Claims (1)

Способ анестезиологического обеспечения вертебрологических операций высокого риска путем комбинированной анестезии (эпидуральная анестезия в условиях интубационного способа ИВЛ), отличающийся тем, что после проведения премедикации диазепамом (0,15 мг/кг) и димедролом (0,3 мг/кг) устанавливают эпидуральный катетер, проводят эпидуральную анестезию 0,75% ропивакаином (20-25 мл), вводный наркоз проводят атропином (0,007-0,01 мг/кг) и фентанилом (1,4-2 мкг/кг), индукцию осуществляют введением пропофола (в дозе 2-2,5 мг/кг), далее вводят дитилин в дозе 2 мг/кг, после наступления адекватной миоплегии производят ларингоскопию и интубацию трахеи, дальнейшую седацию и миоплегию интраоперационно достигают непрерывной инфузией пропофола (8-10 мг/кг/ч) и атракуриума (0,5-0,6 мг/кг/ч) с искусственной вентиляцией легких через эндотрахеальную трубку в режиме IPPV с ДО=60-80 мл/кг и МОД=80-100 мл/кг, FiO250% под контролем SpO2 и EtCO2.The method of anesthetic management of vertebrological operations of high risk by combined anesthesia (epidural anesthesia under the endotracheal ventilation method), characterized in that after premedication with diazepam (0.15 mg / kg) and diphenhydramine (0.3 mg / kg) establish an epidural catheter, they carry out epidural anesthesia with 0.75% ropivacaine (20-25 ml), induction anesthesia is performed with atropine (0.007-0.01 mg / kg) and fentanyl (1.4-2 μg / kg), induction is carried out by administration of propofol (at a dose of 2 -2.5 mg / kg), then dithylin is administered at a dose of 2 mg / kg, pic After adequate myoplegia occurs, laryngoscopy and tracheal intubation are performed, further sedation and myoplegia are intraoperatively achieved by continuous infusion of propofol (8-10 mg / kg / h) and atracurium (0.5-0.6 mg / kg / h) with mechanical ventilation through endotracheal tube in IPPV mode with DO = 60-80 ml / kg and MOD = 80-100 ml / kg, 50% FiO 2 under the control of SpO 2 and EtCO 2 .
