Claims (1)

Способ пластики дефекта нервного ствола путем выделения трансплантата в виде сосудисто-нервного комплекса из икроножного нерва с малой подкожной веной в одном блоке и перенесения его в дефект нервного ствола с предварительным пересечением вены и икроножного нерва на дистальном и проксимальном уровнях и шва концов нервного ствола с соответствующими концами трансплантата и вены трансплантата с реципиентной артерией, отличающийся тем, что на первом этапе формируют трансплантат, заключая сосудисто-нервный комплекс в середину манжеты, образованной выкроенным из подлежащей собственной фасции голени фасциально-клетчаточным островковым лоскутом с сохранением его связей с икроножным нервом и веной и длиной, равной длине дефекта нервного ствола, пересекают икроножный нерв ниже дистального конца манжеты, а пересеченную на уровне основания пятой плюсневой кости вену анастомозируют с выделенной и пересеченной тыльной артерией стопы, а на втором этапе осуществляют разрез в проекции поврежденного нервного ствола с освобождением из рубцов его концов и в образовавшийся дефект переносят выделенный трансплантат с оставлением на его манжете фасциально-рубцового слоя и сосудистую ножку в виде артериализованной вены с последующим анастомозированием ее дистального конца с одной из магистральных артерий, а проксимального конца - с тыльной подкожной веной оперированной области.A method for plasty of a nerve trunk defect by isolating a transplant in the form of a neurovascular complex from the gastrocnemius nerve with a small saphenous vein in one block and transferring it to the nerve trunk defect with preliminary intersection of the vein and gastrocnemius at the distal and proximal levels and the suture of the ends of the nerve trunk with the corresponding the ends of the graft and veins of the transplant with the recipient artery, characterized in that at the first stage they form a graft, enclosing the neurovascular complex in the middle of the cuff, developed by a fascial-cellular islet flap cut from the underlying fascia of the lower leg with its connections with the gastrocnemius nerve and vein and with a length equal to the length of the nerve trunk defect, the gastrocnemius nerve is cut below the distal end of the cuff, and the vein intersected at the base of the fifth metatarsal bone is anastomosed from and the crossed dorsal artery of the foot, and at the second stage, an incision is made in the projection of the damaged nerve trunk with the release of its ends from the scars and into the resulting defect the selected transplant is transferred with the fascial scar layer and the vascular pedicle in the form of an arterialized vein left on its cuff, followed by anastomosis of its distal end from one of the main arteries, and the proximal end with the posterior saphenous vein of the operated area.
