Abstract

FIELD: medicine. ^ SUBSTANCE: invention relates to field of medicine, namely to surgery, and can be applied during application of cosmetic surgical suture. Essence of method lies in the following: when needle is stuck into epidermis, it is turned in direction of operation cavity and passed in derma layer with intake of operation cavity bottom by its opposite side, behind which needle is turned in back direction with intake of derma and brought out into operation cavity at the level of wall third from epithelium. After that, at the same level needle is stuck into opposite wall of operation cavity and through derma and epidermis is stuck out of epithelium indenting from stick-in point in direction of operation cavity edge. After that, operation cavity is compressed and tightened by suture material. The latter is fixed above epithelium with surgeon's knot. After that upper third of operation cavity is covered with epidermis by application of suture material with uninterrupted linear suture. It is possible to stick needle into epidermis at distance 2-3 cm from operation cavity edge. ^ EFFECT: method allows to increase cosmetic effect of suture application. ^ 2 cl, 2 dwg

Известен способ выполнения косметического хирургического шва, включающий вкол иглы с шовным материалом в кожу, после иглу проводят внутридермально параллельно коже на расстояние 2÷5 см от края раны, поворачивают и доводят до дна раны, затем проводят через подкожно-жировую клетчатку с захватом дна раны, а выкол осуществляют через все слои до середины дермы с противоположной стороны в 2÷5 см от края раны, затем иглу поворачивают и выводят внутридермально к месту вкола (Патент RU №2360624 С1. Способ выполнения косметического хирургического шва. - МПК₇: А61В 17/04. - 10.07.2009 г.). Приведенный заявителем анализ уровня техники позволил установить, что аналоги, характеризующиеся совокупностями признаков, тождественными всем признакам заявленного способа выполнения косметического хирургического шва, отсутствуют. Сущность заявленного способа выполнения косметического хирургического шва заключается в следующем.The essence of the claimed method of performing cosmetic surgical suture is as follows.

Сначала выполняют внутридермальный хирургический шов, над которым накладывают непрерывный линейный шов эпидермиса. Для этого вкол иглы (условно не показана) с шовным материалом, например лигатурой 1, в эпидермис 2 выполняют на расстоянии 2÷3 см от края операционной полости 3. После вкола в эпидермис 2 иглу поворачивают в направлении операционной полости 3 и проводят в толще дермы 4 с захватом дна 5 операционной полости 3 за ее противоположную сторону, за которой иглу разворачивают в обратном направлении с захватом дермы 4 и выводят лигатуру 1 в операционную полость 3 на уровне трети его стенки от эпителия 6. Затем на том же уровне иглу вкалывают в противоположную стенку операционной полости 3 и через дерму 4 и эпидермис 2 в направлении первоначального вкола и осуществляют выкол иглы с лигатурой 1 из эпителия 6 на краю операционной полости 3, не доводя до места вкола на расстоянии l (Фиг.1). После этого операционную полость 3 сжимают и стягивают лигатурой 1, последнюю закрепляют над эпителием 6 хирургическим узлом 7. Затем оставшуюся верхнюю треть операционной полости 3 закрывают эпидермисом 2 с наложением непрерывного линейного шва лигатурой 8 по общепринятой методике (Фиг.2).First, an intradermal surgical suture is performed, over which a continuous linear suture of the epidermis is applied. After that, the operating cavity 3 is compressed and tightened by the ligature 1, the latter is fixed over the epithelium 6 by the surgical unit 7. Then, the remaining upper third of the operating cavity 3 is closed with the epidermis 2 with a continuous linear suture by the ligature 8 according to the standard method (Figure 2).

Предложенный способ выполнения косметического хирургического шва использовали в клинике у 38 больных, в том числе у 10 - при лапаротомиях, у 7 - при аппендэктомиях, у 12 - при грыжесечениях и 9 - при удалении поверхностных доброкачественных опухолей. Claims

1. A method of performing a cosmetic surgical suture, including injecting a needle with suture material into the epidermis at the edge of the operating cavity, piercing the dermis with a grasp of the bottom of the operating cavity beyond its opposite side, then turning the needle to puncture through the dermis and epidermis, characterized in that after injection the epidermis the needle is turned in the direction of the operating cavity and is carried out in the thickness of the dermis with the capture of the bottom of the operating cavity for its opposite side, behind which the needle is turned in the opposite direction from behind with a dermal vat and remove into the operating cavity at the level of a third of the wall from the epithelium, then at the same level the needle is pierced into the opposite wall of the operating cavity and through the dermis and epidermis a puncture from the epithelium is carried out with an indent from the injection site towards the edge of the operating cavity, after which squeeze and tighten with suture material, the latter is fixed over the epithelium with a surgical unit, then the upper third of the operating cavity is closed with the epidermis by applying a suture material in a continuous linear stitch.

2. The method according to claim 1, characterized in that the injection of the needle into the epidermis is performed at a distance of 2 ÷ 3 cm from the edge of the operating cavity.
