Description
本發明係關於一種椎間盤彌補器,有基板、與基板對立之頂板、延伸貫穿基板和頂板中心之中心軸線,以及各具有環形段和二自由端之補數彈簧,配置在包圍中心軸線,其中自由端分別連接基板和頂板,又其中環形段係指向背離彌補器之中心軸線。
例如從US 4,309,777號已知一種椎間盤彌補器,使用金屬彈簧,配置在基板和頂板之間。此種人工椎間盤包括上盤部和下盤部,有複數蝸形彈簧,從上盤部延伸至下盤部,可屈服性逼使盤部彼此分離。
US 6,770,094 B2號記載一種椎間盤彌補器,包括頭盤和尾盤,在被盤簧壓縮時,彼此彈性相偎支持。彈簧為記憶金屬合金組成,在體溫時顯示超彈性之性能。在一具體例中,彈簧係由葉片彈簧形成,呈長條形狀,其一端安裝於上盤,而另一端安裝於下盤。
US 7,201,776 B2號揭示一種人工盤替代品(ADR),包含一對相對立的端板組件,各附設於上、下椎骨之一,有一緩衝組件,介置於端板組件之間;以及充填材料,含於緩衝組件內。在一具體例中,緩衝組件係輪胎狀組件,與裝置中心的金屬轂合作,輪胎內保持空氣、液體、凝膠或其他材料。
商業化的椎間盤彌補器,有一或若干缺點,諸如無軸向阻尼、無轉動控制、不恢復所有自由度,以及較自然椎間盤過份之運動範圍(ROM)。
本發明之目的,在於提供一種椎間盤彌補器,具有改進軸向阻尼,加上轉動控制和屈曲/延伸控制。
此目的是由申請專利範圍第1項之椎間盤彌補器達成。進一步發展列於申請專利範圍附屬項。
本發明椎間盤彌補器達成高度軸向阻尼,利用環形彈簧,其環形部指向背離中心軸線。由於彈簧以其自由端分別連接基板和頂板,可達成轉動控制。屈曲/延伸之運動範圍(ROM)減少大約±5∘,保護周圍關節,尤其是小平面關即。
末端止動部宜由彈料性材料製成,額外限制和阻尼軸向運動。
屈曲性保護軸套防止血管和組織成長進入彈簧間之空間內。
本發明其他特點和優點,凡精於此道之士從以下就本發明具體例參照附圖所述,即可明白。須知本發明可有其他不同的具體例,其細節亦可在諸方面加以修飾。因此,附圖和說明,只視為參考性質,並非僅限於此。
由第1至4圖可見,椎間盤彌補器包括基板1,和基板1對立之頂板2。基板1和頂板2各有板部1a,2a和中央圓筒形凸部1b,2b。基板1和頂板2的配置,使圓筒形凸部1b,2b彼此面對。中心軸線M延伸穿過凸部1b,2b的中心。螺紋腔孔11延伸貫穿與圓筒形凸部1b,2b共軸的基板1和頂板2。板部1a,2a在具體例內呈圓形,但可有任意其他形狀,尤其是適合相鄰椎骨端板輪廓之形狀。板部1a,2a稍微向外彎曲,各板部包括複數齒3,供固定於相鄰椎骨之端板。
基板1和頂板2係生理相容性材料製成,諸如鈦或不銹鋼,或生理相容性塑膠材料,諸如聚芳基醚醚酮(PEEK)。
複數彈簧4配置在基板1和頂部2之間。詳見第3圖,各彈簧4呈實質上平坦長條形狀,類似葉片彈簧,並包括二自由端段5a,5b,和自由端段5a,5b中間之環形彎段6。在較
佳具體例中,彈簧為葉片彈簧。
自由端段5a,5b係筆直平坦段,詳見第1圖。形成彈簧4的長條寬度使複數,例如六個彈簧,可配置包圍中心軸線M,相鄰彈簧間有空間。環形段6最外部份的寬度w最大,朝自由端段5a,5b遞減,見第3圖。彈簧4從自由端段5a,5b至環形段6最外部份之長度l是,當彈簧安裝於環形段6時,完全位在板部1a,2a之間。環形段6沿彌補器中心軸線M平行方向之最大直徑,使得在第1和2圖所示安裝狀態時,於彈簧4和基板1及頂板2之間有距離。
彈簧4可由任何高度屈曲性生理相容性材料製成。例如,彈簧可由顯示超彈性的形狀記憶性材料製成,尤指形狀記憶體金屬,諸如鎳鈦合金,此合金之特例為鎳鈦諾(Nitinol)。
由第1、3、5圖可見,安裝螺釘7係供彈簧安裝於基板1和頂板2間之用。各安裝螺釘7包括有螺紋的螺釘段8及其自由端之凹部9,供旋入工具結合之用。安裝螺釘7自由端對面,包括盤形部10,其直徑相當於圓筒形凸部1b,2b的直徑。各螺釘段8與分別延伸貫穿基板1及其圓筒形凸部1b和貫穿頂板2及其圓筒形凸部2b的共軸螺紋腔孔11結合。螺釘段的長度使得安裝螺釘7分別連接於基板1和頂板2,而彈簧4被盤形部10及圓筒形凸部1b,2b夾持時,凹部9可與旋入工具結合。安裝螺釘7係由類似基板和頂板的生理相容性材料製成。
在安裝螺釘7的盤形部10之間,設有阻尼元件12。阻尼元件以盤形為佳,更好是彈料性材料製成,諸如聚碳酸酯胺酯(PCU)。選用碟形厚度使其充滿安裝螺釘7間之空間。選用材料以達成所需阻尼效應。

椎間盤彌補器經預組裝,其方式是把彈簧4夾在安裝螺釘7的盤形部10和基部1及頂部2的圓筒形凸部1b,2b之間。阻尼元件12可固定於安裝螺釘的盤形部10之自由側,例如以粘著方式。當全部彈簧夾持時,安裝保護軸套。
使用時,把椎間盤彌補器插入二相鄰椎骨之間,以取代椎間盤。基板和頂板外表面之齒部3,結合入相鄰椎骨之端板內。於脊椎屈曲和延伸之際,即對一或若干彈簧施壓或釋壓,加以壓縮或延伸。同時,阻尼元件12阻尼純軸向運動。彈簧4亦提供轉動控制,因為自由端段5a,5b固定,當基板和頂板發生彼此相對扭動時,造成環形段6扭曲至某一程度。
彈簧的彈性和彈簧形狀在屈曲/延伸中限制運動範圍(ROM)至大約±5∘,以保護相鄰結構,諸如小平面關節和韌帶不致超過負荷。實質軸向阻尼(與天然椎間盤相似),限制軸向壓力至生理範圍,並容許人工盤和小平面關節(「鎖定小平面」)之彈性互動。
可以修改。基板和頂部形狀在解剖學上可更為適應椎骨的端板。因此,可呈腎臟形或橢圓形,或其他形狀。齒部可呈圖示以外的形狀。例如,齒部可呈釘形或不對稱形狀。再者,基板1和頂板2的外表面可粗糙化或加塗,以改進生長入端板內。
彈簧的安裝可直接旋入基板1或頂板2的凸部1b,2b,代替夾持。
阻尼元件12可以其他方式實施,例如用水凝膠墊材,或其他類彈簧,諸如蝸形彈簧或盤式彈簧。
阻尼元件12亦可省略,雖然會減少軸向阻尼的優點。此外,保護軸套可以省略。

Claims (11)
- 一種椎間盤彌補器,包括:基板(1);頂板(2),其中心軸線(M)延伸貫穿基板和頂板的中心;至少二彈簧(4),配置在基板和頂板之間,彈簧各有環形段(6)和自由端段(5a,5b);其中自由端之一(5a)連接至基板,另一自由端(5b)連接至頂板;其中環形段在徑向指向背離中心軸線;而且環形段(6)在中心軸線(M)平行方向之最大直徑(d),使彌補器在安裝狀態時,環形段(6)與基板(1)和頂板(2)有一段距離者。
- 如申請專利範圍第1項之椎間盤彌補器,其中彈簧(4)係實質上平坦長條者。
- 如申請專利範圍第1項之椎間盤彌補器,其中設有夾持裝置(7),把自由端段(5a,5b)夾持於基板(1)和頂板(2)者。
- 如申請專利範圍第1至3項之一項椎間盤彌補器,其中基板(1)和頂板(2)各有凸部(1b,2b),與中心軸線共軸,凸部彼此面對者。
- 如申請專利範圍第4項之椎間盤彌補器,其中彈簧之自由端段(5a,5b),夾持於凸部(1b,2b)者。
- 如申請專利範圍第1至3項之一項椎間盤彌補器,其中夾持元件(12)配置在基板和頂板之間,夾持元件以彈料為佳者。
- 如申請專利範圍第1至3項之一項椎間盤彌補器,其中設有複數環形彈簧(4),在中心軸線(M)周圍呈圓形配置者。
- 如申請專利範圍第1至3項之一項椎間盤彌補器,其中彈簧(4)係由顯示超彈性之材料製成,以鎳鈦等形狀記憶性合金為佳者。
- 如申請專利範圍第1至3項之一項椎間盤彌補器,其中屈曲性保護軸套(13)係配置在基板和頂板之間者。
- 如申請專利範圍第1至3項之一項椎間盤彌補器,其中基板和頂板在彈簧(4)相反側,包括結合結構(3),可與椎骨的相鄰端板結合者。
