Abstract

FIELD: medicine.

SUBSTANCE: invention concerns medicine area, namely to gynecology and concerns a non-operative way of treatment of hysteromyoma. For this purpose wearing of loose leaves "Piokal" for 8-15 hours a day within 6-9 months is prescribed to the patient. Biologically active additive to nutrition "Beloyar with calendula extraction" after 1-2 dining rooms of a spoon 3 times a day before meal, a course of 10-30 days is simultaneously entered. In the presence of cardiovascular system diseases it is entered "Beloyar with hawthorn extraction", and in case of presence of an allergy and the lowered immunity - "Beloyar with licorice extraction". In addition "Chitovit" is entered on 1-2 tablet 3 times a day within 10-15 days. From the first day of a menstrual cycle 2-3 times a day the "Migs" are prescribed within 15 days on1 table-spoon. DS "Dobrodeya" is entered into second half of an ovarian-menstrual cycle, 30 minutes prior to food intake 2-3 times a day within 15 days - two cycles successively for women that are younger than 40 years, and for women that are older than 40 years or in the presence of an accompanying pathology - three cycles successively. The invention relates to medicine, namely to gynecology, and relates to a complex non-surgical method for the treatment of patients with uterine myoma, taking into account a new perspective on etiopathogenesis.

The importance of finding conservative methods for treating uterine fibroids can be judged by the frequency of this disease. Based on clinical studies, uterine fibroids occurs in the age of up to 35 years in the range of 20-25%, after 35 years - 25-30%. Autopsy studies make significant changes in the epidemiological data regarding uterine fibroids, the incidence reaches 85% (Tikhomirov A.L., Lubnin D.M. New non-invasive approaches to organ-preserving treatment of uterine fibroids. - Materials of the 9th All-Russian Forum "Mother and Child" . - 2007. - P.537-538).

Such a frequency of development of this pathology cannot be explained by any of the available theories. Our view on the etiopathogenesis of uterine fibroids is quite suitable for explaining such a frequency of the disease.

The existing method of surgical treatment of uterine fibroids does not always coincide with the desire of a woman. The desire of a woman to save the uterus at all costs is difficult to explain, not all women are able to give a clear, definite answer. For many women, the presence of the uterus is a subconscious element of their femininity, their involvement in the female sex. In other women, removal of the uterus causes fear, fear of subsequently encountering problems in an intimate life. It is known that in some women, during the orgasm, the uterus contracts, which enhances the severity of the experienced sensations.

Conservative methods of treating patients with uterine myoma are diverse, but do not always give the desired effect and are not recommended in the following conditions:

- with the size of the uterus with nodes over 12 weeks of gestation;

- a combination of uterine fibroids with ovarian tumors and adenomyosis;

- rapid tumor growth, causing suspicion of uterine sarcoma;

- menorrhagia causing anemia;

- submucous node location.

Often, for the conservative treatment of uterine fibroids, hormonal drugs are used, but they also have a number of contraindications:

- history of thromboembolism and thrombophlebitis;

- phlebeurysm;

- hypertension;

- history of malignant tumors.

The goal of non-surgical treatment of a tumor is to inhibit tumor growth and, in the best case, its subinvolution.

A known method of treating uterine fibroids by herbal medicine (RF patent No. 2145232 of the Russian Federation, 7 A61K 35/78 C1, 2000).

The prototype of the invention is a comprehensive method for the non-surgical treatment of uterine fibroids (RF patent 2102079, class A61K 35/78, 1998), which is used for all age groups. The essence of this method lies in a specially developed treatment regimen, accompanied by a selected method of nutrition and taking collections of medicinal herbs and their components on the background of a separate-sequential diet, consisting in taking products depending on the speed of their digestibility, in the morning - quickly digestible, in the afternoon - slowly digestible, in the evening - even more slowly assimilated. At the same time, patients take collections of medicinal plant materials that have antioxidant effects throughout the course of treatment until the stage of therapeutic fasting. Duration of treatment is up to 3 months. Then, the gastrointestinal tract is cleansed while taking preparations from plant materials of the Enrich international company containing fiber and soft laxative tea. Then the liver is cleaned and the antimutagenic drug collection is taken against the background of therapeutic fasting. This method is mild and gentle, can be repeated several times, however, the therapeutic effect of the use of these funds is low.

We believe that the existing methods of treatment are not effective enough, since the etiopathogenesis of uterine fibroids is not taken into account, therefore, relapses are so frequent after both surgical and non-surgical methods of treatment.

The objective of the invention is to develop a method for the treatment of uterine fibroids with a new perspective on the etiopathogenesis of the disease, which allows to cure or significantly reduce the size of uterine fibroids in women of all age groups.

EFFECT: elimination of chronic hypoxia of the brain, normalization of microcirculation in the body and, in particular, the brain, normalization of the function of the hypothalamic-pituitary-ovarian system, prevention of relapse of the disease.

Based on ten-year studies of patients with uterine fibroids (under the age of only 35 years old), we concluded that the cause of its occurrence is chronic hypoxia of the brain against prolonged intracranial hypertension and mild circulatory disorders of the brain.

The sequence of global pathological changes in the brain, and subsequently in the body of a woman, leading eventually to the growth of uterine fibroids, is as follows:

- moderate or severe fetal hypoxia that occurs during pregnancy and / or delivery of the mother;

- intraventricular and other localization of hemorrhage, severe cerebral ischemia, thrombohemorrhagic syndrome, distress syndrome;

- gliosis of the brain, primarily in places of hemorrhage;

- in the end: prolonged intracranial hypertension;

- progression of brain hypoxia;

- neurotic disorders, autonomic dysfunction;

- high infectious index;

- DMK juvenile age and the progression of neurotic syndrome;

- DMK of reproductive age, polycystic ovary;

- uterine fibroids.

The chronicle of the occurrence of prolonged hypoxia of the brain starts initially in most women from the moment complications during pregnancy occur in their mothers and / or from complications in childbirth.

If the history does not have data on the occurrence of hypoxia during pregnancy or childbirth, then the fact of a skull injury in childhood (falling from a swing, window sill, into the underfloor, cellar, etc.) or already in an adult state (falling during the icy back of the head about ice with loss of consciousness, car accidents, etc.). The examination was carried out in patients with uterine fibroids only up to 35 years old in order to have complete anamnestic data not only from the patient, but also an anamnesis from their mothers. Another equally important factor of the examination, only until the age of 35, was the desire to understand the cause of a benign tumor even at such a young age, it was assumed that for this there should be some “extraordinary stressful moment”.

For the first time, fetal brain hypoxia can occur when complications occur during pregnancy:

- with the threat of abortion;

- complication of pregnancy with late gestosis, especially of a severe degree;

- chronic fetoplacental insufficiency;

- pregnancy overtaking;

- with placental abruption, when acute hypoxia occurs as a result of bleeding (occurs within 2-11% of cases).

Hypoxia that occurred during pregnancy can continue during childbirth or occur for the first time in the following pathological conditions:

- Anomalies of labor;

- fast, rapid childbirth, especially with premature pregnancy;

- postponed pregnancy;

- tight cord entanglement of the umbilical cord around the neck, in the case of childbirth through the natural birth canal;

- very early and early preterm birth;

- birth of a large fetus.

Mild hypoxia and moderate severity, lasting a short time, are compensated by the fetus due to its reserve capabilities. Prolonged and severe hypoxia lead to a sharp inhibition of adrenal function. A drop in the level of cortisol and catecholamines indicates a sharp decrease and even depletion of compensatory reserves in the fetus at both the humoral and cellular levels. It is accompanied by disturbances in electrolyte balance, manifested by hyperkalemia and hyponatremia. An imbalance of electrolytes leads to a violation of neuromuscular conduction, the activity of the heart.

It is known that the brain (especially the cells of the cerebral cortex) is most sensitive to oxygen deficiency and is energy dependent on glucose, the source of ATP, which is so necessary for brain cells to maintain their functions and maintain ionic homeostasis. Compensatory capabilities of the fetus are not unlimited. They are clearly not enough for the fetus with severe severe hypoxia and acute hypoxia, which occurred during the total detachment of a normally located placenta or placenta previa. The oxygen supply in the fetus is quickly depleted in such cases, and low-energy anaerobic glycolysis enters into energy production. Its disadvantages are:

Our point of view on the etiopathogenesis of uterine fibroids does not contradict the existing views: uterine fibroids is a dishormonal tumor with disorders in the system of the hypothalamus-pituitary-cortex of the adrenal gland-ovaries. Hypothalamic-pituitary disorders can precede the onset of a neoplasm (V.I. Kulakov, N.D. Selezneva, V.I. Krasnopolsky. Operational gynecology. - N. Novgorod: Publishing House of the Novosibirsk State Medical Academy, 1998, - 504 p. (167 p.) ), and a feature of regional blood circulation is the development of congestion in the vessels of the brain. These changes are persistent and are not eliminated in the near future after surgery (removal of uterine fibroids).

In our opinion, congestion in the brain after amputation of the myomatous uterus is not only not eliminated in the near future, but continues to progress, as the consequence is deleted, and not the cause of the tumor.

Thus, we believe that one important link has been missed here, the cause of congestion in the vessels of the brain is the changes that occurred in the fetus from the side of the brain in the event of complications during pregnancy, childbirth or already in childhood, these are hemorrhages in the brain of varying severity and different localization, which entailed not only functional disorders, but also organic, in the form of tissue gliosis, pronounced intracranial hypertension, decreased blood circulation in olovnom brain and chronic cerebral hypoxia.

The proposed complex non-surgical method for the treatment of uterine fibroids, taking into account a new view on etiopathogenesis, is carried out as follows:

the patient is prescribed to wear Piokal inserts for 8-15 hours a day for 6-9 months. At the same time, the gastrointestinal tract is cleaned with herbal and / or animal-derived drugs: from herbal preparations, the biologically active food supplement “BELOYAR with CALENDULA EXTRACT” is prescribed 1-2 tablespoons 3 times a day before meals, for 10-30 days. If there is a disease on the part of the cardiovascular system, “Beloyar with Hawthorn EXTRACT” is prescribed, and if a woman has allergies and reduced immunity, “Beloyar with MALTIC EXTRACT”. Example 1. Patient P-a, 26 years old, complained about the absence of pregnancy for a year and a half of regular sexual activity. In connection with the transferred viral hepatitis in childhood, the patient is offered the described complex of treatment for uterine fibroids. From the very first days of treatment, “Pyokaly” wore 6-13 hours a day, Beloyar was prescribed with calendula - 1 tablespoon 3 times a day, washed down with plenty of water (at least 200 ml), then hitovit - 15 days, 2 tablets 3 times a day. After cleaning the gastrointestinal tract with the arrival of menstruation - MIGI 1 tablespoon for 15 days (the patient has a 30 day cycle) and then Dobrodey for 15 days, carried out for 2 menstrual cycles. After a month's break, she again accepted MIGI and Dobrodey - 2 cycles. During the break - Lohelan Ѕ teaspoon 3-4 times a day for 15 days. Pyokaly wore for 1 year and 3 months (2 pairs).

The patient was under control with an additional examination after 3, 6 and 9 months.
