Abstract

FIELD: medicine; surgery. SUBSTANCE: method for application of anastomosis after wide wedge- shaped resection of the trachea and bronchi involves adaptation of the defect edges differing in length. For adaptation of the walls to be stitched together, the anastomosis angle in the membrane part is displaced along the longer defect edge, and the displacement distance is half the difference between the perimeters of the defect edges. It is easier to make the anastomosis starting from its angle on the cartilage part of the tracheobroncheal wall. U-shapes stitches are applied to be equal in length and equally spaced from one another. As the membrane part is approached, the last stitch on the shorter defect wall limits the non-sutured part located predominantly on the longer defect edge. The anastomosis angle is moved to a point in the middle of this part by way of application of a U-shaped suture 2 mm both sides of this point. EFFECT: simpler method; higher efficacy.

Claims (1)

Предложен способ наложения анастомозов после широкой клиновидной резекции трахеи и магистральных бронхов при разной длине сшиваемых краев образовавшегося дефекта. С целью адаптации сшиваемых стенок, угол формируемого анастомоза в мембранозной части перемещается по более длинному краю дефекта на расстояние, равное половине разницы между периметрами краев дефекта. Техническое выполнение предполагаемого способа облегчается, если анастомоз начинать формировать от его угла на хрящевой части трахео-бронхиальной стенки. П-образные швы накладывают одинаковой ширины и на одинаковом расстоянии друг от друга. При подходе к мембранозной части крайний шов на короткой стенке дефекта ограничивает оставшийся неушитым участок, расположенный преимущественно на длинном краю дефекта (избыток стенки). Угол формируемого анастомоза перемещается в точку на середине этого участка путем наложения П-образного шва, отступая на 2 мм в обе стороны от указанной точки.A method for applying anastomoses after a wide wedge-shaped resection of the trachea and main bronchi with a different length of the stitched edges of the resulting defect is proposed.
