一具體例中,一或多個孔14僅部分地延伸過膜10以控制藥物釋出或增加膜10之起初強度。

在一具體例中,膜10包含抗生素。所選之抗生素對於抵抗在整形外科植入物相關感染中所發現之大部分細菌可以是活性的。這些主要包括金黃色葡萄球菌及格蘭氏陰性桿菌。
在一具體例中,所選之藥物在製造該植入物所需之製造過程中必需是安定的。在一具體例中,膜10包含建它黴素。建它黴素硫酸鹽在100℃以上是熱安定的,且對有機溶劑(包括DMSO,其在一些具體例中係用於製造方法中)是安定的。

產生所要之二期釋出。在一具體例中,該藥物經過2至3週之時間被釋出。在其他具體例中,該藥物經過較短或較長時間範圍被釋出。
在一具體例中,在此二期之期間所釋出之藥物的相對量受粒子尺寸所控制。在一具體例中,藥物成分30被均勻散佈在整個膜10內,且與膜10表面接觸之任何藥物成分30比不與膜10表面接觸之藥物成分30更快地溶解。在一具體例中,在植入時與膜10表面接觸之藥物成分30的量係被配置以在植入時之爆發中釋出。在一具體例中,藥物成分30之尺寸愈大,則與表面接觸之藥物成分30之比例愈高,且該爆發式釋出愈大。因此之故,在一具體例中,藥物成分30之尺寸保持在直徑10微米以下以使該爆發式釋出降至總藥物含量之約20至35%。在一具體例中,藥物成分30直徑是在20微米以下。
在一具體例中,膜10被配置以從一或多個獨立層(某些可不含藥物)傳遞多種藥物。在其他具體例中,膜10可包含多種藥物成分,每一者特徵在於從膜10釋出之速率係不同,以致第一藥物係與不同於第二藥物之第二釋出狀況的第一釋出狀況相關。

在一些具體例中,在此所述之方法使該薄膜10之形成及孔14之形成能在單一步驟中進行。在一些具體例中,在本文中所用之方法使膜10能有數千個孔14並形狀及放置之精確控制及膜厚度之精確控制。
參考圖3A-3G,在一些具體中,膜10係使用模18澆鑄。在一具體例中,模18包含多個由模18之底部18a延伸之凸出體或柱狀體20以形成孔14。在一具體例中,模18係由噴射成形之聚丙烯所配置。該模可由其他材料製成,包括聚合物(參見圖3F)、玻璃、金屬(參見圖3G)或陶瓷。在一具體例中,模18係由二種以上之材料配置。例如,模18可具有由金屬與聚合物塗料所製成之基底以降低該澆鑄膜對該模之黏合及/或以形成柱狀體20。在該模中之凹穴可藉由澆鑄方法、壓縮成形方法、噴射成形方法、化學蝕刻方法或機械處理方法形成。

在一具體例中,柱狀體20被安排以產生上述孔14之所選尺寸、形狀、形態、及安排。在一具體例中,圍型體或周圍壁22限定模之總面積,且該多個柱狀體20所限定之面積基本上等於或對應於膜10之開放孔面積。
在一具體例中,模18包括溝24,其延伸環繞模18之至少部分周圍壁22。在一具體例中,溝24延伸環繞模18之整個周圍壁22。在一些具體例中,溝24保留任何流動或被促動過周圍壁22之過量聚合物。在一具體例中,模18包括由模18之至少一外緣橫向延伸之延伸體40。在一具體例中,提供延伸體40以持住及操縱模18卻不與模18內之聚合物溶液接觸。
在一具體例中,形成用於添加至模18之聚合物溶液28。在一具體例中,聚合物材料在高溫下以4:1之溶劑對聚合物的比率溶在二甲基亞碸(DMSO)中且建它黴素硫酸鹽藥物被添加至13重量%。在一具體例中,聚合物溶

參考圖4A、6及8,一旦製備該聚合物溶液28,聚合物溶液28即放入單面模18中。在一些具體例中,聚合物溶液28之黏度及/或柱狀體20之密度基本上抑制該聚合物28在無外力下流遍模18。在一具體例中,在添加聚合物溶液28至模18之後,聚合物溶液28之上表面是在模18之底部18a以上的高度h2,此大於模凹穴及柱狀體20之高度h1。

膜10可藉由替代的方法製造。在一具體例中,聚合物溶液28可被澆鑄至具有支撐用緩衝層的穿孔膜材料上,然後該穿孔膜由該緩衝層移除,移除該澆鑄溶液,其中在澆鑄片中有多孔洞。此種方法與上述方法的差異是:在一些具體例中,彼不在孔14周圍產生高起的開口。
在另一具體例中,多孔膜也可藉由低壓凍乾方法或冷凍乾燥方法形成。在一具體例中,聚合物溶液之薄的固態膜被澆鑄在模中,然後該模急冷至該溶液之冰點,然後置於真空下以由該膜移除該溶劑。在一些具體例中,此方法也產生遠比在以上一些具體例中所述者小的細孔。
在一具體例中,用以形成該澆鑄膜之聚合物材料可以是可交聯之預聚物液體,其經澆鑄於如所述之膜模中,滾
壓以填充該模且移除多餘材料,然後在合適地方藉由UV輻射、溫度、觸媒或其他措施交聯。在一具體例中,此方法可產生與上述者極類似之終產物,除了該澆鑄膜之最終厚度會接近或同於該模深度,且在孔14周圍會有甚少或無新月形體或開口14a。
在另一具體例中,可以藉由網版印刷方法形成薄多孔膜。在一具體例中,溶液層被網版印刷成最後圖形,然後乾燥。在一具體例中,這產生甚薄的層,然而多個聚合物層可一層接在一層上地被網版印刷且乾燥以配置所要膜厚度。
在另一具體例中,可如上述地使用具有由疏水性聚合物(諸如聚矽氧)製成而有所要之穿孔形狀之圖形的玻璃板,進行類似澆鑄方法。在一具體例中,當薄的聚合物溶液層被澆鑄至該板上時,在該玻璃及圖形化之聚合物間之表面張力差使該溶液在該玻璃表面上濃縮,且由該圖形化之疏水性聚合物表面拉離。在一具體例中,然後該溶液被乾燥以形成具有與該聚矽氧聚合物相同圖形之孔的固態膜。在一具體例中,也可利用如上述之可交聯預聚物液體進行此方法。
在另一具體例中,使用雙面模製作薄的多孔聚合物膜,其中該聚合物/溶劑溶液被注入該模中,且急冷以固化該溶液。在一具體例中,該模然後被打開且一面被移除,留下該經急冷之溶液於凹穴面中。在一具體例中,該經急冷之溶液面被放置於爐中以乾燥該聚合物溶液且形成
膜10。
參考圖2及11A-11D,在產生膜10之後,膜10被成形且塑造成所要形式。在一具體例中,膜10被成形且塑造以通常符合醫療裝置12之尺寸及形狀。在一些具體例中,膜10被成形且塑造成鞘或袖體32、34、36、38。
參考圖11A-11F,袖體32、34、36、38包括經配置以將膜10形成為鞘之至少一接縫16。在一具體例中,袖體32、34、36、38係藉由在該外圍之周圍貼合第一膜10至第二膜10而形成。在一具體例中,膜10被摺疊且至少部分地固定至其本身。例如,膜10可被成形為圓柱體以接合二相反邊緣。在一具體例中,第二表面10b與第一表面10a重疊以形成接縫16。在一具體例中,第二表面10b與第二表面10b重疊以形成接縫16。在一具體例中,接縫16係藉由加熱膜10之重疊部份且使之再固化而固定。在一具體例中,袖體32、34、36、38之二端仍開放以供醫療裝置12之插入。在一具體例中,一或多端是封閉的。在一具體例中,聚合物片在形成袖體32、34、36、38之前首先形成所要尺寸及形狀。
除了袖體32、34、36、38之外,膜10在一些具體例中可用於其他醫療應用諸如疝修補網(hernia repair mesh)、沾黏阻體(adhesion barrier)、軟組織增強物、過濾薄膜、藥物傳遞膜、骨接合固定(例如在例如脊柱融合術過程中,或在長骨之節段性骨缺損接合中使骨接合保持在合適位置上)、創傷照護產品諸如繃帶。
在一例示的具體例中,植入物係藉由植入羊體內測試。該植入物是金屬板,而具有管狀之薄(0.05-0.08毫米)而透明之聚合物袖體在正要將金屬板外科地插入且連結至骨之前滑動過該金屬板。該袖體牢固的配合,在整體長度上完全的覆蓋該金屬板,雖然袖體在板之二端是開放的。該等袖體係由合成的共聚酯(乙交酯、己內酯、碳酸伸丙酯、乳交酯)配置且整面具有等間隔之直徑1.5毫米之穿孔。一組群之袖體含有1%濃度之三氯沙(2,2,4’-三氯-2’-羥基二苯醚),一組群之袖體含有10%濃度之建它黴素,且一組群之袖體含有三氯沙(1%)及建它黴素(10%)二者之混合物。基於活體內測試選擇建它黴素及三氯沙之濃度以測定每一化合物之治療窗(therapeutic window)。
疏水性三氯沙在該聚合物內係成為完全溶液,此與親水性建它黴素相反的,建它黴素仍以10-20微米小粒子形式懸浮。試管內測試已顯示:因三氯沙之差的水中溶解度,伴隨最小之起初爆發式釋出,其在1至3週期間僅緩慢地由這些膜釋出。
曝於該袖體表面之約50%的更具水溶性的建它黴素在插入後24小時內釋入鄰近組織中。包封在該聚合物深處之其餘的建它黴素更緩慢地溶解且在植入後2至3週期間
被釋出。該聚合物被設計成在手術後60天內經由水解作用降解。
含有或不含有抗菌劑之袖體被證實是生物相容的,對軟組織及骨治癒有最小的影響且對金屬植入物不具腐蝕性。該實驗之另外的細節可在Vet Surg.2012 Jan 12.Biodegradable Sleeves for Metal Implants to Prevent Implant-Associated Infection:An Experimental In Vivo Study in Sheep,von Plocki SC,Armbruster D,Klein K,Kämpf K,Zlinszky K,Hilbe M,Kronen P,Gruskin E,von Rechenberg B.,發現,全其部藉由引用方式併入本文。
在一例示的具體例中,膜10藉由以下方法製造:
建它黴素硫酸鹽粉末之水含量係藉由在乾燥方法時之損失測量。約0.5克之建它黴素在玻璃罐中秤重,然後在真空下加熱至110℃ 3小時且再次秤重。重量損失紀錄為水含量,其用於計算水分百分比。
秤取14.69克之建它黴素硫酸鹽粉末,補償如上所計算之水含量百分率。使用槳式混合將其混合於1升槽內的400克DMSO中。攪拌此混合物30分鐘直至該建它黴素
均勻地散佈。將100克之含有乙醇酸、己內酯、乳酸、及碳酸伸丙酯單體之共聚物添加至該懸浮液,且將該混合槽加熱至65℃。持續混合2小時直至該聚合物完全溶入該溶液中,然後該溶液溫度降至55℃。
使用澆鑄模及由高密度聚乙烯製成之拉引片狀體以由該聚合物溶液澆鑄薄的穿孔膜。使用鹼性清潔劑溶液清潔該澆鑄模及拉引片狀體且將二者裝載於自動化之CNC澆鑄定位器中。將15毫升之該聚合物溶液引上聚丙烯注射器中,該注射器裝載於該澆鑄定位器中。該澆鑄定位器自動地將該溶液分配至該澆鑄模上,且將該片狀體拉引過該模表面。填充聚合物溶液之模放入在85℃下之溶劑乾燥爐中約90分鐘以乾燥該膜。該模由該乾燥爐移除且在2分鐘內將該膜由該模剝除。
使用具有特殊形狀之鑄模的脈衝熱密封壓機以密封且切割該澆鑄膜成袖體形狀。將二澆鑄膜放入該壓機中,然後該壓機用80 psi之壓力關閉且加熱至200℃ 4秒。該袖體由過量之膜材料移除且切成合適長度。密封的袖體可在50℃真空下乾燥且密封於防水包裝物中以防止生物可吸收之聚合物的降解。
精於此技藝者將領會:可對以上所顯示且描述之例示

Claims (30)
- 一種用於藥物釋出之可植入之醫療裝置,其包含:具有第一表面及相對之第二表面的可撓性聚合物膜,該可撓性聚合物膜具有多個由該第一表面延伸至該第二表面的孔及多個由該第一表面所凸出之高起的開口(lip),以致該多孔各自受該多個高起的開口之一者所環繞;其中該膜包含經配置以自該可撓性聚合物膜釋出之一或多種藥物;其中該一或多種藥物包含多個獨立的洗提藥物成分且其中該可撓性聚合物膜被配置以在植入該之醫療裝置後在不同時段洗提該多個獨立的藥物成分;及其中該多個獨立的洗提藥物成分的直徑是在20微米以下。
- 如申請專利範圍第1項之可植入之醫療裝置,其中該可撓性聚合物膜包含生物可吸收之聚合物。
- 如申請專利範圍第2項之可植入之醫療裝置,其中該生物可吸收之聚合物含有選自以下之重複單元:L-乳酸、D-乳酸、L-乳交酯、D-乳交酯、D,L-乳交酯、乙交酯、內酯、內醯胺、碳酸伸丙酯、環碳酸酯、環醚、對-二酮(dioxanone)、β-羥基丁酸、β-羥基丙酸、β-羥基戊酸、及其混合物。
- 如申請專利範圍第2項之可植入之醫療裝置,其中該生物可吸收之聚合物含有選自以下之重複單元:L-乳 酸、D-乳酸、L-乳交酯、D-乳交酯、D,L-乳交酯、ε-己內酯、碳酸伸丙酯、對-二酮、及其混合物。
- 如申請專利範圍第2項之可植入之醫療裝置,其中該生物可吸收之聚合物是乙交酯、碳酸伸丙酯、乳交酯及己內酯的共聚物。
- 如申請專利範圍第1項之可植入之醫療裝置,其中該第一表面包括在該多個高起的開口間延伸的連續平面部份。
- 如申請專利範圍第6項之可植入之醫療裝置,其中該多個高起的開口各自具有比該連續平面部分高約0.1毫米至約1.0毫米的外緣。
- 如申請專利範圍第1項之可植入之醫療裝置,其另外包含至少一個經配置以將該可撓性聚合物膜形成為鞘(sheath)的接縫。
- 如申請專利範圍第1項之可植入之醫療裝置,其中該可撓性聚合物膜在第一平面方向上具有第一抗張強度及在垂直於該第一平面方向的第二平面方向上具有第二抗張強度,其中該第一抗張強度基本上等於該第二抗張強度。
- 如申請專利範圍第1項之可植入之醫療裝置,其中該可撓性聚合物膜具有不大於0.06毫米之標稱厚度。
- 如申請專利範圍第1項之可植入之醫療裝置,其中該第一表面所具有之第一觸感與該第二表面之第二觸感不同。
- 一種製造如申請專利範圍第1項之可植入之醫療裝置之方法,其中該可撓性聚合物膜係藉由下述步驟予以製備:製備含有聚合物、溶劑和藥物之聚合物溶液;將該聚合物溶液放入具有多個由模底部延伸之凸出體的單面模(one sided mold)中,其中該聚合物溶液特徵在於抑制該聚合物無外力流遍該模之黏度;促動該聚合物溶液環繞該多個凸出體之每一者;且固化該聚合物溶液。
- 如申請專利範圍第12項之方法,其中該模包括延伸高度基本上等於該多個凸出體之每一者的高度的圍型體。
- 如申請專利範圍第13項之方法,其中該促動包含拉引促動裝置越過該圍型體及該多個凸出體以趨使該聚合物溶液流過該多個凸出物周圍且流遍該模,以致該聚合物溶液具有基本上均勻之厚度。
- 如申請專利範圍第14項之方法,其中每一凸出體之外表面在該拉引後基本上不含聚合物溶液。
- 如申請專利範圍第13項之方法,其中放入包括將該聚合物溶液沉積在該模中,以致一部分之該聚合物溶液是在圍型體及該凸出體之高度以上。
