Abstract

시술 대상인 뼈에 삽입되어 인공 관절로 사용되는 인공 관절 장치는 금속 재질로 이루어지고, 시술 대상인 뼈에서 삽입될 공간에 대체로 대응하는 형태를 갖는 주대 및 이 주대와 일체로 형성되는 두부를 구비하는 몸체와 플라스틱 재질로 이루어지고, 몸체의 주대의 외면을 둘러싸는 자켓을 포함한다. 자켓은 골 친화제를 포함하는 외층과 실질적으로 플라스틱으로만 이루어진 내층을 구비한다.
인공 관절 장치는 골 친화제를 구비하기 때문에 접촉하는 뼈의 성장을 촉진할 수 있으며, 골 친화제가 용해된 공간으로 뼈가 성장해 들어가 자켓과 뼈가 단단히 결합될 수 있다. 또한, 골 친화제가 자켓의 내부에도 존재하기 때문에, 지속적으로 뼈의 내성장을 촉진시킬 수 있다.

Description

인공 관절 장치{ARTIFICIAL JOINT SYSTEM}
도 1은 본 발명의 일실시예에 따른 인공 관절 장치를 개략적으로 도시한 도면이다.
도 2는 본 발명의 일실시예에 따른 인공 관절 장치의 플라스틱 자켓의 사시도이다.
도 3은 도 2에 도시된 플라스틱 자켓의 단면을 도시한 확대도이다.
도 4는 본 발명의 일실시예에 따른 인공 관절 장치가 대퇴골에 배치된 상태를 도시하는 도면이다.
본 발명은 인공 관절 장치에 관한 것으로서, 더 구체적으로는, 골 친화제가 포함된 플라스틱 소재의 자켓을 구비하는 인공 관절 장치에 관한 것이다.
인공 관절, 예를 들어, 인공고관절 수술에서 사용되는 인공 관절은 골반에 삽입되어 비구를 대체하는 부분과 대퇴골에 삽입되어 골두를 대체하는 부분으로 이루어져 있다. 이러한 인공 관절은 주로 금속, 세라믹, 플라스틱 등의 소재로 제작 된다.
이러한 인공 관절 중에서 대퇴골에 삽입되는 부분은 골두에 해당하는 부분과 대퇴골에 삽입되는 부분인 주대로 이루어지는 것이 일반적이다. 이를 시술하는 경우, 대퇴골의 골내강을 일부 제거하여 적절한 공간을 형성한 후에, 이 공간에 적당한 양의 시멘트를 채워 넣고 인공 관절의 주대를 삽입한다. 시멘트는 대퇴골에 인공 관절을 접착시키는 역할을 한다. 한편, 인공 관절의 주대의 표면에는 시멘트와 잘 접착되도록 동일한 재질의 시멘트가 코팅되어 있다. 그러나, 시멘트를 이용하여 인공 관절을 접착시키는 경우, 결합력이 약하기 때문에 인공 관절의 수명이 길지 않다는 문제점이 있었다.
시멘트를 사용하지 않는 다른 종류의 인공 관절로서, 인공 관절의 주대에 다공질의 플라스틱 자켓을 도포한 것이 있다. 표면에 다공질의 플라스틱층이 형성된 주대를 대퇴골에 삽입하면, 대퇴골이 점진적으로 성장하면서 다공질의 플라스틱 표면과 생리학적으로 결합되어 주대가 대퇴골에 고정된다. 그러나, 이러한 종류의 인공 관절을 사용하는 경우, 대퇴골과 주대의 플라스틱층 사이의 접착면에 응력이 집중되어 접착면이 부분적으로 분리되고, 분리되어 형성된 틈새로 이물질이 침투하여 대퇴골의 융해(osteolysis)를 촉진시키는 문제점이 있었다. 또한, 플라스틱은 뼈의 성장을 유도하는 작용이 없기 때문에, 인공 관절이 대퇴골에 고정되는데 상당한 시간이 소요되는 문제점이 있었다.
대한민국 특허공개 특2003-62812호에는 주대에 다공질의 플라스틱 자켓이 마련된 인공 관절이 개시되어 있다. 이 인공 관절의 플라스틱 자켓의 표면에는 결합 촉진제가 도포되어 있다. 결합 촉진제로는 수산화 인회석 또는 제3 인산칼슘이 사용된다. 이러한 결합 촉진제는 플라스틱 자켓과 대퇴골의 접착을 촉진하여 인공 관절이 신속하게 고정될 수 있다. 그러나, 전술한 종래기술과 마찬가지로 플라스틱 소재가 갖는 다공질 특성을 이용하여 플라스틱 자켓과 대퇴골을 접착시키기 때문에, 반복되는 하중에 의하여 주대의 플라스틱 자켓의 결합 촉진제가 이탈되어 자켓과 대퇴골 사이의 접착면이 분리되고 이물질이 침투하여 대퇴골이 융해될 우려가 있었다.
본 발명은 전술한 종래기술이 갖는 문제점을 해결하기 위한 것으로서, 별도의 접착제를 사용하지 않으면서 플라스틱 자켓과 대퇴골의 접착 강도를 지속적으로 증가시켜 인공 관절의 수명을 늘리는 것을 목적으로 한다.
본 발명에 따라, 시술 대상인 뼈에 삽입되어 인공 관절로 사용되는 인공 관절 장치로서, 금속 재질로 이루어지고, 시술 대상인 뼈에서 삽입될 공간에 대체로 대응하는 형태를 갖는 주대 및 이 주대와 일체로 형성되는 두부를 구비하는 몸체; 및 플라스틱 재질로 이루어지고, 상기 몸체의 주대의 외면을 둘러싸는 자켓을 포함하며, 상기 자켓은 골 친화제를 포함하는 외층과 실질적으로 플라스틱으로만 이루어진 내층을 구비하는 인공 관절 장치가 제공된다.
바람직하게는, 상기 골 친화제로는 능동적 친화제로서 수산화 인회석, 제3 인산칼슘을 포함하는 다종의 인산칼슘 또는 생물의 뼈가루가 사용된다. 또한, 자켓에는 능동적 골 친화제 외에 수동적 골 친화제로서 인체에 무해한 세라믹 또는 금 속재 입자가 더 포함될 수 있다.
상기 몸체의 주대를 형성하는 소재는 스테인레스, 티타늄 및 이들의 합금으로 이루어진 그룹에서 선택된 금속인 것인 것이 좋다. 또한, 상기 몸체의 주대를 형성하는 소재로서 보강된 세라믹 또는 탄소섬유가 사용될 수 있다.
이하에서는 본 발명의 바람직한 실시예를 첨부된 도면을 참조로 하여 상세하게 설명한다.
도 1은 본 발명의 일실시예에 따른 인공 관절 장치를 개략적으로 도시한 도면이다.
도 1에 도시된 바와 같이, 인공 관절 장치(100)는 시술 대상인 뼈에 삽입되는 주대(110)와 골두 형상을 갖는 두부(112)를 구비하는 몸체와 주대(110)의 외면을 둘러 싸는 자켓(120)을 포함한다. 주대(110)는 금속 소재로 이루어져 있으며, 시술 대상인 뼈에 미리 마련된 삽입 공간에 대체로 대응하는 형상을 갖는다. 자켓(120)은 플라스틱 소재로 이루어져 있으며, 입자 형태의 골 친화제를 구비한다.
인공 관절 장치의 몸체가 구비하는 주대(110)는, 도 4에 도시된 바와 같이, 시술 대상인 뼈(200)의 내부에 마련되는 공간에 삽입되는 부분으로서 가늘고 긴 형상을 갖는다. 주대(110)가 삽입되는 공간은 시술 대상인 뼈(200)의 골내강을 깊게 파내어 형성된다. 주대(110)는 금속 소재로 이루어지며, 예를 들어, 높은 강성을 갖는 코발트-크롬 합금이 사용될 수 있다. 바람직하게는, 주대(110)는 스테인레스, 티타늄, 또는 이들의 합금으로 이루어질 수 있다. 이들 소재는 인체에 무해하다는 것이 알려져 있으며, 코발트-크롬 합금에 비하여 저렴하기 때문에 인공 관절을 시 술하는 비용을 줄일 수 있다. 또한, 주대의 소재는 반드시 금속에 한정되는 것은 아니며, 세라믹 소재나 탄소 섬유 등과 같은 비금속 소재도 사용될 수 있다. 주대(110)에는 골두 형상을 갖는 두부(112)가 일체형으로 연결된다. 두부의 형상은 대응하는 인공 비구컵의 형상과 대응하도록 형성된다. 본 실시예에서 두부는 반구형인 형상을 갖는다. 주대(110)와 두부(112)는 목 부분으로 연결된다.
주대를 둘러싸는 자켓(120)은 플라스틱 소재로 이루어진다. 인공 관절이 시술 대상인 뼈에 삽입되는 경우, 자켓(120)의 외면과 뼈의 내면이 직접 접촉한다. 자켓(120)은 입자 형태의 골 친화제가 구비되어 있다. 골 친화제로는 수산화 인회석, 제3 인산 칼슘, 동물의 뼈 가루, 기타 뼈의 성장을 촉진하는 특성을 갖는 모든 종류의 인산 칼슘이 사용될 수 있다. 또한, 세라믹 입자, 금속 입자 등과 같이 뼈의 성장을 적극적으로 촉진하는 것은 아니지만 뼈의 내성장(in-growth)이 증명되었고 플라스틱 자켓(120)의 마찰과 표면적을 넓혀서 자켓(120)이 뼈에 신속하고 단단하게 고정되도록 보조하는 입자들도 사용될 수 있다. 이들 입자는 자켓의 표면에만 존재하는 것이 아니라, 후술하는 바와 같이 자켓의 내부에도 존재하는 것이 좋다.
도 2에는 부분적으로 절개된 단면을 보여주는 자켓(120)이 도시되어 있으며, 도 3은 도 2의 원 A로 표시된 부분을 확대하여 나타낸 것이다. 도시된 바와 같이, 플라스틱 자켓(120)은 상부가 열린 주머니 형태를 갖는다. 플라스틱 자켓에 구비된 골 친화제 입자(130)는 자켓의 표면과 내부에 존재한다. 시간이 경과하면 골 친화제 입자는 용해되어 뼈에 흡수되는데, 이 때 골 친화제가 있던 공간으로 뼈가 성장해 들어가기 때문에 자켓이 단단하게 뼈에 고정될 수 있다. 또한, 골 친화제 입자 가 자켓(120)의 표면에만 분포되는 것이 아니라 그 내부에도 분포되기 때문에, 표면에 분포된 골 친화제 입자가 뼈에 흡수된 후에도 지속적으로 뼈의 내성장을 촉진시킬 수 있다.
바람직하게는, 골 친화제 입자(130)는 자켓의 외측 표면에 가까운 부분에 많이 분포되고, 내측 표면에 가까운 부분에는 상대적으로 적게 분포된다. 이런 방식으로 입자를 배열하는 경우 플라스틱 자켓(120)의 강도를 적절하게 유지할 수 있다. 예를 들어, 골 친화제가 혼합된 플라스틱 재료로 자켓의 내층을 먼저 형성한 후에 골 친화제가 혼합된 플라스틱 재료로 자켓의 외층을 형성하면 도 3에 도시된 바와 같이 골 친화제가 분포된 플라스틱 자켓(120)을 제조할 수 있다.
도 4에는 본 발명에 따른 인공 관절의 사용 상태가 도시되어 있다. 시술 대상이 되는 뼈(200)는 미리 골내강이 제거되어 인공 관절이 삽입될 공간이 마련된다. 이렇게 마련된 공간에 인공 관절(100)이 삽입된다. 시멘트와 같은 접착제는 사용되지 않는다. 삽입되는 부분은 주로 플라스틱 자켓(120)이 둘러싸는 주대(110) 부분이며 뼈의 외부로 골두의 역할을 하는 두부(112)가 돌출된다. 플라스틱 자켓(120)의 표면 및 내부에는 골 친화제가 마련되어 있기 때문에 시멘트를 사용하지 않더라도 인공 관절과 뼈가 단단하게 접착될 수 있다.
이상에서 본 발명이 바람직한 실시예를 참조로 하여 설명되었지만, 당해 기술분야에서 통상의 지식을 갖는 자라면 후술하는 청구범위를 벗어나지 않는 범위 내에서 다양하게 변경 또는 수정하여 실시할 수 있을 것이다.
위에서 설명한 바와 같이, 본 발명에 따른 인공 관절 장치의 플라스틱 자켓은 골 친화제를 구비하기 때문에 접촉하는 뼈의 성장을 촉진할 수 있으며, 골 친화제가 용해된 공간으로 뼈가 성장해 들어가 자켓과 뼈가 단단히 결합될 수 있다. 또한, 골 친화제가 자켓의 내부에도 존재하기 때문에, 지속적으로 뼈의 내성장을 촉진시킬 수 있다.

Claims (6)

  1. 시술 대상인 뼈에 삽입되어 인공 관절로 사용되는 인공 관절 장치로서,
    금속 재질로 이루어지고, 시술 대상인 뼈에서 삽입될 공간에 대체로 대응하는 형태를 갖는 주대 및 이 주대와 일체로 형성되는 두부를 구비하는 몸체; 및
    플라스틱 재질로 이루어지고, 상기 몸체의 주대의 외면을 둘러싸는 자켓을 포함하며,
    상기 자켓은 골 친화제를 구비하고, 골 친화제는 자켓의 외면 및 내부에 분포되어 있는 인공 관절 장치.
  2. 청구항 1에 있어서,
    상기 자켓이 구비하는 골 친화제는 자켓의 외면 및 외면에 근접하는 내부 영역에 분포되며 자켓의 내면에 근접하는 내부 영역에는 분포되지 않는 것을 특징으로 하는 인공 관절 장치.
  3. 청구항 1에 있어서,
    상기 골 친화제는 수산화 인회석, 제3 인산칼슘, 생물의 뼈가루로 이루어진 그룹에서 선택된 재료인 것을 특징으로 하는 인공 관절 장치.
  4. 청구항 1에 있어서,
    상기 자켓은 인체에 무해한 세라믹 또는 금속재 입자를 더 구비하는 것을 특징으로 하는 인공 관절 장치.
  5. 청구항 1에 있어서,
    상기 몸체의 주대를 형성하는 소재는 스테인레스, 티타늄 및 이들의 합금으로 이루어진 그룹에서 선택된 금속인 것을 특징으로 하는 인공 관절 장치.
  6. 청구항 1에 있어서,
    상기 몸체의 주대를 형성하는 소재는 보강된 세라믹 또는 탄소섬유인 것을 특징으로 하는 인공 관절 장치.
